Age 51 years male. Slip disc happened jan'2010 and get to normal after 4 years doing only exercise till now and used lumbo scrarol belt when out of home. Due to filling uneasy during walking, a tmt brace protocol suggested. Is it ok to do the test.

Surgical Treatments For Uterine Cancer

The uterus is an organ situated in the pelvis of a female. It is hollow and is usually called the womb where child conception takes place. The uterus functions to help in developing the foetus until birth. Abnormal cell growth that consists of uterine tissues causes uterine cancer.

Although the exact reason for uterine cancer is not known, the risk factors usually can be seen in women with hyperplasia, obese women, and women who have never had kids. Common signs and indications of uterine cancer are unusual vaginal bleeding or discharge, pain while urinating and having sex, and pelvic pains.

We have more than 200 bones in our body and each of them is susceptible to bone cancer. However, long bones in the arms and legs are most susceptible to this condition. Bone cancer can be primary or secondary. Primary bone cancer involves uncontrolled and abnormal cell division within the bones while secondary bone cancer refers to cancer that originated somewhere else in the body and later spread to the bones. While children and adults are equally at risk for primary bone cancer, adults and elderly people are more susceptible to secondary bone cancer. If diagnosed early enough, bone cancer can be treated and even cured with surgery, chemotherapy or radiation.

Hence it is essential to recognize the signs and symptoms of bone cancer. Here’s what you should look out for.

  1. Pain in Bones: Pain is one the primary symptoms of bone cancer. As the tumour grows larger, this pain can become more intense. In its early stages, the pain may be experienced as a dull ache inside the bone or the affected part of the body. It may also increase or decrease according to your activity level or may be experienced only at night. However, not all bone pains signify ‘cancer’ as this is also a symptom associated with osteoporosis.
  2. SwellingIn some cases, the abnormal growth of bone cells can result in the formation of a lump of mass that may be felt through the skin. In other cases, the affected area may also show signs of swelling.
  3. Breaking of the Bone: Cancer can weaken the bones and make them more brittle. This may make the bones more susceptible to fractures. A bone breaking in an area that has been painful or sore for a long period of time may be a sign of cancer. This is known as a pathologic fracture.
  4. Reduced Flexibility: If the tumour is located near a joint, it may affect the range of movements possible and make simple actions uncomfortable. For example, a tumour around the knee may make walking and climbing stairs a painful exercise.

Other symptoms to look out for are sudden and drastic weight loss, tiredness, excessive sweating at night, fever and difficulty breathing in case cancer has spread to other organs. Since many of these symptoms are common to other medical disorders, you should conduct a doctor immediately if you notice any of them. A physical examination and a couple of tests along with a biopsy will be required to confirm a diagnosis of bone cancer. Consult an Expert & get answers to your questions!

My hba1c is 8.6 and average blood sugar is 200. I am male 63 years. I have problems in L4 & L5. What exercise I should do as doctor bars to do rotator and step up exercise. My weight is 92 kg and my height is 170 cm. My lipid is OK. Please help.

Hello, Thanks for the query. Hba1c% 8.6 is high, showing poor control. Ideally it should be <7%. Secondly to reduce weight you will have to look closely at your diet and do those exercise which are permitted. If walking is allowed, please do it regularly. Because with a BMI of 31.83 kgs /M2 (Normal < 23 kgs), you are already in grade 1 obesity. Weight reduction it self will help in better glucose control. Thanks.

How To Diagnose Breast Cancer ?

Breast cancer is a form of cancer which develops from the breast tissue. Breast cancer is indicated by signs such as a lump in the breast, changes in breast shape, fluid flowing from the nipple, dimpling of skin, or the development of red scaly patches. Breast cancer is a fatal form of cancer in women and immediate diagnosis is required on observing the symptoms.

