Intracytoplasmic Sperm Injection (ICSI)

M.Sc, MD, MBBS
IVF Specialist, Delhi
Intracytoplasmic Sperm Injection (ICSI)

Intracytoplasmic Sperm Injection i.e. ICSI is different from conventional IVF i.e. In Vitro Fertilization because a single sperm is selected by the embryologist to be injected by a needle into the egg rather than a dish with many sperms placed near the egg so that the fastest swimming sperm enters it. Once the sperm is injected directly into the egg, it fertilizes, after which the embryo is transferred into the womb or uterus.

Your doctor might recommend ICSI treatment for you if you have an extremely low sperm count resulting in infertility or other medical sperm conditions such as poor motility or poor morphology. It may also be suggested if prior attempts at IVF procedures have failed in fertilizing the egg or the fertilization rate is extremely low. You must also consider ICSI treatment if you resort to embryo testing or if the sperm needs to be collected from the epididymis or testicles surgically.

However, you must keep in mind that if your partner has a genetic problem resulting in low sperm count, it could be passed on to your child, if you decide to use your husband's sperm for this process.

Success Rates of ICSI:
The success rate of ICSI treatment depends on your age and your fertility problem. Your eggs are healthier when you are young and as you age, chances are that they will become less healthy.

  1. 35 per cent if you are under 35 years of age
  2. 29 per cent if you are between 35 to 37 years of age
  3. 21 per cent if you are between 38 to 39 years of age
  4. 14 per cent if you are between 40 to 42 years of age
  5. 6 per cent if you are between 43 to 44 years of age
  6. 5 per cent if you are above 44 years of age

Advantages of ICSI Treatment:
Intracytoplasmic Sperm Injection gives you a chance to conceive a child genetically when most other options are exhausted. Males may be unable to ejaculate on the day the eggs are collected due to anxiety. ICSI proves to be advantageous because sperm may also be surgically extracted for this process. It is extremely helpful for those couples who suffer from unexplained infertility. However, one of the most important factors is that ICSI treatment does not affect your child's physical or mental health.

If a male put his sexual part in female's vagina Then the sexual part can go inside without condom? If I use a lubricant (cream, gel etc) in place of condom then is it good? Does male's part get any harm or pain?

Hello- If you are a first timer then its better to use condom. Due to its slippery surface, its better to use a lubricant while condom on. It will make things easier for you. Condom can make your stay on bed longer. Condom can protect your from any kind of STd or unplanned pregnancy risk. However you can also insert your penis without condom, it will cause no harm or pain on your behalf if your partner is infection free.

Does touching the vaginal outer part and intercourse outside just touching the vaginal part makes us unvirgin.

MD - Obstetrtics & Gynaecology

Only if the finger or penis is inserted inside the vagina, then only hymen is torn and virginity is lost. Act you described can not make you un-virgin.

I had my periods around 10-13 May, 2016 and had protected sex on the 29th May, 2016. I took a pregnancy test on 14th June which was a negative and then had my periods the next day, on the 15th and it lasted for around 3 days. Is there any chances of it being implantation bleeding? I had cramps and bloating around the 23rd - 30th of June, and took a pregnancy test on 28th of june which was negative. I took another pregnancy test on the 1st of July, 2016 which was again negative. It is 15th today and I still have not got my periods and I have very scanty white discharge and slight cramps. Is there a possibility of me being pregnant? Also I took an ultrasound on the 4th of July. The doctor said my uterus was normal. I am getting so paranoid. Please help.

Hello, the period you had in june seems like a normal period. The chance of a pregnancy in a month is only about 20-30% so do not stress out abt it. It may take a while to get pregnant! do a pregnancy test only wen your periods are delayed and not thru out the cycle. There are a few hormonal changes that occur normally with each menstrual cycle like bloating etc. In case of further queries I wud be happy to help.

If you have uncomplicated pregnancy, then you can fly up to 36 weeks of pregnancy. However, airlines are sometimes unwilling to carry women who are more than 28 weeks pregnant because of the risk of premature labour.

Antibiotics are not very good for health. It kills harmful as well as beneficial bacteria in our body. It supresses the immune response. Better fr you to go for homoeopathic treatment. It will cure all your problem. Taking one molecule for long can make you resistant to that molecule alsoo. Soo try to avoid it. If you want you can contact me through Lybrate.

Dear Sir, I would like to know my married daughter got pregnant and found diabetes her Fasting sugar 120 and 2 hours ...
It requires continuous monitoring and follow up. Let the doctor decide based on the facts. Insulin is not harmful and many times its given during pregnancy ( gestational diabetes). After the delivery your daughter may not require it. If still in doubt take a second opinion from another doctor. There is nothing to worry. Safety of the mother and the baby is more important and so let the doctors do their best.

My Pregnancy test is negative but missed periods which were expected to be on 1st march.
Pregnancy test should be done only after 7 days of missed period. So if it was expected on 1march then it should be done on 10 march (added 2 days for the feb). If negative then keep repeating it every week till menses comes.

Treating Infertility With Ayurveda

Treating Infertility With Ayurveda

Infertility has emerged as a common problem as a result of the modern day lifestyle. Extremely hectic schedules, stress, burnout, depression and unhealthy diets can contribute to fertility issues. As per a study published by Pharmion, around 30 million Indian couples were estimated to suffering from lifetime infertility, in 2015. Also, the Total Fertility Rate (TFR) fell to 2.3% in 2013 against 3.9% in 1990.
Couples often seek help with methods such as artificial insemination and in vitro fertilization (IVF), but the problem can be solved in natural ways too. Sometimes, the root cause of infertility is simply irregular functioning or blockages in the body systems. The basic principle of Ayurveda is to get the body functioning to its optimum capacity and this is often all that is needed to treat diseases.

Here are a few ways in which Ayurvedic medicine can help treat infertility:

  1. Phal Ghrit: The medicine is made by combining a number of herbs and it is taken with warm milk. It has been developed especially to treat infertility by controlling the level of hormones secreted in the body. It cannot remedy structural problems, but it can effectively treat functional disorders in the reproductive system.
  2. Shatavari (Asparagus): This plant is used to treat female infertility by regulating the cycles of menstruation and ovulation.
  3. Triphala Churna: A combination of three different fruits - baheda, amla and haritaki, this medicine helps to cleanse the body of toxins, strengthens the immune system and clears blockages in the fallopian tubes.
  4. Ashwagandha Churna (Indian ginseng): The medicine is used to treat male infertility by naturally increasing the production of sperm in the system.

There are also a few Ayurvedic therapies which can help to cure infertility:

  1. Vamanam: In this treatment procedure, medicated oil is swallowed to induce vomiting, which expels all the contaminating elements in the body. This process can be done regularly depending upon the overall health of the patient and also helps to combat digestive and psychological conditions.
  2. Swedanam: This therapy is usually done before other Ayurvedic therapies to prepare the body systems. For patients of infertility, the patient is made to sweat excessively. This also flushes most nitrous compounds out of the body.
  3. Dhatu Nirman Karma: The cells of the body are made to metabolize, absorb and assimilate nutrients leading to the formation of healthy cells and destruction of the unhealthy ones. This is turn rejuvenates the organs. Usually, this is helpful for female infertility.

If you wish to discuss any other problem, Consult IVF Specialist.
