I am 64 yrs having sugar, bp and cholesterol, taking medications. Please of late having problem while walking causing imbalance. Please ct scan of brain done - no probs - report says - please age related cerebral and celebellar atrophy. Done some physio, but no improvement, your valued opinion solicited.

MPT, BPT
Physiotherapist, Chennai
Dear , if you' re having imbalance problems then you should do specific balance exercises. Start with simple exercises like in sitting: touch your nose with your finger tip, touch a point in front of you (this can be done by placing a sticker on a wall) making sure you touch the center of the point, walking in a straight line, walking around obstacles. Initially these exercises may prove to be challenging but as you go along it gets easier. Start with one exercise at a time and everyday do the next exercise along with the previous day' s exercise. It takes 3 months for the improvement to be of significant level. If you require anymore clarifications please feel free to contact me. Preethi mathew (mpt, maiap) neuro-physiotherapist

Hi doc, I am a medical student. I have minute tremors which mostly get aroused during conditions like handling an object, or when I get scared or when I don't eat. My thyroid pro. Done b4 6-8 months were normal. I have this prob since last 4-5 years. What to do?

LCPS, BAM&S
General Physician, Pune
CML is a rare condition and you are fighting it. The medications are perfect and you should be able to lead normal life soon. So continue your treatment with a positive mindset. You will be alright soon. The real cause of tremors is just nervousness coming out of the battle. You are a brave young man. Toss all these jittery thoughts into air by the flick of mind and pursue your education with a focus. You will be a great doctor one day. Don't pat any attention to tremors. They will vanish the moment you neglect them.

Mr. lybrate-user, Thanks for the query. For hypothyroidism there is only one medication and that is adequate dose of Levothyroxine to be taken daily early morning on empty stomach. If TSH is maintained between 3 to 4 mU/L then most of the symptoms of hypothyroidism are in check. As regards migrane it is a difficult condition to treat. Multiple remedies have to be tried, plus precaution should be taken to avoid known triggers. Thanks.

I m having migraine nd headache from last 15 days so please prescribe me a good treatment so I can feel good.

COMT, MPTh/MPT
Physiotherapist, Surat
Headaches due to migraine is very common these days especially in young adults because of there lifestyle and working environment. Sometimes pain is very severe on one side of the head.  So I would recommend you to avoid stress and anxiety, quiet the mind from distractions. Stay calm positive and do deep breathing exercises, listen some music that would give you some relaxation. If pain is severe then place ice pack over forehead and lie down for some time to relax your mind and give you comfort and stress full life. Massage your head using levander oil or clove oil as it removes toxins from body. You can use mint or clove spray for inhaling in case of headache. If pain is severe you can also apply ginger paste over the painful area. Avoid taking caffeine, alcohol, phenylethylamine (found in chocolate and cheese), tyramine (found in nuts and fermented meats, cheeses, and soy), and aspartame (in many artificially sweetened foods) as sometimes these all exaggerate pain instead use basil leaves tea or ginger tea boiled nicely in water without milk. Regular intake of healthy dietry food rich in proteins and vitamins. Avoid fasting - Eat breakfast, lunch and dinner at fixed timings. Make yourself busy by fulfilling your hobby as it will give you freshness and your mind will be diverted from normal life which help in pain reduction. Take min 7 hours of sleep. Make a habit of 20-25 min walking daily along with some meditations. If required then only take pain relieving tablets like naprosyn For more help or instructions please consult. Good Luck and Get Well Soon.

I am 71 yrs 5ft 8 in weight 90 kg am suffering from diabetic for last 5 yrs lately I find that sole of both my feet become numb when I get up in the morning and also my right heel is very painful please advice what I should do ?

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist, Delhi
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Your symptoms of numbness could be due to long standing diabetes in your case although other causes cannot be ruled out especially issues in lower back region. I would advise you for a very strict sugar control, you can take medications like pregabalin or gabapentin in low doses initially, take vitamin b12 and adequate calcium and vitamin d supplementation. Also heel pain could be due to inflammation in your heel fascia. Treatment modalities are aimed at relieving inflammation of the fascia, like ice packs, physical therapy, footwear modification and anti-inflammatory therapy. I will observe you with this management for around 2 weeks and then we will see if we require any investigations in your case. Do not hesitate to contact me if you need any further assistance.

First thing that should be done in this problem is to improve sleep hygiene. - Fix your time to go to sleep. - Avoid alcohol, tea, coffee in evening - Light exercise will also help in getting good sleep - Use your bed for sleeping only and don't watch TV, read books on bed - Avoid day time naps - If you fail to sleep within 10 minutes of going to bed, get up and start to distract yourself. - Bath with warm water in evening will help. Try to improve sleep hygiene and if condition don't improve consult a psychiatrist or sleep specialist. Medicines like Benzodiazepines like Clonazepam, Alprazolam etc help in getting good sleep. Z-drugs like zolpidem or zopiclone also help in getting good sleep. These drugs have abuse potential so should be taken for short time only and under proper prescription. Thanks, hope this helps you.
