It appears you r suffering from sciatica. Pl visit a good acupuncturist in your place for treatment. Meanwhile try this. Identify the dipping portion on the back of your knee folding. Press this dip with both middle fingers and hold your knee with your thumbs for support. Please swing 20 times morning and evenings. This will relieve your problem. Press the point on the web between 2nd and 3rd finger from toe in the foot 20 times by pressing and releasing with a blunt edge of a pen. Further rotate your foot clockwise and anticlockwise 10 times. Massage your leg with both hands. Pl consult after a week.

Hi Thanks for your query and Welcome to Lybrate. I am Dr Akshay from Fortis Hospital, New Delhi. You have a history of pain in your knees since 2 days. Can you tell me whether you had any twisting injury to your knee region? Also you have not mentioned whether you have pain, swelling, any instability etc in the affected knee? You can start with activity modification, ice packs and gentle anti inflammatory medication initially and let me know in 2-3 days time how you are feeling? Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation. Keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.

Hello. I have gone through your question about knee pain. I understand your concern. I feel that these symptoms are because of your weight. You will have to reduce your weight. Atleast 15 kg over 2 years period. You need diet modification. Stop oily food. Don't eat rice at night. Very low calories diet at night atleast 2 hours prior to sleeping. Take vitamin B12 medicines. Due to muscle spasm. Lumbar vertebrae appears normal in alignment .No listhesis. Small Schmorl's nodes noted involving the endplates of multiple lumbar vertebra. Degeneration ofL1-L2, L2-L3 and L4-L5 lumbar intervertebral disc Posterior annular tear noted at L1-L2, L2-L3 and L4-LSlevels. At Ll-L2 level there is right paramidline protrusion, No foramina narrowing. At L2-L3, L3-L4 diffuse posterior disc bulge noted ,causing effacement of ventral thecal sac and inferior recess of bilateral neural foramina narrowing at L3-L4 level. At L4-L5 level, posterocentral and left forminal protrusion causing, significant canal stenosis and left neural foramina narrowing ,resulting in compression of exiting L4 and traversing LS nerve roots at this level. At LS-Sllevel, no disc bulge or herniation. Conus medullaris appear unremarkable. Impression: - Lumbar spondylosis degenerative changes as mentioned ,predominant at L4-L5 level. - At L4-LSlevel, posterocentral and left forminal protrusion causing, significant canal stenosis and left neural foramina narrowing ,resulting in compression of exiting left L4 and traversing Left LSnerve roots at this level. TECHNIQUE: The :rv1RsItudy of the lumbosacral spine was obtained by sagittal Tl and T2 weighted images and axial Tl and T2 weighted images through T121L1 to LS/S1 disc spaces. FINDINGS: The MRI study of the lumbosacral spine disclosed evidence of degeneration of lumbar intervertebral disc spaces depicted by low signal intensity on T2 and reduced height at T121L1, Ll/2, L2/3 and L4/S disc spaces. There is mild dorsolumbar kyphosis at L1I2. The spinal canal is of normal satisfactory caliber. The conus is of normal shape and normal signal. The L 112disclosed posterior right paracentral broad based disc protrusion. The L2/3 disclosed posterior left paracentral broad based disc protrusion with focal high signal intensity consistent with focal tear in the annulus fibrosus at posterior left paracentral region. The L4/5 disc space disclosed posterior central broad based disc protrusion with left predominance causing significant compression on the anterior surface of the thecal sac at this level and associated with narrowing of lateral recesses bilaterally more pronounced on the left side with compromising of the exiting left L5 nerve root. CONCLUSION: The MRl examination of the lumbosacral spine disclosed degenerative changes of lumbar intervertebral disc spaces more pronounced on T 121L1, L 112 and L4/5 with mild dorsolumbar kyphosis at L 112 and posterior right paracentral broad based disc protrusion at L 112, posterior left paracentral broad based disc protrusion at L2/3 and posterior central broad based disc protrusion at L4/5. Sir you are having left Lowe limb radiating pain along with tingling and Numbness, you can't walk for long because of leg pain. This is all because of nerve compression by disc material. As per the reports compression is significant but I can comment with confidence after seeing the mri films. Two options are there in your case if pressure over nerve is moderate we can try pain block injection or if pressure over nerve is severe then surgery is the only choice left. But we can decide the treatment modality after seeing the mri films only. Just to inform you now a days technology is so advance that hardly it required more then three stitches after the surgery. No bed rest you can go home next day your own. We are using minimal invasive techniques like endoscopic and microscopic surgery for such type of cases with excellent results.

All About Rheumatoid Arthritis

Rheumatoid arthritis is an auto-immune disorder which causes abnormal swelling of the body joints due to a sudden attack to the joints by the immune system of the body. This disorder is fairly common in the body parts that are more frequently used such as the knees, fingers, shoulders, wrists, elbows and the hips. Although rare, this disorder can also affect the ligaments or the tendons along with kidney, lungs, eyes or the heart.

This disorder tends to be chronic. Although the exact cause of the disease is still unknown, the loss of immunity generally leads to the disorder. Thus people who are obese, regular smokers, or who have had a family history of this condition are the most vulnerable to this disorder.

Symptoms

The symptoms of Rheumatoid arthritis are:

  1. Severe pain in the joints.
  2. Severe pain coupled with high fever.
  3. Stiffening and inflammation of the joints, resulting in intense pain.
  4. Exhaustion, fatigue and sudden weight loss.
  5. The pain gets aggravated in cold weather conditions.

Treatments

If rheumatoid arthritis is not treated immediately after diagnosis, it might cause further complications in the future such as lymphatic blood cancers, osteoporosis (a medical condition resulting in brittle and weak bones), blockage of the arteries and dry mouth syndrome (Xerostomia).

The treatment of rheumatoid arthritis includes:

  1. Prescribed dosage of non-steroidal anti-inflammatory drugs such as Ibuprofen and Naproxen, anti-rheumatoid drugs such as Methotrexate and steroidal drugs such as Prednisone help in reducing inflammation and pain. The dosage depends on the intensity of the condition.
  2. Performing regular exercises which will help you retain flexibility of the joints and subsequently cure rheumatoid arthritis.
  3. In extreme cases, the doctor might suggest surgeries such as:
    • Synovectomy: A surgery that removes the damaged portion of the joint
    • Joint fusion: This option will realign the joint and reduce the pain If you wish to discuss about any specific problem, you can consult a internal specialist.
