How to Diagnose and Manage Acute Coronary Syndrome

The word acute coronary syndrome refers to a group of symptoms that are caused by blockage of the blood flow to the heart muscles.  The most common result of this is myocardial infarction or heart attack as it is popularly called. Reduced blood flow leads to death of some portion of the heart muscle wall. While the word heart attack sounds almost fatal, it need not be the case. Knowing how to identify an attack and being aware of some simple measures can help save lives. 

Symptoms
The tell tale signs of a heart attack are as follows:

  • Chest pain and discomfort usually described as a tightness or burning in the chest region
  • Pain along the left side of the shoulder and neck, going up into the jaw, down to the arm
  • Nausea and vomiting 
  • Profuse sweating 
  • Difficulty breathing 
  • Dizzy or fuzzy feeling 
  • Tired, extreme fatigue
  • Anxious, apprehensive feeling 

However, be also aware that there are a lot of people who experience a silent heart attack. Women, obese, elderly, and diabetic patients can have silent attacks and depending on severity, either they go on with life as usual or can have a fatal attack.

Diagnosis
Once you are doubtful of a heart attack, the next step is to reach the closest medical facility for a diagnosis.  In addition to a detailed examination and history, the following two tests will be performed.

  1. Electrocardiogram (ECG): A 12-lead ECG will measure electrical activity of the heart and identify irregular electrical activity, which is indicative of a myocardial infarction.
  2. Blood tests:  Presence of certain enzymes in the blood, CK-MB and troponin are indicative of a heart attack.  A complete electrolyte profile also will be done, and increase or decrease of some electrolytes is helpful in diagnosing a heart attack.
  3. In addition to these two, chest radiography, cardiac angiography, echocardiogram, stress test, and computed coronary tomography may also be required to confirm the diagnosis.

Management
Once diagnosed, the first step would be to relieve the symptoms, negate the effects of reduced blood flow, and restore cardiac function. 

  1. Dissolve the clot: Using thrombolytics like clopidogrel 
  2. Nitroglycerin: To dilate the blood vessels and improve blood flow, especially to the heart muscles
  3. Anticoagulant therapy: Blood thinners are usually used to avoid blood clot formation; aspirin and heparin are the most commonly used agents.
  4. Drug therapy: Blood pressure maintaining drugs like beta blockers and/or angiotensin-converting enzyme (ACE) inhibitors are also used
  5. Use of Statins: Statins are used to reduce the amount of cholesterol in the blood and stabilize plaque deposits.

In very severe cases, angioplasty and stenting or coronary bypass surgery may be required. Educating people on how to identify a heart attack and manage it is very useful and can help save lives.

I am 37 years old and I have recently married. I am a smoker. My wife has 35 years old. She get laparoscopy surgery two times in 2010 for removal of the chocolate cyst on outside uterus and in Right side tube. Doctors removed the cysts and right side tube. Now she has a cyst in left side tube and uterus is in normal condition as per doctors report. Is she become pregnant and also give suggestions for improve my sperm count and mortality.

Hi lybrate-user as far as your sperm count and motility is concerned most important for you is to quit smoking. You can also take supplements for sperm health. As far as your wife is concerned, she is suffering from endometriosis which causes a decrease in ovarian reserve. This with the fact that she has undergone 2 surgeries which further reduces ovarian reserve. And with age 35, she requires proper consultation for fertility including investigations if she doesn't conceive after trying spontaneously.

6 Exercises Which are Safe to Do During Pregnancy

What exercises are safe to do while I’m pregnant?

The benefits of exercising in pregnant women are to make pregnancy comfortable and the labor process lesser. This is because the muscles will be ready for support in time that your baby will come in full-term. If you’ve been exercising daily, you can check with your trainer or physician which ones you can still continue while pregnant.

In the meantime, here are the exercises that are safe to do while you’re pregnant:

Swimming

It is on the top list for appropriate exercises in pregnant women because it involves breathing and movement of the hands and legs. It is perfect for you because in the water, you will feel lighter and free to move with the baby in your tummy. If you have back pain, swollen ankles, or feel a bit nauseous due to your pregnancy, this exercise will help you relieve these pregnancy-related discomforts.

Prenatal yoga

Yoga is among the most popular exercises for pregnant women because of its relaxing effect. The exercise also needs you to focus and do deep breathing which will be needed when you’ll be in labor. There are special yoga classes for pregnant women that only require poses that are safe and comfortable for your condition.

