Hba1c is the best blood test for dm. It's shows the average level of blood sugar over last three months. Random and postprandial can tell only current blood sugar levels.

What shall I do?
Diabetes affects all organs in the body ? Eyes, kidneys, nerves, reproductive system all are affected, In fact, from toe to hair. For diabetic control, patient and doctor should be in constant touch for regular follow up and advice. It is not as easy task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (Self-Monitoring of Blood glucose ? SMBG). Though difficult, this can be done by a glucometer and keep a record. Get an idea of Fasting sugar (FBS) and 2 hours after food (PPBS), & HbA1C (inform me) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ Insulin will be needed. There are different diet plans of varying calories for each person depending on his activity & life style. For sugar control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner

Hello Doctor, I have a query regarding HIV. Actually on 31st Jan, 2015 I had a sex with a girl in Thailand without condom. I came back and undergone a blood test on 6th Feb which was negative. Could you please help me whether I should have more check-ups now and should intercourse with my wife or now. I am very worried now and regretting for my mistake. Thanks.

Hello. What you are talking about here is called seroconversion. Seroconversion is the interval, several weeks after HIV infection, during which antibodies are first produced and rise to detectable levels. (why this is important for you - its because lab tests are designed to detect HIV antibodies ). Seroconversion takes place within three weeks in most infected individuals, although very rare cases are reported in which seroconversion does not occur for up to a year. So, in your case getting yourself tested within a week was not advisable. Get yourself tested after a month ( i.E by 2nd March or later) . If at that date, your test is negative then, you can be reasonably certain that you do not have HIV. , although as mentioned above, 100% confirmation will be only after one year,

Hye lybrate-user thanks for the query. Firstly understand that weight gain is very dependent on genetics and metabolism along with your daily diet and exercise routine. It is important to know that just eating just two heavy meals a day will not help. Weight gain is all about taking more calories than you burn. Eating the right foods at the right time is very important. There is no one food that helps weight gain. Eat meals at frequent intervals. You need to eat at least 6 main and mid meals a day. Eat a mid meal between each main meal. Avoid junk foods. Include dairy, eggs, nuts, fruits, dry fruits, starchy vegetables, rice, bread, whole grain cereals, smoothies, masala milk, ghee, cream in your meals. Just eating sugary, sweet foods is not good. Try to have healthy deserts like kheer, chikki or lassi. Avoid alcohol and smoking. Exercise at least thrice a week. It helps development of muscle mass and improves appetite. Even if you were to take just medicines or supplements they will not work in isolation. You need to have a calorie and nutrient dense meal and good exercise routine, it's crucial.

Leg pain before periods is not uncommon. Do hot fomentation keep a pillow under the legs at night &/when resting. Any way take ibuprofen 200mg od & sos x 5days l-carnitine 500mg od/bd x5 days you will need other supportive medicines also. Do knee exercises it may have to be further investigated make sure you are not allergic to any of the medicines you are going to take. Do ask for detailed treatment plan. Do not ignore. It could be beginning of a serious problem.

I am 45 year old Male, for sometime I going through professional rough time, I mean having tough time (as for sometime there is market recession), also some financial crunch (Money got stuck with business associates), cause of these two setbacks I am thinking a lot and not able to get it off my mind, may be I am thinking a lot cause of that I often get tightness in my stomach and incase I get up in the middle of the night then sleeping again becomes problem, I keep thinking and thinking that something will happen to me, my stomach often stays upset gas gets trapped and then more tension, I have noticed that when this gas gets released I intact gets relief, now I don't know it's this problem is cause of tension I am taking or because of stomach upset. Your stomach upset is definitely related to the anxiety attacks you are experiencing. You may be troubling yourself unnecessarily and too caught up with excessive thinking. You must now act on these problems and only then will a solution appear. Anxiety has all to do with fear and your inability (or so you would tend to think) to handle the same. Fear is based on looking too much into the future and imagining things that usually do not tend to happen (usually 90% of your fears will not happen). You need to change your perception of the fearful stimulus then you can change your response to the fear. The first one is the best and will preempt the need for the second. If you are already far gone into the reaction mode, then you need both responses to counter the attack. The way we see things in life (like half empty or half full glass) is very dependent on our attitude. Attitude may have developed over many years but fortunately can change in a matter of sustained choices made and acted up on now. Accordingly you choose to see things as either threatening or as an opportunity. The opportunist will be positive and work with the situation to make good out of it. The individual who sees it as threatening, will, out of fear, try to avoid/run away from the situation or react with anxiety. Yes, self-belief does impact it too, but you can see that prior to that, perception will tarnish the belief system too. Here, you can see that all the resolution lies in just changing your perception/attitude, even if it is against odds. Changing your response, despite the perception, is building up of your reserves and resources to handle any contingency that life throws up at you. And life throws up situations quite regularly and heavily. For this you need to develop a fit mind and body: The former, by meditation and mind exercises; and the later, by exercises and proper diet. You need good sleep too. Included in the development of the mind is the handling of emotions, in your case, fear. These changes will give you a handle on life and your perceptions and responses. Prayer is really powerful, if you are a spiritual person. Repeat some personally framed affirmations that are geared to calming you down and building confidence. Do a personality course too. Anything that develops you personally will be of great assistance in building resilience.

