Trigeminal neuralgia is a very annoying recurrent illness. Fortunately treatment is available. The permanent solution is nerve block. You may consult your neurologist for the same.

Injecting a dilute solution (4%) of ethyl alcohol for chemical neurolysis is a good alternative to other conservative treatments (such as cortisone injections) and surgical options for foot neuromas. There are very few reported complications with this technique and the success rate is relatively high.

Dear lybrate-user A few suggestions to help your Father feel better. Apply turmeric in ghee on lips to keep them soft and clear the infection in natural way. Add coconut oil in his diet. About 2 big spoons. This will help reduce the heat caused from meds. Help soften bowel movement and also keep his digestion proper. Make sure he has every opportunity to interact with other people his age and walk as much as he can. Keeping the mind engaged will help his health at all levels. Regulate his diet such that he had adequate vegetables and fruit. Usually older people eat only rice or soft carbs and this is not enough. Replace all wheat foods with jowari or other mullets of your choice. This will improve health (for every body) in a great way. Regards.

Klatskin Tumour - Understanding It In Detail!

A fifty-four years old Marwari patient from Assam presented with progressive jaundice and intermittent episodes of abdominal pain for three months. CT scan of abdomen showed a small (1.5 x 1.5 cm) mass in the bifurcation of common bile duct which is causing biliary tract obstruction and hence jaundice. Portal nodes were not enlarged and there was no lesion in the liver parenchyma. At presentation, serum bilirubin level was high (14.7 mg/dl). Endoscopic biopsy and brushing cytology was adenocarcinoma and clinic-radiological diagnosis was ‘Klatskin tumour’. 

  • Metallic stenting was done to relieve jaundice and after stenting serum bilirubin level came down rapidly. PET scan showed increased uptake in the biliary duct region mass without any sign of metastasis. He was evaluated and planned for treatment with robotic radiosurgery. He was treated using robotic radiosurgery (CyberKnife) with high precision radiotherapy technique after fiducial placement (gold seeds) near the tumour. After one year, the patient had no obvious complain, liver function (no jaundice) was normal and CT scan evaluation showed completed resolution of the mass.  
  • Klatskin tumour is an uncommon tumour that arises from the bifurcation of common bile duct in the abdomen (duct that drains bile from liver). Patients usually present with progressive (increasing) persistent jaundice followed by pain in the upper abdomen. Surgical excision is the mainstay of treatment. However, surgery is not possible in majority of the patients owing to the location of the tumour, high jaundice and medical condition. Chemotherapy may not be an optimal option in majority of the patients as they present with high jaundice. Majority of such patients with poor medical condition are treated with only supportive care and prognosis is dismal (survival for a few months only).
  • Patients with metallic stent have relief from jaundice but unfortunately in a few weeks time, the stent gets blocked with tumour growth. Patients again present with high jaundice and have severely impaired quality of life. They complain of severe itching of entire body, loss of appetite and succumb due to impaired liver function from high jaundice/obstruction. The treatment is to have a longer ‘jaundice-free period’ which in turn improves quality of life and possibly survival function as well.  
  • Stereotactic body radiation therapy’s high dose radiation ‘sterilizes’ the metallic stent and bile duct region. It is assumed that with radiation therapy, blockage of bile duct and stent is delayed and patients have longer jaundice-free period.In Klatskin tumour, CyberKnife allows to deliver high dose of radiation in a short duration to the target without significant morbidity. If you wish to discuss about any specific problem, you can consult an Oncologist.

Sir my age is 23. Height 6 ft. Weight 76 kg. Sir from 7 month I am having numbness and tingling all over body along with muscle and bone pain. I am having excessive and new hair growth all over body except my chine beard and moustache have retarded growth but whole body including eyebrows chest head pubic excessive hair growth. Earlier I used to eat 6 chappati all 3 time but due to abdominal pain from 4 month my chappati reduced to 1 at each time. Still I am gaining weight on abdomen and chest. Do take balanced diet and hyderated on regular time schedule everyday do regular physical activity and stretching and strengthening exercises and breathing exercises for about 15 minutes duration each time regularly best wishes.

PARKINSONISM

Parkinsonism or parkinsonian syndrome group is of disorders which manifests as tremor, disturbances in voluntary movement (rigidity, bradykinesia or akinesia) and impairement in the balance and automatic reactions.

This syndrome do occur in various conditions, of which Parkinson's disease is quite common.

Parkinson's disease was observed by james Parkinson in the year 1817. It took  about 100 years to be well verse with the pathology of this disease condition and the treatment was partially achieved by the introduction of Levo-dopa in the year 1960.

 

ETIOLOGY

A. Idiopathic: The etiology is not understood. Example of this type of parkinson's diease, where there is degeneration of neurons in the substantia nigra and corpus striatum. It occurs usually in late adult life, progress slowly, and kills the patient by 10-15 years after the onset.

B. Vascular: Multi-infarct disease(arthreosclerotic parkinsonism).

C. Infection: Infection by various viruses, most commonly influenza virus may be  causing parkinsonism like features. It is termed as encephalitis lethargica that causes focal insult to the brain matter.

D. Due to toxicity: Certain toxic chemicals like manganese, carbon disulfide, carbon monoxide, cyanide exposed in various occupation can cause diffuse damage to the brain exhibiting parkinsonian syndromes.

E. Various drugs: Phenothiazines, butyrophenones, tetrabenazine can cause Parkinsonism.

F. Multiple system degeneration that occurs in condition like Alzheimer disease shy-Drager syndrome, supranuclear opthalmoplegia, Wilson's disease, etc. can cause parkinsonism.

G. Metabolic: Abnormal calcium metabolism can cause disposition of calcium in the basal ganglia causing parkinsonism.

H. Traumatic: Multiple brain damage commonly encountered in boxers usually causes parkinsonism. This types of parkinsonism. This types of parkinsonism are called as punch boxers syndrome.

 

REDUCTION OF RIGIDITY IN PARKINSONISM

Rigidity has been found to reduce, to some extent, by generalized relaxation technique. Generalized relaxation can be obtained by vestibular stimulation that can be achieved by various rocking techniques. Rocking can be achieved by the use of adult vestibular ball, rocking or rotating chair, and cradle. Rhythmic initiation, which is a technique of PNF, can be used to reduce rigidity. This technique, which involves progression of exercise from passive to active assisted to active without any increase in tone, has been found to be very effective in reducing rigidity of the trunk.

Generalized relaxation techniques like Jacobson’s technique of progressive relaxation, yogasana, transdental meditation, biofeedback, savasana, etc. can also be used.
