I am 19 years old and I am having lower back pain (disc herniation l4-l5. I suffer pain everyday. What shall I do?

Kindly show me a photograph of the affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful use no pillow under the head. Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, contact me again. Do not ignore. It could be beginning of a serious problem.

My mri shows. Posterior disc prolapse between l4 l5 causing minimal thecal sac and neural foramina compression my doctor suggest me regular walking. What can I do for full recovery from the disc bulge.

Hello lybrate-user, l4-l5 disc prolapse common yea you can continue walking avoid sitting for prolong period ,lifting heavy weight, floor activity and start core stabilising exercises if you proper physiotherapy protocol you don't have to worry at all and you will get full recovery.

This is quite a common condition rule out diabetes & vit. D deficiency. Sleep on a hard bed with a soft bedding on it. Use no pillow under the head. Any way take caldikind plus 1 tab daily for 10days paracetamol 250mg tds x 3 days contact me after that. Make sure you are not allergic to any of the medicines you are going to take. It may have to be further investigated.

Good morning, I am Dr Dinesh Singh I am director predation oncology at Max hospital, Vaishali and today I will be talking about the most common cancer of women in India, it is breast cancer and there is no age bar. We have seen patients as young as 17 to 18 years and as old as 80 to 90 years. So every woman is at risk of getting the breast cancer, now which are the ladies which are more prone to get breast cancer when those ladies who do not have the breastfed their child, those who are very obese or those who have history of a breast cancer in their first relative like in the father, mausi, sister they are at higher risk, but then any population is at risk of developing cancer. What we need to understand is that we should be aware about our body and we should do a self breast examination every month, so that we know the change which happens in our breast over a period of time and once you notice that there is a lump in the breast, usually, pain is not initial symptom of Cancer any cancer in the body and whenever you notice that there is a lump in the breast then it has to be further investigated. The age is less than 40 years we go for an ultrasound of the breast which is more than 40 years we go for mammography of the breast. And then further investigation like a biopsy of the lump and if it is found cancerous, then we do for the test to see whether it is spread to another part of the body or not. Well, that tells us about the stage the breast cancer is and then depending upon the stage the treatment is decided. That let us say that we diagnose breast cancer at a very early stage that means it is less than 3 to 4 cm and at that time we do not need to remove the entire breast of the person to cure the person, we can remove only a portion the cancerous portion plus a little bit of arching around the tumor and in the axilla we have to do surgery to get out the nodes. For that now it is a sentinel node biopsy is done what happens in that is that we identify the draining lymph nodes we take them no we study them then and there when the patient is still under anaesthesia and if they are not cancerous then we do not take out the further lymph nodes of the axilla, if they are cancerous then we take them out. After that then the further treatments depends upon the final histopathology report of the tumor, if the tumor is very small If in case very sensitive to hormones these patients don't need any chemotherapy, radiotherapy becomes a part of the every breast conservation surgery and if that tumor is larger or it is not sensitive to hormones then we have to give chemotherapy. In chemotherapy now very latest medicines are available there are called targeted therapy and the cure rates have become very high. All the cancers especially breast cancer the side effects are very minimal, we have all the medicines which can take care of the vomiting, the infection and other side effects which are happening along with the chemotherapy. And regarding radiotherapy, the latest technology is targeted radiation or image-guided radiation along with other things we are able to do a very pinpoint radiation to the area where it should go. So that it saves the underline normal structures from getting exposed to radiations the cure rates become very high and the side effects are very low. So I will say that all the girls and all the ladies they should do a self-examination try to be familiar with their normal body oncology, diagnose cancer early to treat it properly and be the cure of cancer. Cancer is the one of the biggest threat to the young Indian population because of the factors that cause cancer, but also due to late detection. However, certain bad habbits and the factors increase the risk of cancer in India.

