In 2008 I was suffering from anxiety disorder and my psychiatrist suggest me paxidep 37.5 for 5 years, after 5 years when he decreased the dose from 37.5 to 25 I again feeling frustrated so he advised me take it one more year same dose. After one year same problem arise, my question is that I am taking this medicine from last 8 years which is affecting my sexual life as side effect, how I can leave this medicine? I am doing night shift from last 8 years and drink on weekend.

Anxiety disorders may at times need long treatments. Paxidep cr may cause sexual problems in some individuals but solutions for these depend upon the side effect experienced. If this side effect is not correctable then you have an option of switching to better alternatives for anxiety. However since you have experienced relapses whenever you tried reducing doses of medications it seems difficult to completely stop these medications, particularly paxidep for anxiety. You need to gradually shift to other medications which can then be stopped gradually later on.

Why our hand and legs getting yellow in jaundice?

MD - Alternate Medicine, BHMS
Homeopath,
It is actually the whole phenomenon, but I will try to make you understand as below in simple terms: Old red blood cells travel to your liver, where they?re broken down. Bilirubin is the yellow pigment formed by the liver?s breakdown of these old cells. Jaundice occurs when your liver doesn?t break down bilirubin the way it is supposed to. Your liver might be damaged and unable to perform this process. Sometimes, the bilirubin simply can?t make it to your digestive tract to be eventually removed through your stool. In other cases, there may be too much bilirubin trying to enter the liver at once or too many red blood cells dying at one time. So this bilirubin appears in the skin of extremities. Jaundice in adults is often indicative of: alcohol abuse infections liver cancer cirrhosis (scarring of the liver) gallstones (cholesterol stones made of hardened fat (lipid) material or pigment stones made of bilirubin) hepatitis (swelling of the liver that decreases its ability to function) pancreatic cancer parasites in the liver blood disorders, such as hemolytic anemia (a decreased amount of red blood cells in the body, which leads to fatigue and weakness) an adverse reaction to or overdose of a medication, such as acetaminophen Jaundice is also a frequent occurrence in newborns, especially in babies who are born prematurely. An excess of bilirubin may develop in these newborns because their livers haven?t fully developed yet.

Leg exercise kijiye daily I hope you will fit and fine……………………………………………….Basic Leg Exercises The following Basic Leg Exercises should generally be performed 1 - 3 times per week provided they do not cause or increase pain. Ideally they should not be performed on consecutive days, to allow muscle recovery. As your strength improves, the exercises can be progressed by gradually increasing the repetitions, number of sets or resistance of the leg exercises provided they do not cause or increase pain. Swiss Ball Squats Begin this leg exercise in standing with your feet shoulder width apart, your feet facing forwards and a Swiss Ball placed between a wall and your back (figure 1). Slowly perform a squat, keeping your back straight. Your knees should be in line with your middle toes and should not move forward past your toes. Then slowly return to the starting position maintaining good posture throughout the exercise. Perform 1 - 3 sets of 10 repetitions provided the exercise is pain free Lunges Begin this leg exercise standing with your back straight in the position demonstrated (figure 2). Slowly lower your body until your front knee is at a right angle. Keep your front knee in line with your middle toe and your feet facing forward. Then slowly return to the starting position maintaining good posture throughout the exercise. Perform 1 - 3 sets of 10 repetitions provided the exercise is pain free Hip Abduction Sidelying Begin this leg exercise lying on your side in the position demonstrated (figure 3). Keeping your back and knee straight and foot facing forwards, slowly take your leg upwards tightening the muscles at the side of your thigh / hip (abductors). Hold for 2 seconds and then return to the starting position. Perform 1 – 3 sets of 10 repetitions on each leg provided the exercise is pain free. This exercise can be progressed by adding an ankle weight to your ankle for increased resistance. DEAR………. Shoulder pain Physiotherapy uses a number of different physical methods to promote healing. If you are referred to a physiotherapist, they should explain to you what treatment they will use and how it will work. Possible treatments include: •specific shoulder exercises – for example, if you have shoulder instability, you may be given exercises to strengthen your shoulder •massage – where the physiotherapist uses their hands to manipulate your shoulder Read more about some of the different techniques used inphysiotherapy. Shoulder exercises If you have shoulder pain, it's important to keep your shoulder joint mobile with light and gentle movement. Not using your shoulder can cause your muscles to waste away and may make any stiffness worse. Therefore, if possible, you should continue using your shoulder as normal. If your shoulder is very stiff, exercise may be painful. Your GP or physiotherapist can give you exercises to do without further damaging your shoulder. You may be given exercises to do on your own, or you may complete the exercises with supervision from your GP or physiotherapist. You may also have manual therapy, where the healthcare professional moves your arm for you. Manual therapy uses special techniques to move the joints and soft tissues in your shoulder. One review of a number of studies found that long-term physiotherapy was as effective as surgery for impingement syndrome (any type of damage to the tendons in the rotator cuff). I HOPE U WILL FITT AND FINE.