Diagnosis of breast cancer
Other than the regular breast screening, the diagnosis of breast cancer involves the following steps and methods:

  1. Seeing your general practitioner (GP): It is very important to visit your GP soon after noticing the symptoms of breast cancer. Your GP will examine you properly and in case your symptoms need more assessment, he/she will refer you to a breast cancer clinic.
  2. Mammogram and breast ultrasoundYou will be required to have a mammogram, as recommended by a specialist breast unit. This is an X-ray of the breasts. An ultrasound scan may also be required. Breast ultrasound should be undertaken only if you are less than 35 years of age. This is because, young women have denser breasts and a mammogram is not as effective as ultrasound in the diagnosis of breast cancer. In ultrasound, high frequency sound waves are used for obtaining an image of your breasts. It is observed to notice any abnormality or lumps. A breast ultrasound is also important for determining whether a lump is solid or contains liquid.
  3. BiopsyIn this diagnosis process, a sample of the tissue cells is taken from the breasts and tested under a microscope to find out if it is cancerous. A scan and needle test for the lymph nodes present in your armpit is also done to check whether they have also been affected. A biopsy is undertaken in several ways, depending upon the condition and severity. A needle aspiration biopsy is used for testing a sample of your breast cells without the removal of the tissues. This is the most common form of biopsy and it is also used for draining a small fluid-filled lump or benign cyst. During the process, you will be given a local anesthetic. Usually, a needle biopsy is carried out guided by an X-ray, ultrasound and an MRI scan as well. This helps in distinguishing it from non-invasive changes such as ductal carcinoma in situ.

Another form of biopsy used for the diagnosis of breast cancer is called vacuum assisted biopsy or mammotome biopsy. Fractures are simply a break in a bone. They can be caused due to injury, (traumatic fractures) or a pre-existing condition like osteoporosis that causes weakening of bones (pathologic fractures). There are many ways to classify fractures. All fractures fall into the major categories of simple and compound fractures. Simple fractures are fractures where bones remain inside the skin and don’t jut out. They are also called closed fractures.

Compound fractures, also called open fractures, are broken bones that penetrate through the skin. These types expose the bone and deep tissues to the environment. Compound fractures are more serious of the two. The healing here may be affected due to deep infections for which antibiotics need to be used. There are many different sub types of fractures and we’re only going to skim through them here.

  1. Comminuted fractures: Severe fractures in which a bone breaks into several smaller pieces.
  2. Avulsion fractures: A small piece of bone is completely torn off from the main bone due to fierce pulling off a part of the body.

Other types of fractures are characterised by the many different angles the bone breaks into like transverse, oblique and spiral fractures.

Diagnosis
When a bone is broken there are symptoms like swelling that doesn’t subside on its own and pain. In such a case it’s imperative that one goes to a doctor for a diagnosis. Doctors can usually recognise most fractures by examining the injury and taking an X-ray. The X-ray also provides a clear idea about the type of fracture and the degree of displacement of the bone. And, it’s important that the patient doesn’t wait too long before approaching a doctor. This is because bones begin to heal very quickly after a fracture and the bone tissue will heal using any tissue available. This can lead to a misalignment of broken pieces of bone and cause disability and loss of function.


There are cases when X-ray may not show a fracture. This is especially common in fractures in the hip and wrist in older people. For diagnosing these, doctors will get some other tests done such as a computed tomography (CT) scan, magnetic resonance imaging (MRI), or a bone scan.

Treatments
Fractures have to be treated by doctors. The doctors set the fractured bones in their proper place and hold them there so that they can heal. Setting a bone is called "reduction." Reduction without surgery is called "closed reduction." But if the fracture is serious, it’s going to require surgery with bone repositioning, called open reduction.

In extreme cases, pins, plates, screws, rods, or glue are used to hold the fractured bones in place, inside the body. Once the bone abutment has been treated, the bone is immobilised to allow the broken pieces to heal. In most cases, the fractured part is set in a rigid cast. The fractured ends of the bone can be fixed into place using metal pins connected to an external frame. The symptoms caused due to herniated disc can be very severe and can also cause a bit of disability. The disc of the spine is like a cushion and separates the set of bones on the backside. The discs are shock absorbers of the spine and are mainly composed of 2 parts, a soft jelly-like centre called the nucleus and a tough outer covering called the annulus.

