There are many lifestyle and dietary guidelines that can be followed to protect against future flares or prevent gout or uric acid problems from occurring in the first instance: maintain a high fluid intake (2-4 liters a day) avoid alcohol eat a balanced diet limit fish, meat and poultry intake. If attempting to lose weight, avoid low-carbohydrate diets. If carbohydrate intake is insufficient, the body is unable to burn its own fat properly, releasing substances called ketones into the bloodstream. This results in a condition called ketosis that can increase the level of uric acid in the blood. It is most important to avoid foods that are high in purines, to ensure that the levels of uric acid in the blood do not get too high. Here is a list of high-purine foods to be wary of: beef kidneys brains dried beans and peas liver mackerel yeast fermented products (breads etc) mushrooms lose some weight by walking or other exercise, avoid meats and get always be on a prescribed diet as prescribed by a registered dietitian. I am a registered dietitian, certified diabetes educator and a doctor.

My father aged 60 have sugar and BP. Now recently 4 days back he got bp of 210. Now he is hospitalised. Presently BP and sugar is normal. Blood urea is 109 mg/dl. S.Creatinine is 6.9 mg/dl. Sodium 126 mmol/L, potassium 3.5 mmol/L, Chloride 99 mmol/L. This is todays report. But yesterday S.Creatinine was 7 and 4 days back it was 4.3. As of now compared to yesterday creatinine is decreasing. Here doctors are suggesting for dialysis is this necessary. As we consulted other doctor they are saying dialysis is not required at this stage. Please suggest me. What to do should we take him for Bangalore.

Hello, it is difficult give an opinion from a distance regarding such acute conditions. However, if bp is near normal and blood glucose also in good contro, then by giving adequate fluid intake electrolytes can be managed. Same is the case with urea and creatinine. Best is to abide by the on the spot clinical expert, who can be the best judge. It may best to shift to a better equipped hospital as in bangalore. Still I suggest go by your attending physician's advice. Thanks. L.

Mr. Lybrate-user, if your blood sugar control is very good, plus you are on oral ani- diabetes drugs, then you can test once a month. It is always good to keep a track at that interval. Hba1c% should be done once in 4 to 6 months, which will give a clear idea of long term control.

’m a 28years old male that was diagnosed with a partial ACL tear in my right knee. I’ve been living with it for 4 years. I’m not nearly as strong lifting or explosive jumping and running and my knee feels tight and weak for days after heavy sports or leg workouts, although it does’t buckle. What would be your thoughts on my situation? Grade || signal intensity in body and posterior horn of the medical meniscus, suggestive of tear. Medical serpiginous low signal intensity lesions on T1 W with central hyperintense signal on T2 W images involving medical and lateral femoral, lower 1/3 shaft of femur likely to represent bony infarcts. Small osteochondral defect with partial loss articular cartilage at lateral femoral condale. Minimal joint effusion Uric acid also high side (end line) 7.1. Thanks.

Partial acl tear usually do not need surgery. Moreover you symptoms are not due to ligament insufficiency. Partial articular cartilage defects are left as such. Grade 2 signal in meniscus are usually not symptomatic. It may not be wise to suggest anything without clinical examination and mri films, but as per your symptoms and mri report, I think you should focus on quadriceps and hamstring strengthening exercises. Any surgery is less likely to improve your symptoms.

I am using thyronorm 125 mg one tablet every day since 2011. Right now all things are going well. My question is whether I have to continue or what to do please suggest.

If your tsh is within range after taking 100 mcg of thyronorm every day then that's all that you need to continuously do. Thyroid hormone deficiency is tackled by proving this hormone in tablet form to the patient every day. Since you are already controlled with a set dose - you just have to continue this good habit.

My uncle has started taking alcohol from 2 weeks before that to 2-3 bottles at a time. He takes sleeping pills for sleeping. He has diabetes, blood pressure, thyroid (i do not know the correct spelling, half of the pancreas had been cut off. My main ques is what can we do so that he stops taking alcohol and sleeping pills. It would be very helpful if you answer my ques.

Since he has already suffered from pancreatitis and half of the pancreas is already removed, he should stop taking alcohol totally other wise he will have serious problem. Let him attend alcoholic anonymous programme near to your locality, where they conduct various counselling sessions with a group of alcoholic to get rid of alcohol. Many people have benefited by attending the programme. Consult physician for further advice.