Brisk walking

Although 30-minute walking is already a good exercise, brisk walking is even better especially for pregnant women. The exercise will strengthen the legs and thighs.

Stair climbing

The federation of obstetric and gynecological societies in india (fogsi) pointed out the importance of caring for the knees and legs. Among the important exercises pointed are walking, running, skipping, weigh training and proper stair climbing. In stairs climbing, fsogi suggested that it will be helpful to climb 15-20 steps per leg. This will strengthen the thigh muscles helpful in labor and delivery.

Pilates 

Pilates is another exercise for strength and flexibility that will be helpful for delivering a baby. It is safe for pregnant women but must be done in the presence of a professional instructor. This exercise can relieve any back problems and improve your posture.

Indoor cycling

Indoor cycling is safe for pregnant women because you can do this exercise without pressuring your knee and ankle. It is convenient because it can be done in the comfort of your home or at a gym that will prevent accidents like falling off the bicycle.

If you are pregnant or expecting to be pregnant, you should start exercising now. The exercises mentioned above will be helpful for you to get through 9 months with ease and comfort. There are a lot of benefits of exercising apart from strengthening the muscles, endurance and promoting muscle tone, it also helps you sleep better, prevent excess weight gain and boost your energy level. Constipation, backaches, swelling and bloating can be reduced or relieved through exercising. So, you should start your safe exercise program now.

I had sex with my girlfriend. I ejaculated outside her but just to be sure I gave her an I pill to consume just after 10-15 mins after the act. She started to bleed within the next two days. But she again bleed after 3 days after the last bleeding She feels it's like the Period bleeding. She had her last period on 30th of May. What is the reason of this and have we successfully avoided the unwanted pregnancy? Please reply urgently. Thanks.

Hello, she is likely having her normal menses which were due by 1st week of june anyhow and hence she bled within 3 days of pill intake, not due to the pill but naturally. This bleeding further rules out pregnancy.

Ovarian cysts is a common problem that affects women in their childbearing years. These cysts develop inside the ovaries at the time of ovulation.

These cysts are of four main types:

  1. Polycystic ovaries

  2. Endometriosis

  3. Cystadenomas

  4. Dermoid cysts

In most cases, ovarian cysts resolve themselves, but in some cases they can cause pain and grow.

Ayurveda is a holistic form of alternate medication that can be quite effective when it comes to treating ovarian cysts. This form of medication has negligible side effects and hence, can be prescribed to women of all ages. Apart from medication, Ayurveda also involves changes in lifestyle and diet. Ayurvedic herbs help balance the hormones in a woman’s body and thereby improve the overall functioning of the ovaries as well.

Vitamin B complex helps in converting excessive estrogen into less weaker forms and hence balances hormone levels. If you had periods after taking contraceptive pills and have no history of intercourse after that, then there are least chances of pregnancy. Sometimes, after taking contaceptives, period dates gets disturbed. So wait for periods, at least 1 week more and if still period doen't come, get a urine pregnancy test done.

Abortion may not be complete as small part may be left inside the uterus. This could be the cause of bleeding or pain.

My period was on june 1 missed period for 19 days on 15 th june I used home pregnancy test it is negative. Doctor advised me to do tvs n us in that no sac found but thicker wall 14.3 mm. On 19 th day after scan done bleeding started for 2 days n 2 days spotting. Doctor advised me to take susten vt 200 table. Iam very much confused whether i am pregnant or not please help me. Dear, do not be stressed. There might be a chance of a missed abortion. Please undergo a proper ultrasound (not tvs) doctor has prescribed progesterone considering the possibility that you may have conceived and progesterone will help in retaining the embryo. Nevertheless ultrasound will give a clear picture. Best of luck.

Oral contraceptive are not safe, sometimes it can cause hormonal disorder in female, which interrupt to become pregnant if future. So don't use oral contraceptive pills, using condom is safest.

Periods can start any time. Pregnancy can occur even without periods. Either to protected sex or use IUD as contraceptive.

Don't start taking treatment do soon you may become dependent on that one year is not a long time as you said last year you conceived give some more time may be another year and wait for the things to happen on its own if you get too anxious for pregnancy it can delay further as it disturbs your hormonal balanc and pregnancy is totally dependent on right hormonal balance.