Effects of Herniated Disk

A herniated or cracked disc is a severe condition and it seems to happen most commonly in the lower back or neck.  It happens when a fraction of the soft centre gets pushed through the destabilized area due to degeneration, trauma or by putting pressure on the spinal column. 

Nerves located at the back of every disc are responsible for transmitting pain, motor impulse, bladder control etc. in our body. While a disc gets herniated, the external covering of the disc tears and creates a bulge. The soft jelly gets shifted from the centre of the disk to the region where the damage has occurred on the disc. Most commonly, the bulge occurs in areas where the nerve is located and it causes strain and irritation of the affected nerve. It has been observed that individuals may or may not feel any painful sensations even if their disc gets damaged. Other symptoms may be weakness of muscle groups or difficulty in controlling the bladder.

When is surgery recommended for herniated disc?

Surgery for herniated disc is recommended only after options like rest and pain relievers do not work. If the pain persists even after these options, then it becomes important to go for surgery. Surgery is also considered early if there is weakness of muscle groups or acute problem in bladder control. At times, emergency surgery is also required to avoid paralysis in a patient.

However, there are certain risks involved in this surgery like infection, bleeding or nerve damage.  There are chances that the leftover disc may bulge out again. If you are a patient suffering from degenerative disc disease, then there are chances that problem occurs in other discs. It is very important that a patient maintains healthy weight to prevent any further complications.

The main factor that increases the risk of herniated disc is excess body weight, which causes a lot of stress on the lower back. A few people become heir to a tendency of developing this condition. Even individuals with physically demanding jobs are prone to this condition.

Herniated disc is likely to have a course with repetitive episodes of back and leg pain. It is important to avoid postures that may complicate the situation. Surgery is usually recommended, if there is any evidence of neurologic deficit, such as weakness of toes, persistent numbness or bladder/ bowel dysfunction. Most such cases can be managed conservatively with a combination of medication and physio.

हमारे शरीर में होने वाला दर्द कई तरह का होता है. मांसपेशियों में होने वाला दर्द भी इसी तरह का दर्द है. ज्यादा मेहनत करने से मांसपेशियों में दर्द होना आम बात है. शरीर में मांसपेशियों की एक जटिल प्रणाली होती है. मांसपेशियों में दर्द एक छोटी सी झुंझलाहट से शुरू होता है लेकिन कई बार यह आपके स्वास्थ्य के लिए गंभीर भी हो जाता है. इससे पीड़ित लोगों को अपने शरीर को पर्याप्त पानी से हाइड्रेटेड रखना होगा. अगर आपके शरीर में पर्याप्त पानी नहीं रहेगा तो आपकी मांसपेशियां अकड़ जाएंगी, और साथ ही आसानी से उन्हें चोट भी लग सकती है. आइए मांसपेशियों में होने वाले दर्द को दूर करने के उपायों को जानें.
1. सरसों का तेल
सरसों का तेल को एक प्राकृतिक औषधि माना जाता है. जिसके उपयोग से त्वचा की सतह पर रक्त प्रवाह को बढ़ाने में मदद मिलती है. इसके उपयोग से आपकी मांसपेशियों की अकड़न कम होगी और दर्द से भी जल्द राहत मिलेगी.

