So, we specialize in laparoscopic surgery or minimal surgery in any form be it obesity surgery, hernia surgery, we do laparoscopic surgery for various ailments. What is hernia is the question I am quite often asked. So, hernia is any defect in your muscles of the abdomen cavity. Now, what are the different types of a hernia. So, the hernia can be divided into different types and location. Most often seen in men is a groin hernia which happens usually in the groin and it can also be an inguinal hernia, which happens because the testis which causes the problem to the kidney. And the track which leads testis to come down and that's why you get an inguinal hernia. You also get a femoral hernia. You can get the hernia which is located in the belly button which is umbilical hernia or it can be around the umbilical region called paraumbilical hernia.

This usually happens because of obesity and it is more commonly seen in women. You can also get the hernia which can also happen because of the previous surgical scar and these are called incisional hernia. You can have a hernia at the lateral side of the abdomen cavity which called spigelian hernias. The hernia can also happen because of lung cavity to abdominal cavity. Hernia can happen congeniality in your wall. Or it can have a small defect in the muscle because of the obesity and it is called hiatal hernia which causes a lot of acidities. Sometimes it is called as internal hernia that can happen post surgery especially after bypass surgery and the small defects left inside and called an internal hernia.

So, is there any emergency for surgery. Whenever the intestine comes up, either they stuck or obstructed and you start vomiting or absolute constipation. And some of the blood supply to that part of intestine gets stuck and the skin around the hernia defects starts turning into blue, start turning into fever than it becomes an emergency. Right I said an internal hernia needs high emergency. Most of the surgeries are not emergency. And in old patients, you can avoid some of these surgeries. There are various reasons that why hernia might need to operate. Sometimes you need to make lifestyle changes. Especially in hiatal hernia you might have to sleep at 30 degree of position, eat your meals and walk at least for half an hour. Try to avoid lifting heavy weights.

Try to avoid anything which is very acidic, dark coffee, dark chocolates or the citric drinks. In terms of lifestyle changes, you have to avoid lifting weight after having surgery. All you try to prevent holding stress. How do we treat the hernia? A hernia is the defect in the muscles definitely come with any kind of medications. Gone are the days when we used to do open surgeries for hernia to now laparoscopic and robotic surgery. Very less scars with these surgery. So, most surgeries either gives few stitches or place a piece of pen which is an artificial material which covers the defects. These surgeries work really well. When sometimes if you use 2 smaller size of large defects and if surgeries are not done correctly or you have not taken proper stitches or it will pull the muscles too tight than you can get the recurrence of hernia.

Sometimes, the defects are too large, then you have to go for new techniques called AWR abdominal wall hernia repair or the trans-abdominal repair of hernia wherein you pull muscles the sides and put them closer. In the case of hiatal hernia, laparoscopic surgery is done to repair the muscles defect. In case of an internal hernia, again it is the case of emergency, you have to bring the intestine through the defect, bring it out and than close it with surgery. So, what happen when your BMI is more than 37.5 or it is above 32.5 and with uncontrolled diabetes, with the most conservative association across the world that such as American Diabetes Association, The International Diabetes, if your sugar is not under control with medications, at best you can go for metabolic surgery or bariatric surgery. You can't stick with good sugar control. A chronic diabetic would have the vessels of his heart, his eyes, his kidneys, kidney damage, heart failure because of diabetes, you can have blindness because of diabetes or loss of sensation in the foot in diabetic neuropathy. The best way to get treated with metabolic surgery. What happens with type 1 diabetes. In this bariatric surgery does not cure. But if you are obese, reduce your weight so that the insulin level comes down and other complications will also come down. Bariatric Surgery - How It Can Prevent Type 2 Diabetes?

Diabetes is a serious health problem that affects millions throughout the world. It is a disorder that has almost no cure and can lead to serious problems related to vision, kidney failure, gangrene of the toes and feet, tingling numbness and loss of sensation in the lower extremities and many more. While currently available medication tries to regulate your blood sugar levels, it fails to control damage on various organs listed above in the long term. Furthermore insulin can lead to further weight gain and worsening of diabetes in the obese people.

Bariatric Surgery has now been recognised as a treatment option for the obese diabetics by most major diabetic associations across the world, the ADA (American Diabetic Association) the IDF (International Diabetes Federation) etc. 

So how exactly does Bariatric Surgery work?

