All About ECMO - Extracorporeal Membrane Oxygenation

Fellowship in Electrophysiology, Fellowship in Interventional Cardiology, DM - Cardiology, MD - Medicine, MBBS
Cardiologist, Delhi
All About ECMO - Extracorporeal Membrane Oxygenation

How many of us had heard about Extracorporeal Membrane Oxygenation (ECMO) before Tamil Nadu, Chief Minister late J Jayalalithaa was put on it for recovery? Hardly a handful. Patients who are placed on the device have 50-50 chance of survival and while it could not help the late Chief Minister, her condition put a spotlight on ECMO.

What is ECMO?
ECMO is a life support device that is put in place when a patient’s heart and lungs stop functioning completely post a cardiac arrest. When the patient does not respond to Cardio Pulmonary Resuscitation (CPR), the patient is put on ECMO.

Function of ECMO
The core function of the device is to separate carbon dioxide from the blood and add oxygen into it. It takes the load off from heart and lungs, providing critical time to the body organs for its treatment and recovery.
Patients are only put on ECMO, if there is a chance that the condition can be reversed.

How does ECMO work?
The device drains blood from a vein, which is attached to an oxygenator machine. The machine then removes carbon dioxide from the blood and adds oxygen to it. Depending upon the condition of the patient, the warm blood is either pumped into the body with the help of the machine or is transferred back into the body through an artery.

How long a patient can be on ECMO?

  1. After seven days of being put on ECMO,there are chances of patient getting an infection.
  2. In case the patient contracts an infection, it is recommended to take the patient off ECMO support.

Such issues are critical questions requiring very meticulous judgement which is possible only after getting to know entire case history of the patient. It will not be prudent to give any affirmative response to your query as I can guess the cardiologist in charge has probably advised for angioplasty and based on that you are seeking second or third opinion. That is very essential and you are doing exactly what I would have done for my own family. But no doctor will be able to opine without having all relevant details of the case. Medicine is not a prefect science like mathematics and that's why there is no formula. Only we get guidelines and protocols based on evidences. To follow protocols the doctor has to see that his judgement is based on criteria as laid down and those criteria are to be obtained from patient' case history.

I suddenly feel pain in my heart, it occurs in every 2 months. Please suggest me what can I do to relief from this pain?

DHMS (Diploma in Homeopathic Medicine and Surgery)

I suddenly feel pain in my heart, it occurs in every 2 months. Please suggest me what can I do to relief from this pain?
The sudden pains only in your age are not suggestive of any heart problem. For heart linked problems we must also keep other factors related to heart in our mind. It can be either a nervous or muscular pain and in your case the chances are of for a nervous pain as these pains are intermittent every 2 months. Homoeopathic medicine hypericum 30 (dr reckeweg) drink 5 drops in 1 spoon fresh water 3 times daily for 2 months.

High triglycerides and cholesterol can cause atherosclerotic heart disease or narrowed coronary arteries in the heart can cause the symptoms of angina, when theheart muscle is not provided with enough oxygen to function. Decreased blood supply to the brain may be due to narrowed small arteries in the brain or because the larger carotidarteries in the neck may become blocked. This can result in a transient ischmic attack or stroke. Peripheral artery disease describes gradual narrowing of the arteries that supply the legs. During exercise , if the legs do not get enough blood supply, they can develop pain , called claudication . Other arteries in the body may also be affected by plaque buildup causing them to narrow, including the mesenteric arteries to the intestine and the renal arteries to the kidney. Change life style .do meditation, yoga, Go walk daily, do any one physically activity daily i.e.play any game., Avoid fast foods, oily foods, junk foods, alcohol, smoking, fatty foods, non-veg.,avoid stress, take plenty of water in day time. I am 48 working women recently got tmt positive. What to do?

Diploma in homeopathy, B. Sc
Homeopath,
Improve your living standard, light exercise and diet your diet must include fresh fruits and a full serving of salads at every meal: cucumber, tomato, carrot, radish, onion slices or spring onion, raw cauliflower, cabbage leaf, etc. All this is essential to give you a balanced nutrient boost. Cut out any junk food and aerated drinks totally. Generally foods that come out of a packet, or can, or bottle shud be avoided. Exercise to increase circulation:- 2 mins of medium exercise then 1 min rest - do this four times a day. You can either do it all in one go or split it into sessions through the day. In total it is only 12 mins a day so should not be a big issue. A very useful exercise is to jump in one place simulating skipping. Build up your time slowly. Start with 30 jumps and keep increasing slowly. If you are not going in for any surgery in the near future, take homeo remedies to clear your arteries.

Creat upto 1.4 is considered normal. But you need to be careful about the protein intake as excess might increase the creat inine level. Focus on quality proteins from milk or egg white and measure the salt added to the food. This is very important because there are hidden sources of sodium in our daily foods. Keep a watch on fluid intake as ef is low. Give him small frequent nutrient dense foods throughout the day.