3. खट्टी चेरी का करें प्रयोग
खट्टी चेरी आपको व्यायाम के बाद होने वाले मांसपेशियों के दर्द से राहत दिलाती है. अध्ययन के अनुसार खट्टी चेरी में कई एंटीऑक्सिडेंट और सूजनरोधी गुण होता हैं जो सूजन और मांसपेशियों में होने वाले दर्द को कम करने में मदद करते हैं. मांसपेशियों की सूजन और पीड़ा को कम करने के लिए आप बिन चीनी के खट्टी चेरी के जूस का भी इस्तेमाल कर सकते हैं.
4. बर्फ का करे इस्तेमाल
जहां जहां आपको मांसपेशियों में दर्द है वहां अगर आप बर्फ के पैक को लगाते हैं तो इससे आपको दर्द और सूजन को कम करने में मदद मिलेगी. बर्फ रक्त वाहिकाओं को कसने में मदद करेगा जिससे कि आपके रक्त का प्रवाह प्रभावित क्षेत्रों में कम होगा.
5. गर्म पानी
दर्द के शुरू होने के 24 घंटे बाद एक गर्म पानी से अपने प्रभावित क्षेत्रों पर सिकाई करें. इससे आपके मांसपेशियों में दर्द नहीं होगा और जल्द राहत भी मिलेगी. इसका उपयोग करने से रक्त के प्रवाह में वृद्धि होगी और उपचार प्रक्रिया को भी एक गति मिलेगी. दर्द शुरू होने के 24 घंटों के भीतर इस प्रक्रिया का इस्तेमाल न करें.
6. केला पहुंचाये फायदा
केला मांसपेशियों के दर्द को कम करने के लिए एक बहुत अच्छा घरेलु उपाय है. यह फल पोटेशियम का एक समृद्ध स्रोत है. पोटेशियम की कमी से मांसपेशियों में कमजोरी, थकान और ऐंठन शुरू होने लगती है.
7. हल्दी है फायदेमंद
हल्दी एक दर्द निवारक और सूजनरोधी गुण के रूप में कार्य करती है. हल्दी का इस्तेमाल कैसे करें – एक कप दूध में एक चम्मच हल्दी को मिला लें. अब इस मिश्रण को हल्की आंच पर गर्म करें. अब इस मिश्रण को गर्म होने के बाद पी जाएँ.
8. रोज़मरी
ताज़ा और सूखे रोजमेरी की पत्तियों में सूजनरोधी गुण होते हैं जो मांसपेशियों की सूजन और दर्द से जल्द रहत दिलाने में मदद करते हैं.
9. अदरक
अदरक एक प्राकृतिक सूजनरोधी गुण के रूप में कार्य करता है और रक्त परिसंचरण और रक्त प्रवाह को बेहतर बनाता है. अदरक के सेवन से मांसपेशियों का दर्द दूर होता है. एक अध्ययन के अनुसार अगर आप कच्ची अदरक का सेवन करते हैं या उसे जलाकर खाते हैं तो आपकी मांसपेशियों और चोट का इलाज बहुत अच्छे से होगा.
10. सेब का सिरका
सेब का सिरका आपके मांसपेशियों के दर्द को दूर करने के लिए एक और महान उपाय है. इसमें सूजनरोधी और क्षारीय गुण भी होते हैं जो दर्द और सूजन को कम करने में मदद करते हैं.

Aur iska jo incidence hai woh badhta ja raha hai jiske kai karan hai, isme breast cancer ke ilaj mein pichle 30-40 saalon mein bahut tarakki hui hai aur aaj ke din hum dekhte hain ki breast cancer ke marijoo ka survival rate hai woh 90% se upar hai, lekin itna achha survival achieve karne ke liye hame jo hai uski diagnosis bahut jaldi karni padti hai. Toh yeh ek bahut sabse important baat hai, jo sabse common symptom hai breast cancer ka woh hai breast mein ek lump ya ganth ka hona.

Ab breast ki ganthe kuch cancerous hoti hai kuch non-cancerous hoti hai, lekin jab bhi koi ganth ho toh yeh possibility zaroor sochna chahiye ki kahi cancer to nahi hai aur turant ek cancer specialist ya breast surgeon se opinion lena chahiye. Cancer ki diagnosis banane ke liye usme kuch janche hoti hai jisme mammography ya ultrasound hota hai, fine needle aspiration cytology ya biopsy hoti hai aur kabhi kabhi breast ka MRI bhi kiya jata hai.
