All About Knee Replacement

Knee replacement is a surgery wherein an artificial joint is used to replace a diseased, damaged or worn out knee. This surgery is common among people who fall in the age group of 60-80, but recent trends seem to suggest that younger people are opting for this surgery as well. The lifetime of the artificial knee joint is around 20 years, provided the knee is well cared for.

Why do you need Knee Replacement?

Most common reason is “high grade osteoarthritis” due to wear and tear of the knee joint. The pre-hospital study of the Knee joint is mandatory and would decide what kind of Replacement is suitable to the patient. If there is diabetes or hypertension associated with this, then it should be controlled well before undergoing surgery. Hemoglobin of at least 10 gm% is required.

If the mobility in your knee joint is reduced leading to impaired functioning of the knee joint, then you might need a knee replacement surgery. You may experience pain while walking, sitting and, in some cases, resting as well.

Some of the common reasons why you may opt for this particular surgery are:

  1. Gout, where, small crystals are formed inside the joint.
  2. Rheumatoid Arthritis, an autoimmune disorder, wherein the immune system of the body attacks the body’s healthy tissues.
  3. Hemophilia, wherein, the blood ceases to clot normally.
  4. Injuries to the knee
  5. Disorders that cause unusual bone growth (bone dysplasias)
  6. Death of bone in the knee joint following blood supply problems (avascular necrosis)
  7. Knee deformity with pain and loss of cartilage
  8. Unusual growth of bones in the knee joint

Knee replacement surgery is classified into:

1. Partial Knee Replacement: In this surgery, only one part of the joint is replaced.

2. Total Knee Replacement: Total knee replacement surgery involves replacement of both sides of the knee joint.

The usual hospital stay period is around 2-3 days after the surgery is completed. Initially, you will require the help of crutches to walk for at least 2 months. You may also be asked to do gentle knee strengthening exercises. It may take up to two years to recover completely from a knee replacement surgery. If you wish to discuss about any specific problem, you can consult a orthopedist.

Source of back pain in your case is the intervertebral disc bulge causing pressure over spinal cord and nerve roots. You have not mentioned details of your wife's problem like duration of pain or associated symptoms like weakness of legs or bladder / bowel incontinence. Treatment depends on duration of symptoms and neurological symptoms. Such disc bulge generally responds to rest, anti inflammatory drugs and exercises within 12 weeks of onset. In cases where symptoms persists beyond 12 weeks or progress intervention is needed. You can consult an Interventional Pain specialist in such situation and get treatment called epidural steroid injection. In this procedure anti inflammatory drugs (steroid along with local anaesthetic ) is injected at the nerve roots affected under X Ray guidance in operation theatre. This gives pain relief and decreases nerve swelling too. Patient can undergo effective physical rehabilitation and exercises after pain relief. Surgery like decompression of nerve roots is needed in less than 1% cases where there is weakness of leg muscles or loss of bladder , bowel control. You should follow the treatment plan given by your doctor after patient assessment.

Avoid sitting cross legged. Avoid squatting- quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain.

Here are some tips for how to handle your back pain while sleeping: -while sleeping, try lying in a curled-up, fetal position with a pillow between your legs. If you usually sleep on your back, place a pillow or rolled towel under your knees to relieve pressure. -find a mattress with back support. A good mattress should provide support for the natural curves and alignment of the spine. Medium-firm mattresses usually provide more back pain relief than firm mattresses. -do not perform activities that involve heavy lifting or twisting of your back -avoid bending forward and sitting in a place for long time -stretching and strengthening exercises are important. Consult a physiotherapist for detail assessment and management of your pain. Best wishes.

Massage with your thumbs from the mid points of the shoulder blades on both sides towards the lower back of your head up to lower level of the ears. Use oil or water for massage. Rotate your head clockwise and anticlockwise, forward and down, left and right a few times. Do for 5 to 10 times morning and evening. Give pressure the dipping point on the portion between the thumb and the index finger on the back of the palm. Press and release for 15 times where the pain is more. This will solve the problem. Further do some wrist rotation exercise. Consult after 5 days. For further guidance and treatment.

Can you tell me whether there is any weakness in legs, numbness or any other associated neurological symptoms etc? you can start with physical therapy, calcium and vitamin d supplementation, gentle anti inflammatory medication like etoshine for 5-7 days, daily brisk walks for 2-3 km in mornings. We will again see after 2 weeks whether you are improving or not otherwise we will get some blood tests and x rays of the involved region. Most probably you are suffering from oa of knee. Knee surgery-total knee replacement (tkr) is quite safe & gives uniformly good results, in our hands. We have well equipped center in delhi. Do ask for detailed treatment plan. Surgery should be done when patient is still active & will be able to take benefit of surgery. Do not ignore. It could be beginning of a serious problem. Get a mri done to pin point the cause and site of pain. Meanwhile go for ayurveda's panchkarma therapy - we have many option for you - kati basti (specialised panchakarma procedure for backpains), localised massgae with herbal oils, localised medicated steam, pottali massage. Continue these procedures for atleast 15 days. Regular oiling of the joint will improve the lubrication also. These procedures will relax your back muscles and give strength to your nerves and muscles of back. Also start some yogic exercises - tadasana, dhanurasan, halasana along with these therapies. This will greatly help. Please avoid pushing / pulling heavy objects, long sittings, long two-wheeler drives. Although some herbal medicines might also help you but after proper history taking.

Hello, as regards diabetes it is essential to control blood sugar as close as possible to normal. Fasting should be around 100 mg, pp should be 150 to 160 mg and hba1c% <7 (ideally <6.8%). About cervical and lumbar pain it is better to consult an orthopedician and get it assessed/ if certain physiotherapy is suggested then that should be followed. Most often certain exercises do help in improving those conditions. Even effectively controlling diabetes also improves those conditions considerably.
