Hello, I am Dr. Pooja Sharma Dimri. I'm a gynecologist occupation and an infertility specialist. I also specialized in minimally invasive surgery, gynecology including operative laparoscopy and histroscophy copy. Now I'm going to talk about a very common complaint with women present with us, which is irregular menstrual bleeding. Irregular menstrual bleeding may be in the form of delayed or irregular periods or heavy menstrual bleeding or frequent menstrual cycles. Most of the common cause of this irregular menstrual bleeding is hormonal issues like polycystic ovarian disease or hyperthyroidism.

In other cases, the cause of heavy or irregular menstrual bleeding maybe some pathology in the uterus like fibroids, polyps, abnormalities in the uterine lining or the endometrium to diagnose the cause of the regular or the heavy menstrual bleeding.

Before we go on to treatment we may need to do certain investigations like the hormonal profile to her on hormonal issues, sonography of the uterus and the pelvis to see for any nutrient pathology but sometimes all pathology may not be detected by the imaging studies and the blood investigation and we may require a hysteroscopy or a minimally invasive surgery to diagnose and treat these pathologies. Now hysteroscopy is a minimally invasive technique in which a specialized scope or telescope is put inside the uterine cavity in which we can directly visualize any pathology like a small uterine polyp or thickened endometrium at the same thing. That pathology can be removed and the endometrial lining can be sent for a biopsy.

In cases of older patients and specially in the ferryman age group, it is very important to rule out chances of cancers of the uterus. So in case of in irregular menstrual bleeding it is better to consult your gynecologist to rule out abnormal pathology.

Is condom 100% avoid pregnancy? And why people speaks condoms are not 100% safe to avoid pregnancy?
It's not 100% effective because of some chance of slippage and breakage. In fact nothing is 100%. Each method has its own failure rate

If your wife has a regular 28-30 days cycle the baby days start from 12-16 th day when the first day of bleeding is considered as day 1. You can try daily or atleast alternate days during this period.

If you are having normal penetration there is no problem. Some amount of semen will come out. Sperms which are normal swim up the vagina and intercourse. Do a semen analysis to see for sperm count and motility.

U can have intercourse if it is not prohibited by your gynecologist for any reason. But your wife should be comfortable. If there is any problem of bleeding during pregnancy, low lying placenta or pain please avoid intercourse.

Hello Dr. I wanted some advice on a problem my friend is facing All the days are calculated from the starting day of her last periods She indulged in an act of mutual masturbation in an afternoon of the 12th day after her periods started and she was worried about getting pregnant as the guy masturbated himself too during the act (he fondled the vulva after self masturbation when there was precum in his hands which he wiped but not sure he did or not. So when I was told about this I went to a chemist and asked for a solution and he gave an I pill which she took in the morning of the 14th day. There were signs of nausea vomiting and moods swings after taking the ipill. She also engaged in overexertion by exercising and running as it could induce miscarriage through early periods if she was pregnant. She also had sesame seeds and pineapple juice due to the same concern. On the 29th day (expected day for the next mensuration cycle) she had severe Cramp's and she got her periods but it was not the same as the normal one (it had a clot's which was brownish in color and happened only on the 1st day) and on the next day she got dark red clots and the flow wasn't much and had a light bleeding than the normal periods. Today is the 31st day There were no symptoms till today after her periods started on the 29th day such as nausea or vomiting but she had Cramp's Today is also the 4th day of her periods but she haven't got any. Could she be pregnant Are the abnormal period because of the ipill.

Paranoia Hi I am a virgin, something recently happened but my boyfriend ejaculated around my vagina, by the labia and clitoris. Close to his navel but I am afraid some might have dripped down. There was no penetration. Out of being paranoid I went to take a pill called escapelle. An hour after it happened. It happened in my luteal phase. Now my period is a little late by a day, and today I got a brownish discharge. Could I be pregnant? Why is my period late.

There is no absolutely safe period. In a regular 28 days cycle day 1-10 and day 18-28 are relatively safe days but safe period method is very unreliable and has a high failure rate.

Daljindar Kaur's inspiring story of motherhood: How IVF became her savior

The success of In vitro fertilization continues in India as a septuagenarian woman gives birth to a baby boy. Even after 46 years of marriage, the couple from Amritsar was childless. Daljinder Kaur, along with her 79-year-old husband, Mohinder Singh Gill, had been visiting National fertility and the Test Tube Baby Centre in Hisar since 2013 and did not give up even after two failed IVF cycles. They had faced ridicule for their inability to have a baby and yet they kept their hopes up.
On the 19th of April, the couple was blessed with a baby boy who is nothing less than a miracle. The groundbreaking achievement of the embryologists from Hisar has been an inspiration and a beacon of hope for couples all across the country. Once again, IVF has proved to be one of the most successful procedures to have babies for couples with fertility issues. Following are some of the advantages of In vitro fertilization, which you should know about-

1. It can be used by anybody - In-vitro fertilization is not necessarily used by the going to be a mother. It can be used by gestational carriers or by a surrogate as well. So it depends on whether the parents want to take the help of a surrogate or themselves want to be involved in the pregnancy. IVF gives an opportunity to single women and same-sex couples to become parents.
2. Use of donated eggs and sperms - There might be circumstances when the doctor can recommend using donated sperms or eggs. In that case, the eggs can be fertilized manually in the lab and the fetus developed can be used for in vitro fertilization. This increases the chance of pregnancy by a fair amount.
3. Control over timing - In vitro fertilization is a solution for couples who have a very engaging professional life, and can be an option for people who want to have babies after a certain point of time. Eggs and embryos can be preserved for future use. It is also helpful for couples who already have a child and plan to have another later.
4. Decreases chances of miscarriage - Genetic abnormality is one of the primary causes of miscarriage. The technique of preimplantation genetic diagnosis (PGD) is used to ensure the viability of the embryo much ahead of time so that the mother can have an uncomplicated pregnancy and carry her baby full term.
