Some times I have muscular pain in my shoulder that converts in acute headache at night. What should I do?

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist, Delhi
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Normally these two symptoms are no connected with each other as headache is a separate entity in itself, and if you have regular headaches, then it is wise to get yourself evaluated by a neurophysician. For your localized shoulder pain, you can start with gentle anti inflammatory medication which suits you, physical therapy with modalities initially till your pain is there, and then maybe you can switch to strengthening exercises, take adequate calcium and vitamin d supplementation, give rest to your affected shoulder for few days. And let me know in few days time how you are feeling. Do not hesitate to contact me if you need any further assistance. Thanks & regards

I am 44yrs old. I have back pain at lower portion. I am diabetic. Tell me what to do?

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist, Delhi
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. The foremost importance is to keep your sugar levels under strict control by taking medications regularly as advised by your endocrinologist. Regarding your back pain, if you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like dynamic x rays of lower back and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance. Thanks & regards

Massage with your thumbs from the mid points of the shoulder blades on both sides towards the lower back of your head up to lower level of the ears. Use oil or water for massage. Rotate your head clockwise and anticlockwise, forward and down, left and right a few times. Do for 5 to 10 times morning and evening. Give pressure the dipping point on the portion between the thumb and the index finger on the back of the palm. Press and release for 15 times where the pain is more. This will solve the problem. Further do some wrist rotation exercise. Consult after 5 days.

Sprain means you have ligament injury. It has nothing do with fractures. As you have night pain too that means you may have grade 2 or 3 sprain. Please protect your leg and take full rest. Don't do any massage on the affected area. Do not put weight on the leg. Keep your leg elevated. Do visit nearest Physiotherapy Clinic for further rehabilitation.
