My wife is 19 weeks pregnant I just want to know what type of exercise/yoga does she required for normal delivery? What precautions required at the time of exercise?

MD - Obstetrtics & Gynaecology, MBBS
Gynaecologist, Kolkata
Walking is the best exercise. It should be brisk walk, as far as comfortable, not just a stroll. 20-30 mins a day, 3-4 times a week. That apart there are ample breathing, back and sitting exercises during pregnancy. In yoga, don't go for difficult pose which might lead to accidents. Avoid excessive back bending. Whatever you do should be gentle and comfortable without excessive straining or sweating. When in doubt consult your doctor.

Hello doctors. I had a unprotected sex with my gf few months ago. From that day she has a irregular periods. They delay each month by 7-10 days and only for 2 days from 4 months. Is there any chance of pregnancy? We take many home pregnancy test with prega news. They always come negative. And she has no any symptoms of pregnancy. But she feel some noise like tik tikk in lower body. What is that? Sir please tell me is there any chance of pregnancy?

MD - Obstetrtics & Gynaecology
Gynaecologist, Mumbai
She is not pregnant as she already had menses after the sex which you had 4 months back. 1 week delayed menses lasting for 2 days does not need any treatment. Stress and hormonal changes can make menses come like this. There is no need to take any pregnancy test in future, and relieve her of this stress. Menses will start coming normal. Sometimes, when woman think they r pregnant, bowel movements can be perceived as some noise in the abdomen. All this is normal and does not require any tyreatment.

Yes first periods after delivery can be heavy at times. If it has settled now and there was no issues with bleeding after delivery there is nothing to worry. If your bleeding is still heavy and continuing or next periods are heavy then see a Gynaecologist.

Doctors I'm very tensed. I'm 34 weeks pregnant. and my cervix is 1.5 cm and 40% effaced. Came to know in the internal examination. Also I'm suffering from vaginal infection and Dr. has prescribed 7 days pessaries. I just want to know that how many chances are there that may go in to full term labor? Please reply.

MBBS, MCCEE, Fellowship in Sexual Medicine
Sexologist, Jaipur
Pessaries are notorious for causing infection as they stay wet for long time inside. They should be avoided. Show yourself to nearby good Gynecologist and take treatment. As for labour, it usually happens when time is ripe. So do not get uselessly worried. Relaxxx. It is important.

It's always 6-7 days late But today it is late for 12 days Am I pregnant? I took a pregnancy test on 2nd April but it came out negative Did I took it so fast? How long should I have to wait to confirm the pregnancy test with irregular cycles?

BHMS, pgdcbm, MD(AM)

If your menses are always Irregular, think of the longest time gap between two cycles that you get and then do the test after 4-5 days of the last expected date. If you got your periods on 25thfeb, next were expected around 1st April. You can now get the test repeated in 2-3 days time. Use early morning sample preferably.

Treating Infertility Caused by PCOS With IVF

MBBS, DGO, MD, Fellowship in Gynae Oncology
Gynaecologist, Delhi
Treating Infertility Caused by PCOS With IVF

PCOS is an abbreviation for Polycystic Ovarian Syndrome. It is a problem that is caused in women when their hormones are out of balance. It affects menstrual periods and can even hinder conception when aggravated to an advanced stage. Many women having PCOS develop small cysts on their ovaries. These cysts might not be harmful, but they lead to hormonal imbalances.

Common symptoms include:

  1. Gain of weight or Weight loss problems
  2. Acne
  3. Excess hair on body and face
  4. Irregularities in periods
  5. Fertility problems
  6. Depression

It is possible for most women with PCOS who fail to attain pregnancy easily to use other treatments to get pregnant. Using In vitro fertilization (IVF) is one such mode of treatment. The procedure is helpful if done within the age of forty. You should undergo IVF only in a high quality clinic under the supervision of an experienced doctor.

There is a considerable amount of risk of having a higher order multiple pregnancy due to treatment of PCOS infertility using injectable gonadotrophins. Recent developments have allowed the patients to maintain high rates of pregnancy while simultaneously controlling the risk of multiple pregnancy. A variant of the standard IVF treatment known as Blastocyst embryo transfer is useful in the control of multiple birth rates. Generally, success rates for IVF treatment in cases where infertility is caused due to PCOS is quite high.

IVF Protocols for PCOS
There are two simulation protocols that are commonly used with PCOS:

  1. Ganirelix with Lupron trigger protocol: Used to avoid ovarian hyperstimulation syndrome.
  2. PCOS down-regulation protocol

This protocol combines:

  1. Oral Contraceptive Pills
  2. Metformin

A calendar is followed for the down regulation protocol for PCOS. Before starting with Lupron, a dosage ( 500 mg, 3 times a day) of metformin is followed daily. After this, oral contraceptive pills are prescribed for around 21 days. Lupron coincides with the last 3 pills of birth control.