I am having this problem but every time I asked my doctor, he ignored it saying you are single no need to worry. This problem will be over after you get married but I still have so much of doubts regarding this. My problem is actually related to my sexual life. Let me tell you that I am still virgin and have not indulged in anything other than just kissing a girl. Now to start with the problem. Whenever I get erection after watching some adult movies, I am not able to control my self for long and there is something sticky in my underwear and then I get the urge to pee. If I masturbate, I get the urge to pee after masturbation. Is it normal. If I am with a girl and I get the erection but after some time there is a leakage in my underwear and I get the urge to pee and if I delay the activity of going to washroom then I feel pain in my lower abdomen. One more problem is whenever I go to pee. My bladder is never full empty I feel. And even after peeing when I get up. There is some drops of urine drop in my underwear and when I discussed with my doctor about this he said you have active bladder and no need to worry. I do not know whether both the problems are related to each other or not but I would like to get some idea and advice from the best doctor. Best Regards,

Dear lybrate user. I can understand. Both thd problems you are mentioning here are normal and are not related in essence. You see liquid when you are sexually excited. This liquid is not urine but a lubricator called precum. This lubricator helps in easy vaginal penetration. Urination urge is normal after masturbation. Don't worry. Take care.

Role of Neurosurgery in Management of Stroke

MCh - Neurosurgery, MBBS
Neurosurgeon, Chennai
Role of Neurosurgery in Management of Stroke

A stroke may be called a brain attack. It may happen to any person at any time. A stroke occurs when blood supply to the brain is cut off. The brain cells become oxygen deprived due to the stroke and eventually die. Dead brain cells result in brain dysfunction due to which patients lose control over muscle and memory.

Strokes can be mild to life threatening and requires immediate medical attention. Stroke requires advanced care and should be treated by extensively qualified and experienced medical experts.

Neurosurgery satisfies all the required clauses for treatment of stroke and accounts for being one of the best ways for management of stroke. Renowned hospitals have a dedicated neurosurgical wing and a team of neurological specialists who know how to deal with stroke the best.

Vascular Neurology
This form of neurology focuses on patients who have undergone a stroke or have experienced symptoms of stroke. A multidisciplinary approach is taken for the prevention of recurrent cerebrovascular conditions.

Types of stroke

  1. Ischemic stroke: This kind of stroke occurs when there is a blockage in a blood vessel, which supplies blood to the brain. This form of stroke is the more common. The underlying cause of this stroke is known as atherosclerosis, which is a condition where fatty deposits occur in the walls of the blood vessels and cause blockage in them.
  2. Hemorrhagic stroke: This type of stroke occurs when a blood vessel gets weak and ruptures into the brain. This is caused by two types of weak blood vessels known as aneurysm and arteriovenous malformation.
  3. Transient ischemic attack: This type of stroke is also known as a mini stroke. These attacks occur because of blood clots. The blockage caused is temporary or transient in nature.

Treatment of Ischemic stroke

  1. Tissue Plasminogen Activator: This is one of the best ways to treat ischemic strokes. This medicine is given to the patient intravenously. It dissolves the blood clot and improves blood flow to the area of the brain which is affected. The medicine should be given within three to four hours after stroke symptoms appear.
  2. Endovascular procedure: This is a process by which the blood clot is removed using a catheter, which gets inserted into the area of the blocked blood vessel. It helps in restoring blood flow to that area.

Treatment of Hemorrhagic stroke

  1. Endovascular procedure: Several endovascular techniques and methods are used so that the weakening of blood vessels is reduced and less bleeding is caused.
  2. Surgical treatment: Different modes of neurosurgery may be undertaken to stop bleeding and for relieving pressure within the skull.
