Childhood Sexual Abuse - A Mental Health Issue

Child sexual abuse is a pressing issue in society today. Statistically, there has been a considerable increase in the number of cases related to juvenile sex abuse over the past decade. Most of the cases report the abuser to be a close family member or a family friend rather than a complete stranger.

Future Consequences- 

  1. A sexually abused child may develop severe mental health conditions and have major difficulties coping with as an adult. One of the most common disorders affecting survivors of child sexual abuse is Post-Traumatic Stress Disorder (PTSD). This involves the individual reliving the trauma all his/her life, thus leading to panic, stress and difficulties in living a healthy life in the longer run.

  2. The individuals who were abused as a child normally suffer from a very low self -esteem. They continue to blame themselves for the events that occurred and hence have a rather demeaning view of themselves, leading to adjustment issues in the future.

  3. They may also be diagnosed with clinical depression owing to the previous trauma. This hampers the everyday functioning of the individual. The individual may continue to relive the past traumatic events in the mind that further fuels their depression and contribute to the vicious cycle, eventually.

  4. Most often, survivors of child sexual abuse have a rather impulsive nature owing to the hampered functioning of impulse-control. They have lesser control over their emotions, especially anger and are rather quick to act on it.

  5. Because of all the complications, they might be less socially competent and thus fail to build firm peer relationships. They grow up to be adults with severe trust issues; they are hence, often side-lined because of their cynicism which leads to further depressive symptoms.

  6. Genophobia or the fear of sex is another complication that they may experience as adults. They may avoid sex altogether or engage in sexual activities that are rather violent in nature. Some survivors may develop certain sexual fetishes that can be rather dangerous; the most common being ‘paedophilia’ or the sexual arousal involving prepubescent children.

Conclusion-

The seriousness of the condition is often undermined. In treatment, various therapeutic techniques are used according to the need and age of the patient. Therapists work to promote positive thinking among the sufferers so that it becomes easy for them to accept their past and move on in life with a better outlook. 6 Symptoms Of Gender Identity Disorder

Born as a male but feel like a female OR born as a female but feel like a male. Some persons are unable to feel comfortable in the physical gender that they are born in; their mind, brain, feeling and thoughts are all of the opposite sex. This uncomfortableness in their mind about their physical gender is termed as Gender Identity Disorder.

What's the cause?

We do not know the exact cause of this disorder, but it is hypothesized that developmental changes in the brain and the body do not sync, and the brain/mind is left with a different gender than the body. This is mostly an unidentifiable and unpredictable change.

What are the symptoms?

1. Children might express a desire to dress up as the opposite sex.
2. They might express replusion at one's own sex organs/genitals.
3. As adolescence begins, they do not develop feelings of attraction towards members of the opposite sex. Some complain of revulsion about their pubertal changes.
4. Males want to pee while sitting down, while females want to do it standing up.
5. Their behavior is often more like that of the opposite sex, e.g. boys might display feminine gestures, while girls might indulge in risk taking and reckless behavior.

6. As they become adult, a realisation that they are stuck in a body that they do not like causes depression and anxiety.

Diagnosis:

A diagnosis of Gender Identity Disorder (GID) is made based on a detailed assessment by a Psychiatrist, who is specially trained in this assessment. WPATH standards mandate that this is an ongoing assessment, and involves facilitating the process of Hormonal Treatment and eventual Sex Reassignment Surgery. The whole process can take 2-3 years.

Treatment:

For those who have severe GID, eventual sex reassignment surgery is the treatment and outcome. However, support of family, friends and a Good Psychiatrist can enable a smooth transition. 1.Take paracetamol 500mg, one tablet sos upto a maximum of three tablets daily after food 2.Drink plenty of water 3.Take rest 4.If no relief do blood examination like ESR, CBC, Widal/ Typhidot test, Blood for mp/malaria antigen and Dengue antigen(NS1) test after consulting doctor 5. consult me for further advice

Hi I have one small lymph node in neck below right ear inside skin. If we touch deeply only we can detect that. Its paining sometimes. Its there from last one year. I have consulted one ENT Doctor. But he told no need to worry leave that simply. But its paining sometimes. I am getting ear pain also sometimes. Please suggest me what I have to do?

If there is pain in the lymph node you may take a course of antibiotic after consulting your attending doctor. If there is no further infection the enlarged lymph node will subside within a few months. If it still persists and if you wish, you may get a FNAC examination done from the affected node.

I am 17 years old and I am having a problem in excretion of bowels, I had last excreted yesterday morning and then onwards I am having too much formation of gas in my stomach. What should I do?

Limit the amount of white bread, sugar, meat, junk food and milk products (cheese, milk, butter, cream.) and increasing fresh fruits, veggies, water and fiber (beans, oats,) also yogurt really helps due to the probiotics. Maybe you could do a food diary for a period, eliminate a couple of items at a time and build up until you have added enough other of good items to make the stools loose. Oatmeal helps out tremendously. Figure out the diet and once you've added enough good things, slowly pull back on the laxatives and pay attention as you go. This could be a food allergy, do some research, usually its lactose (dairy), nuts, sugar, food coloring, preservatives. Try to avoid boxed and processed foods which have more preservatives, calories, fat, high carb. Treatment is available, safe and without side effects in biochemic system of medication. Consult me in private.

My boyfriend and I had protected sex on 21st Feb after which I took after pill on 23rd. We had unprotected sex on 25th, he pulled out but I took another after pill on 27th. My period was due on 9th March. I'm getting a brownish discharge and lower abdominal cramps. I'm really worried about this being spotting. What should I do?

Its ok to not use condoms but make sure if you are not willing to be pregnant than your husband should make sure that he doesn't ejects inside the vagina, secondly which pills you are using there are various kinds of pills like abortion pills, pre birth control pills, vaginal pills etc. Whatever pills you use always remember they are not healthy for your body and you are going against the nature and against the natural forces of your body. The old age method is- if your husband has still ejected inside your vagina immediately go and urinate sitting in indian style while urination and never in western pots, ask your husband to be extra careful and remind him about the harm of these pills, enjoy your sex life but not in stress.

BP medications are titrated as per changes in the BP. And BP fluctuations are normal, because number of factors play a role in BP management. Depending upon variables you may at times need more medications at times less for optimal control of medication, the decision needs to be taken by your treating cardiologist based on your current parameters, so routine follow up is essential. Also, ensure that you exercise daily for 40-45 mins, avoid salty and junk food, too much of caffeine intake.