Lupron is a subcutaneous injection of a medicine that is taken once daily. Simulation of the ovaries is initiated after 6 or 7 days without using pills. It is very important with polycystic ovary that the dose of FSH hormone is properly controlled and that the ovarian response is cautiously monitored. Over response to the treatment can cause ovarian hyperstimulation syndrome. This should be avoided.

Dysmenorrhoea is one of the commonest female disorders at any age during reproductive period. To prevent it one can take simple medicines like cyclopam 1 at a time. If it is not better you should prefer homoeopathic treatment.

Hello doc, Can anybody pls tell me what actually is vaginitis. I get multiple responses. How can my doc confirm that I am suffering from fungal?

MD - Obstetrtics & Gynaecology
Gynaecologist,
Vaginitis is actually inflammation of vagina ie redness, pain, itching and discharge. Reason could be any. Your doctor can tell after examining you by the nature of discharge if it's fungal or some other infection. Or she can do vaginal swab culture and sensitivity.

Processed Food Vs Natural Food

Processed Food Vs Natural Food

As experience teaches us, 'sin is indeed tempting'. You are aware of the ways in which your health and quality of life would improve and similarly attentive to factors that can harm your biological system and yet you give in to the trap of temptation. Not just once, but repeatedly. Processed food can be extremely injurious to your general health.

A huge mass of people have processed food items after being familiar with its ill effects. The reason propelling them to do so is perhaps the good taste and easy accessibility of these kinds of food. They live up to your cravings at odd hours and are extremely satisfying.

Kinds of processed food you generally intake:

  1. Snacks to savor: Binge eating can make you have some of the savory snacks. In the middle of the night, you might want to munch on chips, flavored and coated nuts or on microwave popcorn. These packed snacks may have dangerous effects on a person's health and hence, should be avoided.
  2. Processed meat: Bacon, salami or corned beef should be strictly kept away from as they include the threat of artificial hormones.
  3. Veggies in a tin: Tinned or canned vegetables could be a feasible option in your fast paced life, but it clearly is not good for your health.
  4. Carbonated drinks: Carbonated drinks are enjoyable but they do interfere with a person's physical fitness.
  5. Instant noodles, soups or other meals: Ready mixes and packed noodles seem to be a simple way of making tiffin or an evening snack, but the health hazards they cause should alarm you. Processed food items such as these can lead to cancer as well.

Natural food substitutes that can be equally gratifying:

  1. Boiled potatoes sprinkled with cucumber, tomatoes, chili flakes and salt should always be preferred over potato chips.
  2. Consume fruits, especially citrus fruits regularly. Try to maintain a safe distance from packaged fruit juices as they contain preservatives and a great deal of sugar.
  3. Tofu that is made from soya is high in nutrients and should be a great substitute for chicken nuggets.
  4. One can deal with his or her cravings for mayonnaise by switching to hummus. Though the two food items don't taste the same, hummus doesn't contain animal fat that is actually harmful when stored or preserved.

BHMS

How many days your period last? It is not necessary that 4 days should be completed. If it is last only one or two days then you are suffering from oligomenorrhoea. Homeopathy can help you.

Breast Pain - Understanding the Types and Reasons Behind it

Gynaecologist, Mumbai
Breast Pain - Understanding the Types and Reasons Behind it

Breast pain, also known as mastalgia, can be termed as any type of discomfort, pain or tenderness in and around the breast or the underarm region. It can also be accompanied by a sharp pain or a burning sensation.

There are two major types of breast pain

  1. Cyclical Pain: As the name suggests, this type of pain occurs on a cyclic basis and is linked with one’s menstrual cycle. It may happen when one is going through her periods and is influenced by hormonal imbalances. This type of pain is generally not a cause of concern.

  2. Non-Cyclical Pain: This is generally not common and its cause can be linked to a variety of reasons; cancer or a malignant tumour in the breast being the most severe consequence.

Causes of Breast Pain

  1. Lumpy breasts along with soreness and pain can be caused due to fibrocystic breast tissue.

  2. Hormonal imbalance can also lead to pain or discomfort in and around the breast region. Abnormal prolactin limits can also be a contributing factor.

  3. Breast cancer can also lead to pain, mostly in the left breast. Along with the pain, it can also induce a dull ache, tightness or heaviness in the chest.

Treatments for Breast pain

The various treatments for cyclical breast pain include:

  1. Making modifications in the diet

  2. Administering Vitamin E and calcium supplements

  3. Including thyroid hormonal supplements in the diet

  4. Reducing sodium intake in your diet

  5. For non-cyclical breast pain, which is often the cause of cancer, a thorough examination is recommended. A clinical biopsy will be advised if the lumpy tissues make the tumour harder to be detected through a mammogram.

  6. If the pain is on account of an injury, then appropriate painkillers will have to be administered to reduce the pain. If you wish to discuss about any specific problem, you can consult an oncologist and ask a free question.