My father is 83 years old. He is suffering from cough last few days & also been in hospital. He is also having breathing problem since long time. Dr. Has prescribed PIPERACILIN & TAZOBACTAM 4.5g. 3 times daily for 7 days for the chest infection. But getting no improvement, after usage of 4 days.

Dear lybrateuser, - Avoid exertion, exercise, lifting weight till pain subsides - do hot fomentation three times a day on affected area - apply pain killer gel three times a day, apply lightly do not rub - if pain is more take a pain killer tablet after meals.

Hello, you may be having headache because of stress/ refractory problems/ any ENT problem or may be migraine headache kindly follow advises given below: you can take: 1. Tablet Crocin (Paracetamol 500 mg) one tablet after food when required for headache after food for 2-3 days (maximum 2 tablets with gap of 12 hour can be taken in a day) 2. Adequate sleep 6-8 hr in a day 3. Get check eyes for refractive error 4. We need to go for ENT evaluation to rule out sinusitis 5. Get BP checked 6. Regular physical exercise and meditation and yoga complete history of your headache for further management like duration, severity, aggravating factors/relieving/ triggering/ associated factor to reach diagnosis Consult physician for further management

Mr Vishwanathan, She does not have hypothyroidism now. She may be having subclinical hypothyroidism. There is no need for any treatment now. She should exercise regularly to maintain normal weight, avoid soy beans, soy containing products, cabbage, cauliflower, broccoli, radish, peanuts, mustard and coffee. Use whole grain flour for making roti and chapati. Consume good deal of green leafy vegetables and fruits. Avoid high fat foods. This should help her. Get TSH levels checked after 6 months.

You are Obese, Diabetic, not stated whether well controlled. Are you taking care of yourself as per advice of doctor? Look sometimes a person get problem in falling asleep. Reason may be simple to some thing complex. Better consult either your treating doctor who should give you some time to find out the reason. In simple cases: for a person with disease like you and of your age gets good result with: 1) Healthy life style. - Exercise like Brisk Walking for 5 KM daily, Diet control appropriate to Diabetics. 2)Timely sleep near about at the same time every day. 3) Meditation 20 minutes daily before going to bed. Your doctor may decide whether you should take the help of any tranquilizers.

Hi, ladies of your age are less likely to have ischemic heart disease, especially when there are no risk factors. The pain could be muscular or arising from the joints of your chest wall, a condition called costochondritis. You can have some pain killer ointments and avoid lifting weights with left hand.

For removing most of fats from milk, industrial processing (double-triple toning) is required. You are able to remove some amount of fat from milk. If you want low fat milk, purchase double toned milk (slim trim).

Hello. When you sleep late at night then you feel tired in morning. Try to sleep early. It gives you good sleep and makes you fresh when you wake up in morning. For hunger you can take homoeopathic tonic alfa alfa malt. Take one spoon in morning and one at night half an hr before meal. It regulate your hunger. Avoid fast food and oily food. The burning sensation is only due to your diabetic. So first let the diabetic is under control, then automatically the burning sensation will subside. So let me know whether your diabetic is under control or not.

Requires detailed examination and investigation. Preliminary investigations suggested: hb, tlc, dlc, esr, blood sugar fasting & pp, uric acid vit d urine r/e x-ray of the affected part. Anyway it may be tried sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomantation paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck, back, knee & general exercises. It may have to be further investigated. Do not ignore it. Let it not become beginning of a bigger problem. Make sure that patient is not allergic to any of the medicines that he/she is going to take. If it does not give relief in 4-5days, inform.

You did well two times so third time it is due to performance anxiety. For burning sensation take more water and test urine routine examination.
