Osteoarthritis - How Best to Manage it?

The human body is able to move fluidly because there are various joints, muscles, tendons and cartilages that work together to make this happen. Movements of the hands, legs, back, hips, knees and wrists are controlled this way. For various reasons, however, these tissues grow stiff, thereby, reducing free movement. Any movement causes pain and stiffness and swelling of the joints, which forces the patient to reduce physical activity. This reduced activity further leads to the joints becoming stiff, thus putting the patient in a difficult position.

What causes it?

The ends of the bone are covered by softer tissue called cartilage, which is responsible for allowing the free movement. In patients with osteoarthritis, this cartilage is lost and the bones rub against each other, producing pain and a crackling sound with the movement. Some of the most common causes include the following:

  1. Age is the most common cause for osteoarthritis, with lifestyle habits playing a major role in this condition. Most joints would be affected here.
  2. Obesity is another major cause for osteoarthritis, as the additional weight of the person puts extra pressure on the knees and the legs, leading to the wearing of the joints. The back and lower extremities are more affected than the hands in this case.
  3. Joint injuries are another common cause for osteoarthritis, which is more localised to the area of injury.

What are the signs and symptoms?

As noted already, painful movement is the most common symptom of osteoarthritis. There could also be swelling and redness around the affected joint(s).
Diagnosis: The doctor will usually be able to identify osteoarthritis after an examination, but in some cases, an X-ray or MRI may be required to confirm the diagnosis.

Treatments available

The treatment for osteoarthritis is multipronged.

  1. For immediate relief of the pain, non-steroidal and anti-inflammatory drugs are used. In cases of localised symptoms, even topical creams or gels may be used.
  2. Weight loss is extremely essential, if there is obesity that is causing the osteoarthritis.
  3. Vitamins and supplements in good amounts can improve bone health and help in preventing and reversing the damage in some patients.
  4. Heat therapy has proven to be quite useful in many cases to provide temporary relief
  5. As much as the movement of the joints is painful for the patient, it is only with regular exercise that the joints can be made more flexible and mobile.
  6. The affected joints need to be specially cared for by avoiding injury and reducing unnecessary movements.
  7. Role of physiotherapy in the management of OA - Strengthening exercises, Taping, Manual therapy, Education/lifestyle changes,  Modalities, Agility and perturbation training.
  8. The definitive therapy of OA is joint replacement (total/partial knee/hip joint replacement).

In addition, support groups and counselling may also help significantly in patients who find their quality of life being reduced due to this condition. Remember, not all is lost with osteoarthritis. It is definitely possible to get your life back to its earlier days when you were able to move about freely.

You might be an elite athlete or just a commoner. There is always a possibility for you to develop back pain while carrying out your daily activities. You might not be aware, but regular activities—like sitting at the computer, running on the treadmill or simply crawling into your bed for a good night's sleep—can affect the health of your spine. You are always at the risk of damaging your spine. These damages lead to back pain. It has been found that faulty postures can cause back pain by causing straining of the back muscles, injuring the tendons and damaging the vertebrae or ligaments. Back pain can be caused due to the overuse of a particular activity or for undertaking a new activity. Besides all these possibilities, most people put the blame on their discs for back pain.

What Are The Discs?

The human spine is composed of 24 bony vertebrae. These are placed one on top of the other. There is a thin intervertebral disc between each vertebrae. These discs look like jam doughnuts. They are made up of a hard outer shell known as the annulus fibrosis. They have a soft, jelly-like inner core known as the nucleus pulposus. They are shock absorbers.

What Happens To The Discs When An Injury Occurs?

