I am 50 year old having problem of auto immune disease called granulomatosis with poly angitis. Diagnosed last year. Started drugs Now I am better but afraid to have the same problems again. Hi lybrate-user, I appreciate you seeking help. As I understand, treatment with chemotherapy drugs works by suppressing the immune system. Simultaneously, co-administration of steroids too are known to arrest systemic inflammation. Today is the WORLD CANCER DAY and the theme of this year is "We can, I can".... Yes we all can battle the cancer. But the irony is that majority of women ignore early signs and symptoms of cancer and do not consult doctors in time. And even some of these cancers are preventable if proper action is taken. 

Ovarian Cancer- 

  • What will you do if you feel gaseous distension (gas, bloating), acidity, constipation, change in bowel habit, decreased appetite, feeling of full stomach, weight loss and abdominal swellling? Majority of the women ignore these symptoms and take antacids before it is too late. But these are often symptoms of early ovarian cancer and thus if you ignore these symptoms, you are harming yourself. Ovarian cancer is difficult to treat if detected at late stage. But in early stage, it is curable. So, don't ignore these symptoms.
  • Who are the women at risk of ovarian cancer- women who are not having baby, who themselves or close relatives (sister/ mother/ daughter) had history of cancer in breast/ ovary/ bowel/ uterus etc. But some women are at lower risk like those using Oral Contraceptive Pills- OCP(- the birth control pills can reduce the risk by 50%) and who had tubal ligation (permanent family lanning methods).
  • If you had such risk factors, you must not avoid any such above mentioned symptoms. Rather you should consult gynecologist early. Ovarian cancer can be detected by ultrasound, CT scan, some blood tests (CA 125) and some cases biopsy may be needed. And remember, majority of the ovarian diseases are NOT cancer but you must be sure that it's not cancer causing your problem.
  • If ovarian cancer is diagnosed, the treatment is surgery (to be done by expert personnels) and most of the women need chemotherapy.
  • Ovarian cancer is common in elderly women but it can afect younger women also. So, all the women should take care of the above mentioned symptoms

 

Cancer of Cervix: Cervix is the mouth of the terus. In our country, this is the COMMONEST CANCER in GYNAECOLOGY affecting women whereas it is uncommon in the Western countries. The irony is that women die of this disease whereas it is TOTALLY PREVENTABLE

  • It is caused by a virus (human papilloma virus- HPV) that is sexually transmitted (so afects sexually active women). But it has a peculiar feature that before cancer actually occurs, there is precancerous stage- when the cells of cervix shows some changes that we can detect and treat. So, if women regularly consult doctor (WHEN THEY DO NOT HAVE PROBLEMS), we can check if she is at risk of cervical cancer or not, by some simple tests (Pap Smear, HPV testing etc). Is such changes are found (that is not cancer) we can treat her so that she does not develop cancer. These method of detection, before disease occurs or before an individual feels that she is having disease, is called "Screening". 
  • Another method of preventing it is vaccination of HPV, that should be given to the adolescent girls before they become sexually active. Even it can be given in older women (who are sexually active), although the response is somehow lower than if given at early age. But vaccination is not alternative to screening. SO, both screening and vaccination should be done to prevent this cancer.
  • Even if detected at early stage, it is totally curable. So, women should not ignore symptoms of excessive vaginal discharge (remember, all excessive discharges are not due to cancer), abnormal or heavy menstrual bleeding, bleeding occuring unredictably in between menstrualtion, bleeding after intercourse etc.
  • It is treated by surgery and some cases may need radiation also.

 

Cancer of uterus

  • Cancer of uterus is common after 50 years of age (although can occur at early years also). It is the commonest gynaecological cancer in the Western World. It mainly occurs in women who are obese (excessive weight), having diabetes or hypertension (high blood pressure), not having any baby, who are having personal or family history of cancer in uterus/ breast/ ovary/ bowel. 
  • It can be prevented by taking OCP (birth control pils) and controlling weight. In many cases, it is also preceeded by some precancerous changes (hyperplasia) that can be detected by ultrasonography and if needed small biopsy procedure.
  • The common symptoms are abnormal heavy bleeding and bleeding after attaining menopause (after totall stoppage of menses). So, any abnormal menstrual bleeding must be properly investigated.
  • It is usually well curable if treated early- by surgery and in few cases, may need radiation, chemo or hormone therapy.

 

Cancer of Vulva: The external genitalia of female is known as vulva. Cancer in vulva is relatively uncommon but still it can happen, especially those who are having diabetes, HIV positive or those having poor hygiene.

  • Never ignore any symptoms of excessive itching in your private parts. It may be the early symptom of vulval cancer. Some cases, patient may have ulcer or tumour or bleeding.
  • It can be detected easily by examination by doctor and then taking biopsy. It is treated well with surgery and some cases, radiation may be needed.

 

Sumary: Majority of the gynaecological cacers are preventable. They can be diagnosed early if women consult doctor at early stage. Majority of them respond to treatment.