Diabetes can be of three basic kinds;

  1. Type 1 diabetes in which the pancreas produces no insulin and insulin requirement is mandatory.
  2. Gestational Diabetes which occurs during pregnancy and usually disappears after it
  3. Type 2 diabetes or adult onset diabetes that usually comes around middle age and is associated with weight again in atleast 80% of people. However type 2 Diabetes is now even seen in adolescence and is a cause for worry.

 Bariatric surgery proves to be most beneficial for obese patients with early Type 2 diabetes.

Insulin is produced by the beta cells of the pancreas to control glucose levels from the food we eat but fat cells are insulin resistant and do not allow insulin to act to its normal capacity therefore in a type 2 diabetic the pancreas produces more insulin but it never seems enough and eventually the pancreatic cells burn out. Another cause is that the liver in an obese individual has decreased insulin sensitivity further compounding the problem.

Bariatric Surgery firstly works by decreasing the calorie intake, this in turn leads to weight loss which also means fat loss so decreased insulin resistance, it also decreases fatty liver, NAFLD (Non Alcoholic Fatty Liver Disease) and this I turn improves liver insulin sensitivity.

  • It further increases gut hormones like GLP1 and PYY good for the diabetics and decreases those hormones, which are not good for the diabetics.
  • Bariatric surgery also exercises a control over cholesterol, blood pressure and triglyceride levels all of which along with high sugars can lead to higher incidence of a sudden heart attack (Acute Coronary Syndrome)
  • Bariatric Surgery causes long term remission of the disease in type 2 diabetics decreasing damaging effects on the kidney, eyes and feet giving patients a chance to live better healthier lives with lesser complications.

What is a Bariatric surgery?

  • Bariatric surgery doesn't refer to a single operative method. It involves a host of surgical procedures conducted on a person to rid him or her of obesity.
  • It might include reduction of the size of your stomach, removal of a portion of the stomach, or even bypassing portions of the stomach and intestine.

The commonest Bariatric Surgeries performed are

  • Laparoscopic Sleeve Gastrectomy: Where the stomach is converted into a thin tube of about 100 ml capacity producing early satiety. About 2/3rds of the stomach is surgically removed, removing a major part of the cells producing the hunger hormone ‘ghrelin’ thus decreasing craving for food.
  • Gastric bypass surgery: This process involves dividing the stomach into a tiny pouch at the upper end of about 30 ml capacity and connecting it directly to the middle portion of small intestine thus bypassing about 90% of the stomach and the first part of the small intestine. The food you eat going directly from the top part of the stomach into the intestine.
  • There are other surgeries as well that bypass major portions of the small
  • Intestine but are usually reserved for extreme cases.

Do I need to take multivitamins and proteins for life after this surgery?

  • Most Bariatric surgeries necessitate you to take multivitamins for life. Vit B12, Calcium and Iron being the common ones, one may also need Vit D supplements. This is necessary as your food intake comes down and some surgeries bypass absorptive areas of the stomach and small intestine.
  • Protein supplementation may be needed in the early stages but not in the long term in most surgeries except in the extreme cases as recommended by your surgeon.

Is it a Must That You Consider Bariatric Surgery?

Weight loss can be achieved through various means, such as behavior therapy, diet and exercise programs, drugs, etc. However, research has proven that once you are obese then the chances of you either losing substantial weight or managing to keep it off long-term is less than 1%. Hence most obese people yo-yo and almost always regain more weight than they started off with on the rebound with non-surgical therapies. Most drugs for weight loss have been banned in the long term because of side effects.

Cosmetic surgical options like liposuction and tummy tuck or abdominoplasty are again not really weight loss procedures but rather body contouring prospering to give a better shape and cannot achieve long-term weight loss.

Bariatric Surgery today remains the best solution for long-term weight loss for those with morbid obesity.

So when you are you’re extremely obese and have a body mass index (BMI) of 37.5 kg/m2 or more. (BMI or body mass index is a ratio of your weight (kg) / height (m2) ), or your BMI is above 32.5 kg/m2 along with associated health problems like Type 2 diabetes, sleep apnea (a common sleep disorder wherein you snore excessively and experience pauses in breathing or shallow breathing instances while sleeping), high blood pressure, high cholesterol levels, osteoarthritis or severe joint pains and have failed to achieve significant weight loss with conservative methods then you are a candidate for Bariatric Surgery.

Weight loss through surgery shouldn’t be looked upon as a shortcut to losing weight. It is a powerful tool to aid you to become healthier, but remember, "with great power comes greater responsibility", because after the surgery you’ll be required to make some lifelong changes to your eating and lifestyle habits to ensure a long, healthy and satisfying life.

Remember this is your second chance at life don’t waste it.