The jelly-like soft core in the discs can often get shifted as a result of the accident or injury. Thus, the discs become a reason behind back pain. The pain could be due to the problems with the disc while carrying out tasks like —

  1. Prolonged sitting

  2. Bending

  3. Lifting

  4. Twisting

  5. Pulling

Symptoms Of Disc-Related Pain:

Symptoms tend to vary due to the nature of the injury and the damage caused. However, some common symptoms have been listed below:

  1. Localised back pain

  2. Pain in thighs, lower legs and feet

  3. Tingling sensations

  4. Numbness or muscular weakness

Treatment-

It is best to consult a doctor to know about the treatment needed for back pain related to disc. Initial treatment after detection of the problem includes the following ways:

  1. Intake of non-steroidal anti-inflammatory and pain medication.

  2. Application of heat and ice packs.

  3. Physiotherapy.

  4. Home exercise program as suggested by the doctor.

  5. Practicing of core and lumbar strengthening exercises.

One must stop movements that aggravate the pain and don’t take difficult physical activities. You must take adequate rest to set things right.

Do You Need Any Surgery?

It is not always needed for all disc related problems. Most issues with the disc vanish after a week. Surgery is sought only after nonsurgical treatments prove useless. The decision must be taken by a doctor. If you wish to discuss about any specific problem, you can consult a physiotherapist.

Go for fasting once a week. You can go for absolute fasting or rely on a liquid diet to digest ama (toxin). Fasting should be followed by hot fomentation or by taking a bath with lukewarm water or steam bath. After fasting and fomentation, mix powdered long pepper, black pepper and dried ginger in equal quantities and take 1 teaspoon of this powdered mix with lukewarm water twice a day. This will help you restore your digestive fire. Take one tablespoon of castor oil before going to bed, once or twice a week. It helps in cleansing ama (toxin) build up and balancing vata. Mix equal quantities of powdered fenugreek seeds, turmeric and ginger powder. Take one teaspoon daily in the morning and evening. For dry fomentation of the affected joints, take a cloth and fold it into four, fill it with hot and sand make a potli (bag) out of it. Use this bundle for the fomentation of different joints. This is very helpful in reducing pain and swelling.

Requires detailed investigation and examination. Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomantation paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. Do not ignore it. Let it not become beginning of a bigger problem. Make sure that patient is not allergic to any of the medicines that he/she is going to take. If it does not give relief in 4-5days, inform.

Knee pain- both legs, since nov 2016 Reports - xray, protein, calcium, vitD, vit b12 all good. Previously vit d nd b12 deficient ESR is 10 Inflammation level - fine took medicines for that in dec for one month IRON LEVEL 38 Diagnosis - muscles weak, doing stretching exercise now Please suggest for what could be d possible reason for such severe pain. Hello Lybrate user. It seems that you hav done the related diagnosis. As you mentioned above ther r possibility you have developed early osteoarthritis in both knee. Increase intake of natural sources of calcium n protein. Reduce all activity which r pain aggravating. Apply cold packs or icing for 20 min 2 time a day .do quadriceps static n slr exercises. Put towel roll under your inme ,press roll down using knee ,it's surrounding muscles. Hold it for 15 sec for 10 repetition for thrice a day n do hamstring stretches. Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily - lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana – Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day. Back Stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day. Do the cat/cow stretch. Get on all fours, with your arms straight and your hands directly under your shoulders; your knees hip-width apart.

Sir I am facing problems of memory loss and concentration lacking. This is due to side effects of allopathy antidepressant orange 5 mg medicine that I was taking for last 8 years. Now I am not taking. I have been advised to take ginkgo biloba 1x. It is improving my symptoms. Continue gingo biloba 1x tabs for 3 months. After that you take the medicine alternate days for a month. Then once in 3 days for a month. Then weekly once for 2 months. Then you can stop. Immediate stoppage can reverse the symptoms. Tapering the dose is the correct method. And also you must think positive always that my memory is good. Everything become alright even if I stop my medicines. Be cool be relaxed. Nothing to worry.

Get some relax during the work and walk for 15 min. In between the work. Intake healthy diet and green leafy vegetables. Drink 1 glass of milk every day with half table spoon of turmeric. Do cervical exercises for that. And not use pillow at night.
