My mother, 78 years, has been diagnosed with can pancreatitis and malignant ascites. Doctors have only suggested palliative care and symptomatic treatment to improve her quality of life. We want to have suggestions on 1. What should be the best nutritional diet and what supplements need to be given. 2. When do you suggest paracentesis or ascites tapping. Is it helpful? Thank you very much for all your help and advice.

MCh HPB Surgery, Fellow, European Board of Surgery, MS - General Surgery, MBBS
Gastroenterologist, Guwahati
Give her normal diet according to her choice and tolerance. You can give a multivitamin supplement and also any protein powder like Ensure etc. Tapping is required if ascites is so much that it hampers her breathing especially when lying down

I am suffering from celiac disease. How can I get rid off it. What should I do please help me.

PDDM, MHA, MBBS
General Physician, Nashik
There is no cure for coeliac disease, but switching to a gluten-free diet should help control symptoms and prevent the long-term consequences of the disease. Even if symptoms are mild or non-existent, it is still recommended to change your diet, as continuing to eat gluten can lead to serious complications. It is important to make sure your gluten-free diet is healthy and balanced. An increase in the range of available gluten-free foods in recent years has made it possible to eat both a healthy and varied gluten-free diet.

His diabetes is not properly controlled. Diabetes affects all organs in the body ? Eyes, kidneys, nerves, reproductive system all are affected, In fact, from toe to hair. For diabetic control, patient should be under proper guidance for regular follow up and advice. It is not as easy task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (Self-Monitoring of Blood glucose ? SMBG). Though difficult, this can be done by a glucometer and keep a record. Get an idea of Fasting sugar (FBS) and 2 hours after food (PPBS), & HbA1C (inform me) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ Insulin will be needed. There are different diet plans of varying calories for each person depending on his activity & lifestyle.

Sequence #1 Begin by sitting on your heals with knees bent. To soften any strain on your knees place a yoga block or folded blanket between your feet. Using your eyes to guide you, revolve your body to the right, looking over your shoulder. Hold and breathe for a count of 10, return to the center and revolve to the left in the same manner. Return to center. Extended Child Pose Extend your hands and arms forward into pose of the child. Try to place your buttocks on your heels. If this is too difficult open your knees wider and keep your feet together as you extend. Continue to breathe normally without forcing or “trying” to breathe. Sequence #2 Lie on your back, legs extended out and bring your right knee in towards your chest. Interlace your fingers around the knee.Elbows are lifted up and out to the side to keep the lungs open.Without forcing the knee, round up and gaze down the length of the extended leg. Hold for the count of 5 then roll down. Lower the right leg and repeat with the left leg. Bring both knees into your chest, and then extend away to arms length. Repeat in and away several times. This is a great massage for the lower digestive system, as well as, your lower back. Repeat the entire sequence 4-5 times then extend the legs out and relax in Savasana or resting pose. Sequence #3 Lying on your back, bring both knees in toward your chest and wrap your arms around your knees as much as possible. Hold for the count of 10, breathing naturally. Release your knees and keeping them bent, move them out and over your hips. Extend your arms out to the side and ground your shoulders and palms of your hands to the floor. Slowly lower your feet and knees over to the right, keeping your shoulders and palms grounded. Gaze towards the ceiling. You can make the rotations with knees bent and feet off the floor or an easier way is to keep the feet flat and together on the floor and lower from one side to the other. Return to the center and lower your bent legs over to the left. Move your legs from right to left inhaling to lift and exhaling to lower for 4-5 times. On the final rotation stay and hold on the right side for a count of 10. Then lift up and over to the left holding for 10 counts. Come back to the center, extend your legs and rest before repeating the sequence.

Actually it is for my grand mother who is 55 years old and she has a problem that blood falls with stool for 4 month. She has many dots in mouth. Then we get her to a md. Nd the first in the test it seems malina but in the endoscopy it seems nothing. What should we do in this situation.

MBBS, MS - General Surgery
General Surgeon, Bangalore
Mr. Lybrate-user, I think your grandmother may be having haemorhoids or piles, consultation with a surgeon can rule it out. If that turns out to be negative, she may require a colonoscopy to rule out bleeding from rectum & colon.

I have done my stool test. Dysentery is found. What should I do now? Shall I go for homeopathy or allopathy? Which will work faster? Because it is diagnosed too late. Earlier the doctor diagnosed my situation as Irritable Bowel Syndrome. So I am taking medicines for IBS for two months. But no relief at all. 70 days gone, I had a severe abdominal pain. I can not walk, can not move, can not low down or bend because of pain in my intestines. My exam are coming, my future will be spoiled if not treated soon. All my ultrasounds, x-ray, blood sugar, BP, urine is normal. Except 4.9 mm right kidney stone. So, the doctor takes it very lightly. But after two month when I done my stool test, mucus is found. Help me please.

I have problem of hair failing and gastric from last three week. Please advice me what should I do?