Causes of cancer in India:

  1. Overpopulation and the problem of nutrition: As per records, India is the world's third most populous country. However, it lacks in resources to feed the multiplying number of mouths. Nutrition plays a key role in deciding the quality of a person's life. Nutrition has therefore emerged as an essential branch of research and medical care in the last few decades. Lack of nutrition directly results in weakened immunity. Your body becomes prone to diseases, some of which can be fatal. To fight off cancer cells, one must have a strong immune system that comes from the right kind of nutrition.
  2. Smoking is a recurrent habit among children and adults: Smoking can cause cancer. Every cigarette packet reads the same warning messages but it doesn't actually deter smokers in any way. Smoking is prevalent among people of all age groups in India. From poor children to conscious educated adults, all are seen smoking.
  3. A Tropical country and its woes: Tropical countries are known to face the wrath of the sun. While most places in central, western, partly eastern and southern India experience extremes of temperature in summer, other places with moderately hot summers are not exempted from the harmful UV rays. Ultraviolet rays can be very harmful for one's skin, as it can cause skin cancer. Indians have a high amount of melanin, which protects them against sun rays, but the threat exists nonetheless.
  4. The concept of fast food: A global economy has opened avenues awaiting your attention in the realm of food. To suit the tones and moods of a fast life, fast food has been made available to you. We take pride in consuming things that can be prepared in an instant: instant noodles, soups and even curries. Packaged food and junk food are sources of cancer cells.
  5. Lack of awareness regarding the most common types of cancer: Breast and cervical cancer are the two most common types of cancer eating away the health of Indians. The problem lies in being unaware about the root causes, symptoms and treatment procedures related to these kinds of cancer. Social repression turns health concerns into matters of insignificance. Women fear social alienation after coming in the open with their problems.

The body produces a number of chemicals called hormones which are essential for the regulation of various functions including growth and metabolism. These hormones are produced by endocrine glands which are located in various parts of the body. One such important gland is the pituitary gland which produces the important growth hormone. This is essential for regulating optimal growth during the growing years and for maintaining proper amounts of body fat, muscles, and bones in the later years of life.

Causes: Deficiency of the growth hormones can occur due to a number of causes, some of which are listed below. It can be congenital (present at birth) or acquired later in life.
The congenital issue could be due to problem in the pituitary gland structure, leading to complete absence or reduced secretion of the hormone.
With age, there is a decrease in the amount of secretion. However, infections, injuries, brain tumors, surgery and radiation can also lead to altered amounts of secretion.

Symptoms: While growth hormone deficiency can occur at any age, symptoms differ depending on the age when the deficiency sets in.
In the early ages:

  • Lower rate of growth for a given age
  • Delayed developmental milestones
  • Delayed onset of puberty
  • Short stature/reduced height
  • Younger looking compared to other children their age
  • Fat deposition around the waist
  • Delayed dental development

When the deficiency sets in during the later years, there is

  • Low energy levels, constant tiredness
  • Decreased strength
  • Decreased exercise tolerance
  • Decreased overall muscle mass
  • Thin and dry skin
  • Increased fat deposition and weight gain around the waist
  • Changes in social behavior including alternate cycles of anxiety and depression
  • Lack of motivation
  • History of pituitary tumors
  • High levels of fat and cholesterol

 

The diagnosis depends on the age of the person

  • Blood tests are carried out to check the hormone levels in circulation
  • In children, in addition to the hormone levels, x-rays to see the status of growth plates is very helpful.
  • An insulin hypoglycemia test where insulin is given intravenously to see the levels of the growth hormone after 30 minutes.
  • Total cholesterol levels, low-density lipoprotein (LDL) cholesterol, apolipoprotein B, and triglyceride levels can be used to supplement the above tests.
  • CT scan and/or MRI of the brain may be needed if tumors are suspected.

Treatment: Once diagnosed, replacement therapy is given as shots a few times a week under the skin. This restores normal growth and helps in controlling the symptoms in adults too.
In cases of tumor, radiation or surgery may be required, but most cases are managed with hormone replacement.
Watch out for the symptoms if your child has delayed developmental milestones and early intervention can help restore growth and function to normal levels. If you wish to discuss about any specific problem, you can consult an endocrinologist.