Hello dr, my name is simran frm barabanki. actually my mother in law is suffering from diabetes frm last 8 years. Presently she is 65. other than high sugar she does not hve any other issue. cholesterol blod pressure was normal nd other parts r working wel. Til nw. normal symptoms occur when sugar rises til nw. Bt Dr. Frm last one month she is sudnly gne through fever high bp nd hgh sugar level simultaneously as wel. Bp raises upto 180 nd sugar upto 700. continuously we r trying fr getting her to normal cnditn finally today she is given insulin. Nd day by day she is getting weak eats nting almost nt able to tke oral meats and food thats why the situation is getting worsen. Dr. is saying to admit her in hospital. We r going to admit her in hind hospital located in lucknow road safedabad near lucknow where she was admitted earlier also. please Dr. Help me to do right think first what to do first with her. What wl be the rit way to make her normal nd healthy. Urgent advise is needed. Everyone is getting scared to see hr cnditn. please help dr.

Hi Developing high blood pressure during the course of diabetes is expected but this has to be kept under control. Looking at her present condition, it would be best for her to start on insulin because she needs control of diabetes at the earliest without any delay as further delay will worsen her health. This sudden shoot of blood sugar and blood pressure could be because of some infection in the body. Get her admitted so that the cause of her fever can be found and treated which will help in controlling the blood sugar as well.

Hello! diabetes n its symptoms can be well managed by a good diet chart. Take small meals in every 3-4 hr. Regular interval. Avoid sweet n sugar also honey n jaggery, fried foods, ready to eat foods, preserved foods n simple carb like maida. Take whole grain cereals in limitation, lot of veggies, 150-200gm fruit (not juice. Don't skip breakfast n take a small portion of milk or product or any whole grain cereal before bed time. Do 30 min of exercise a day. You can choose my diabetic care diet package for personalise diet plan.

Mr. Lybrate-user, diabetes can only be treated by either oral anti-diabetic drugs or insulin injections or both. Plus diet restriction and regular exercise. Unfortunately there are no other drugs in any branch of medicine which control blood sugar. So my suggestion is to continue present treatment. I have seen patients in my practice who stopped modern medicine and started other therapies and have come back with exceptionally high blood sugar. Now the choice is yours.

Res sir i am 75 years old and debetic from the last 30 years.I have stomech problem.Every 10 - 15 days i have loose motions. That continue for two three days. In single day i have to go for above 20 times. I have taken several medicines from most senior doctors. But problem is there. I use to take medicines tab glycomet gp 2 1---0----1 and insuline 30/70 30 unit morning and 20 unit evening. Please suggest for stomech loose motions problem.

1st thing for maintain diabetes a regular healthy diet routine. You should start 8 to 10 seeds of fenugreek in morning. High fibre diet. Low sugar. Low oil. Low starch. Etc.

Lybrate-user, the condition being described looks like he had an attack of hypoglycemia. Did you check his blood glucose level then? if blood glucose level goes below 70 mg or lower like <60 mg or even lower, patient may get such attacks. It is important to achieve a strict blood glucose control, but equally important thing is not to allow episodes of hypoglycemia. Severe hypoglycemia can be very dangerous, particularly in elderly persons. If you have a glucometer at home, it is better to check glucose level. If it is low, then it is essential to give glucotabs 2 or 3 tabs cor candy, sugar ar some sweet etc can help. Thanks.

Hi lybrate-user, Try to keep to a regular timing for going to bed. Avoid fried and heavy foods. Eat early and light food at night so that the food is almost digested by the time you go to bed. Prefer fruits and green vegetables. Fix daily work schedule and be busy with your work. Don’t keep pending problems in mind. Note down the points and leave them for next day’s action. Don’t do stimulating mental work or keep watching TV which can keep your mind agitated or active. This will not allow you to sleep. LCD screen will also have adverse effect. Day’s physical exercise makes you tired and induces sleep. A warm bath in the evening will also be helpful. Low volume music also induces sleep. Keep your bed room neat, dark and cool. You may cover your eyes with bed sheet. Drink plenty of water during the day rather than toward bedtime. Don’t have stimulating drinks like tea or coffee. Drinking a glass of milk before going to bed is fine. Do check your FBS and PPBS once and revert back to me with reports. As increase in sugar levels also causes sleeplessness. If you get back with reports. I'll give you a permanent solution. Take care.

डॉक्टर होने के नाते हमें अनगिनत बार इस सवाल का सामना करना पड़ता है कि इंसुलिन कब बन्द होगी। यह सवाल आधारित है इस मिथ्या पर की इंसुलिन शरीर को कमज़ोर करता है या इंसुलिन लगने का अर्थ है शुगर की बीमारी में गिरावट। लेकिन इंसुलिन शुगर की सबसे क़ुदरती दावा है क्योंकि आपका शरीर भी शुगर क़ाबू में रखने के लिए इंसुलिन ही बनाता है। अतः इंसुलिन को बंद करने से आप कोई लाभ नहीं पायगे। बल्कि मेरे विचार में आप शुगर की अन्य दवाइयाँ बन्द करके केवल इंसुलिन द्वारा ही शुगर को कंट्रोल करे। आपके अच्छें स्वास्थ्य की कामना करते हुएँ।