I have problem of hair failing and gastric from last three week. Please advice me what should I do?
For a comprehensive, hair fall prevention regimen, you need to be equipped with a diet plan that includes foods that can arrest hair fall and stimulate better scalp health. Recommendations here include eating more seeds and nuts (almonds and peanuts) along with green leafy vegetables like spinach and sprouts of a variety of dals (legumes). Basically, foods rich in calcium, protein and iron are vital for ensuring overall health of the scalp. Combine this with some basic dietary supplementation, i.e. using over-the-counter products that help to ensure wholesome nutrition for your hair. Regular intake of Amla juice ensures adequate amounts of Vitamin C but you can boost this with eating more guavas. Ensure you take a multivitamin that combines beta-carotenes, Vitamin B complex, Vitamin E and trace amounts of zinc and iron.

Recurrent anal boils is to be investigated further. I advise you to take medicated sitz bath (sitting in lukewarm tub of water for 10-15 min three-four times daily) please consult a doctor for thorough investigation.

Irritable Bowel Syndrome Treatment

The medical community has not yet determined the cause for Irritable Bowel Syndrome. A combination of treatments may help a sufferer cope with the symptoms; however given the diverse symptoms of IBS sufferers; diarrhea, constipation, pain, bloating, etc, and it's chronic nature, none of these treatments at present will ultimately cure IBS.
IBS is a non-life threatening illness. It does not progress or increase your risk of developing Inflammatory Bowel Disease or Cancer. Treatment focuses on the relief of symptoms so you can live your life as normally as possible.

There is much anecdotal information on the internet as to what may relieve IBS symptoms. We recommend that before starting any new dietary, medical food or over the counter (OTC) remedy, that you consult with your physian. Physician's rely on risk and benefit decisions based on reproducible research data when they suggest a treatment for someone suffering from IBS.

Based on your physician's diagnosis after the appropriate medical tests, treatment, in our opinion, may be different for those suffering from mild, moderate or severe symptoms; however, they may be summarized as follows.

Mild: manage stress and make changes to your diet and lifestyle
Moderate: As per Mild + fiber supplements, over-the-counter (OTC), anticholinergic, or like, medications.
Severe: As per Moderate + tricyclic or SSRI antidepressants, or 5-HT3 or 5-HT4 or Chloride channel activator or guanylate cyclase-C (GC-C) agonist medication.

Your physician may recommend one or more or none of these treatments. See your physician for all recommendations for treatment. It is very important to follow-up with your physician. Refer to the medication page for an overview of medications for IBS.

Treatments:
Role of stress and visceral hypersensitivity
Role of diet
Prevention
Self Care
Coping Skills
Alternative Medicine
Prescription Medical Food

Specific Treatments Now and the Future:
Cognitive Behavioral Therapy (CBT) and Hypnosis: There is research evidence that IBS symptoms respond favorably to other therapeutic approaches such as cognitive behavioral therapy (CBT) and hypnosis. As IBS is a chronic condition, with an ongoing fluctuating course, these treatments may help the individual to develop skills for managing the condition over the long haul. Many people with IBS also have a co-existing anxiety or depressive disorder, the symptoms of which also benefit from these types of treatments.
Probiotics: Probiotics (good bacteria) which contain the strain bifidobacterium infantis 35624 have been shown to relieve abdominal pain, bloating, urgency, constipation or diarrhea. Align and VSL#3 are marketed over-the-counter (OTC) with this strain.
5-HT3 antagonist: Lotronex (alosetron), is used for the control of pain and diarrhea associated with IBS in women (IBS-D). It was removed from the market at the recommendation of the United States FDA in Nov. 2000 and re-introduced in June 2002.
5-HT4 receptor agonist: These medications help relieve the symptoms of constipation and pain associated with IBS and constipation (IBS-C), and chronic idiopathic constipation in women. Zelnorm/Zelmac (tegaserod) was approved by the FDA in the United States. Effective April 2, 2008 Zelnorm is only available to patients in the U.S. Under emergency situations.
Chloride channel activator: These medications help relieve the symptoms of constipation and pain. Amitiza (lubiprostone) was approved by the FDA in the United States for IBS with constipation and chronic idiopathic constipation.
Guanylate cyclase-C (GC-C) agonist: These medications help relieve the symptons of constipation and abdominal pain. Linzess (linaclotide) was approved by the FDA in the United States for IBS with constipation and chronic idiopathic constipation.
Antibiotics: There is some research evidence suggesting that IBS symptoms may be caused in part by an abnormal growth of bacteria in the small intestine, referred to as Small Intestinal Bacterial Overgrowth (SIBO). A specific regiment of antibiotics, namely Rifaximin (xifaxan), have been investigated to treat this condition.
Prescription Medical Food: These products are regulated by the FDA under the Orphan Drug Act and are meant to be used under physician supervision. One prescription medical food is indicated for the clinical dietary management of IBS-D under physician supervision as part of ongoing medical care for a specific condition or disease. Some patients due to chronic conditions, diseases or specific drug therapies have an impaired ability to ingest, digest, absorb, or metabolize food and certain nutrients. This medical food product is prescribed to help people manage their bowel problem.

Dear , you may have threadworm infestation where itcing at anus is associated withallergic reaction around anus. So get stool examination done. If possitive then tratment is simple. This infestation self perpetuates through anus to mouth route so be careful to wash hands thoroughly after toilet and also after every scratch at anus. Dr. Kiran mehta.