Symptoms;--generally pain is main symptom but mainly --------- depend upon the organs involved. Here other symptoms can be -bleeding, obstruction, ulceration, fever, cough, vomit weakness, loss of appetite, indigestion etc. Etc.

My mother has a lump in her left chest since one year ,could you suggest what kind of test should be done to know weather it is normal or any other problem. And recently she has undergone a gynaecological surgery for uterus removal just one week before.

You should show your mother to the same gyane or a surgeon to consult for the lump. Clinical examination asking with mamography and pathological examination (fnac) can diagnose upto 99% cases.

I am a 65 year old female and had a radical mastectomy, as there were traces of cancer which has now spread to the liver etc. Do I have a chance without chemotherapy or radiation? Diet and other health care possibility?

Dear lybrate-user, if the liver was involved it becomes a stage 4 cancer. Since local disease has been removed by radical mastectomy you need some form of treatment for metastasis. We should know the other parameters of the biopsy like hormonal status. If positive you will need convenient chemo or hormone therapy.

We should ideally evaluate the extent of problems with your ovaries. On one side we need to do various blood tests to assess the level of your fertility hormones. On the other hand we need to do blood tests to evaluate presence of ovarian tumour. A mri should be done ideally as it delineates better, what exactly is the problem with the ovaries and also is able to define the tumour better. Nowadays there are many techniques for fertility preservation. We need to evaluate the situation fully to decide on the best course of action.

Since when you have this condition? to help control the symptoms of an enlarged prostate, try to: limit beverages in the evening. Don't drink anything for an hour or two before bedtime to avoid middle-of-the-night trips to the toilet. Limit caffeine and alcohol. They can increase urine production, irritate the bladder and worsen symptoms. Limit decongestants or antihistamines. These drugs tighten the band of muscles around the urethra that control urine flow, making it harder to urinate. Go when you first feel the urge. Waiting too long might overstretch the bladder muscle and cause damage. Schedule bathroom visits. Try to urinate at regular times such as every four to six hours during the day to" retrain" the bladder. This can be especially useful if you have severe frequency and urgency. Follow a healthy diet prescribed by registered dietitan. Obesity is associated with enlarged prostate. Stay active. Inactivity contributes to urine retention. Even a small amount of exercise can help reduce urinary problems caused by an enlarged prostate. Urinate and then urinate again a few moments later. This practice is known as double voiding. Keep warm. Colder temperatures can cause urine retention and increase the urgency to urinate. Do reply back for medication prescriptions. People's daily reported that cancer has become the number one threat to human life. It is a common and frequently-occurring disease which seriously endangers the health of people. Cancer experts have pointed out that, besides the genetic factor, cancer is also highly related to the environmental factors, in which diet is one of the most important.

Relevant researches have pointed out that about one-third of the cancer diseases has something to do with the poor eating habits in daily life. With the gradual improvement of the living standards, people tend to eat more refined foods at the meal, while the intake of coarse grains and cereals is reduced. Such modern poor eating habit will lead to a series of cancers, which seriously affect the health of human body.

For example:

  • First, lack of cellulose will increase the incidence of colorectal cancer.
  • Second, excessive intake of high-fat, high-protein foods has become an important reason for the increasing incidence of breast cancer and colorectal cancer.
  • Third, about 75% of the head and neck cancer is caused by drinking alcohol and smoking.
  • Fourth, people who like to eat hard, hot, and pungent foods will easily suffer from esophageal cancer.
  • Fifth, gastric cancer has something to do with too much intake of smoked foods or foods which contain nitrate, such as sausage, ham, pickles, dried salted fish and so on.
  • Sixth, eating mildewed corn, peanut, rice and other foods or drinking unsanitary water will make people susceptible to liver cancer.
  • Seventh, people who prefer to eat sweet and greasy foods are most likely to suffer from pancreatic cancer.
  • Eighth, insufficient intake of vegetables and fruits has something to do with colorectal cancer, gastric cancer, liver cancer, breast cancer and esophageal cancer.

Plenty of rest, high protein nutritious diet and she should stay positive. Avoid raw fruits and vegetables during chemotherapy and for 10 days after that. You can stew the apples for 15 sec in the microwave and give. If she has vomitting don't stop diet. Give Cerelac which is baby food hence easy to digest. Add black salt to all juices.

What does cervical lymph nodes signify? Is that a indication of cancer and what is reactive lymph node hyperplasia?

Each organ of our body has lymphatic supply like we have blood supply which drain into group of lymph nodes. These lymph nodes get enlarged when ever there is some trouble to primary organ they are draining. This may be due to infection, inflammation, cancers etc anything. Cervical lymph nodes drain mostly head and neck region. Reactive hyperplasia is non cancerous enlargement of lymph nodes may be due to some infection or inflammation. Need not worry.

I hope you got your wife properly examined. There can be many causes of white discharge. Even cervix cancer can be detected by proper examination and a pap smear test. Your wife will get cured if properly taken care of and you should not loose hope. Take her to a good gynecologist for examination who will properly examine her and ask for necessary investigations to rule out the problem.
