Hello lybrate user- recurrent pimple problems might be a start to skin disease. Try these tips - a) use mahamanjisthadi kwath 2 tsp twice daily with water. B) tab gandhak rasayana 1 tab twice daily. I fractured by left olecranon and it has healed after operation with K wire and tension bands. I want to know the procedure for removal of K wire and tension band. How much time required to be in hospital and the pain involved. I am 53 years healthy male.

Dear sir you may have to wait minimum 12 months to remove the same and to ensure it is safety for removal of joints it may require 7 to 10 days depending on basic healing and opinion of the consultant surgeon best wishes.

Madam lybrate-user you have sever deficiency of these minrals and vitakins with L4- 5 bulge in my openion you should immediately consult local Orthopedic surgeon and got your treatment according to his advice. At this age this short of deficiency can lead to further complication.

I am a 36 years male. Suffering in heel pain from last three months. Specially when wake up in the morning. Uric acid is 7.8. X-ray done. Report shows the heel bone spur. Please advice.

you can take otc medicines tab. naproxen 5oo mg sos for heel spur pain..but surgery is last opinion .for uric acid you can tab. bencid 500 mg once a day. tab. zyloric 100 mg once a day. Avoid protein diets,bad habits , fast food/fat/oily,salty foods. Coffee, smoking, alcohol intake. Non-veg., take whole grain,oats,green veg.,plenty of water in day time, 1 glass of water at night before bed.

Knee Replacement

Knee is a hinge joint where the lower leg bone tibia meets the thigh bone femur. During osteoarthritis, cartilage or ligament defects and degenerative arthritis, knee replacement surgery is recommended world-wide to get relief from extreme pain. Knee replacement surgery, also known as knee arthroscopy, is a surgical procedure, in which the affected knee joint is replaced with synthetic material. The most likely candidates for total knee replacement are the patients with severe destruction of the knee joint coupled with progressive pain and impaired function.

Modern technological advances have made computer assisted knee replacement surgery extremely popular around the globe. In this surgery, the surgeon is assisted by a computer to remove the optimum amount and angle of the bone, which otherwise is done by inspecting manually. This is an excellent example of surgery through small incision and it eliminates the chances of human error. A perfect alignment and balance is achieved and hence longevity is also increased to 20 - 30 years. Knee replacement surgery is also specific to gender as the anatomy of male and female patients is different.

There are many types of knee replacements, most common being the total knee replacement or Total Knee Arthroplasty. In addition, there is partial knee replacement, bilateral knee replacement, revision knee replacement and knee arthroscopy.

In knee replacement surgery, the worn out surfaces of joints of knee are replaced with artificial implant of plastic and metal. The lower end of the femur bone is removed and replaced with a metal shell. The upper end of the lower leg bone (tibia) is also removed and replaced with a channelled plastic implant with a metal stem. A plastic ball is also added under the kneecap depending on its condition. These artificial components are commonly referred as prosthesis. The design of these highly flexible implants replicate knee, with the rotating knee replacement implants assist in backward and forward swing of the legs.

Patients whose knee joints have been damaged by either trauma or progressive arthritis should consider total knee replacement surgery. Post-surgical hospital stay after knee joint replacement is usually three to five days. The surgery has a very high success rate and shows dramatic improvement after a month. This improvement is most notable one month or more after surgery. The pain caused by the damaged joint is reduced significantly when the new gliding surface is constructed during surgery. Initially, patient will walk with the help of a walking aid until the knee is able to support full body weight. After six weeks, patient can walk comfortably with minimal assistance. Patients with artificial joints are prescribed to take antibiotics during the course of any elective invasive procedures including dental work. Physiotherapy is an essential part of rehabilitation and it will increase the muscle strength and patient can enjoy most activities, except running and jumping.

Sir my age is 81 and for the past 2 months my two legs are swelling at the evening of the day and in the morning the swelling decreases. If you press the foot it is hard. And I am using amlongat 5mg for pressure. Please advise me what to do?

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Swelling in legs is usually not caused by any orthopaedic disorder generally apart from injury etc. Since you have also been taking medicine for blood pressure, my advice for you is to get yourself checked by your physician as there are many causes for this swelling like cardiac causes, deranged lft, kft etc. Sometimes swelling is just postural, i. E when you stand and walk, swelling increases and it decreases while you lie down and elevate your legs. So get back to me once you are through with your physician consultation, I can suggest you some simple measures at home if there is no particular underlying abnormality. You appear to be overweight. Check your sugar levels immediately. Identify the mid point between the ankle bone and tendons and give pressure with a blunt edge on either side of your foot for 2 minutes. Also give pressure in the web between 2nd and 3rd finger from your toe for 2 minutes. Finally give a good massage for both feet in hot water. Consult after 5 days.

Planer faciaitis

Planter faciaitis is one of the most common problem now days. In most of the cases with medicines pain reduce & subsides temporarily but it reoccurs. Many times pain is very severe & disabling. Medications, cold pack, silicon cushion & if nothing works then local steroid injection. But most of the cases even after steroid injections it is not cured completely. In all these cases we are missing most importing aspect of this management that is corrective orthotics. It is very important to understand the mechanism of plantarfascitis & plan treatment accordingly.

What is plantar fasciitis?

Plantar fasciitis (plantar fasciosis) is inflammation of the plantar fascia. The plantar fascia is a strong bowstring-like ligament that runs from the calcaneum (heel bone) to the tip of the foot. When the plantar fasciia is stretched too far its soft tissue fibers become inflamed, usually where it attaches to the heel bone. Sometimes the problem may occur in the middle of the foot. The patient experiences pain under the foot, especially after long periods of rest. If inflammation persists for longer duration it can lead to calcaneal spur.

Causes: plantarfascitis is not usually caused by a single injury, such as a twist or fall, but rather the result of repetitive stress and pounding of the heel. Still following factors play important role in increasing severity of plantarfascitis.

Arch type: flat feet, high arch or in between
Footwear: wearing too flat or too high heel footwear
Lifestyle: continuous standing, walking or running
Body weight: overweight results in more stress on plantar fascia
Osteoporosis: weak bones will aggravate pain more
Diagnosis:

Podiatrist /foot doctor is the right person to diagnose & treat heel pain. Assessment should include detailed history, lifestyle evaluation, assessment of foot arch & alignment of all proximal joints, gait analysis etc. The real skill is to identify the cause of the condition and then establish the correct treatment plan.

Treatment:

Rest: temporary rest is important to allow plantarfascia to heal
Cold pack: helps to reduce inflammation. Recommended 3-4 times a day, 20 minutes each time (not recommended for diabetic patient)
Right orthotics: to support plantarfascia & correct faulty alignment
Supportive shoes: orthotics with proper supportive shoe gives long term relief
Exercises:
Generally higher arched feet require a strict stretching program and flat feet require a gradual strengthening program and more support. One should not do stretching in acute pain.

What to avoid:

Barefeet walking on hard surfaces
Wearing too flat or too high heeled footwear
Heel cushions: silicone or foam heel cushions commonly available from pharmacies do nothing to actually cure this condition and at best provide temporary comfort.
