Knee pain in elderly is commonly due to osteoarthritis which means degeneration of joint cartilage causing friction between joint surfaces, leading to pain and stiffness. Lifestyle modifications and physical therapy are important treatment aspects to maintain functionality. 1. Knee stretching and strengthening exercises to improve knee strength. 2. Avoid jogging or physical activity that increases knee pain. But absolute rest is not advised. Engage in activity like swimming or walking , but perform activity which is painless 3. Diabetes is a chronic metabolic disorder that results in higher levels of blood sugars in the body. It is assuming epidemic proportions, and India has become the new diabetic capital with more than 50 million diabetic people. The main problem with diabetes is it does not completely go away, and there is a whole myriad of issues that it brings along with it. Heart attacks, strokes, eyesight problems, delayed wound healing, nerve damage and impotence. Therefore, diabetes and by that we mainly mean blood sugar levels have to be managed so that there is a delay in onset of the associated conditions and/or reduced in severity.

Ayurveda refers to diabetes as Prameha (excessive urination) and Madhumeha (sugary urine) and has identified close to 20 forms of the disease. According to Ayurvedic belief, each disease is caused due to imbalance in kapha, pitta, and vata doshas and diabetes is caused by all these, predominantly by kapha. With any stream of medicine, management of diabetes involves two tracks - one is lifestyle changes and the second is medications.

Lifestyle changes include the following:

  1. Diet: Reduce the amount of rice, sugar, potato, sweet fruits, maida, deep fried foods, and red meat. Protein-rich foods like lentils, soya, green leafy vegetables and fish should be increased. Diet plan should change to small, frequent meals instead of binge eating.
  2. Exercise: Include 30 minutes of regular exercise into your daily routine if you have a diabetic predisposition.
  3. Others: Avoid smoking and alcohol, ensure sufficient sleep, avoid sleeping during day time, improved foot and eye care, periodically check sugar levels, and manage stress levels.

Medication:
Ayurveda has a whole lot of home remedies proven to be very effective against diabetes.

  1. Jambhul: Eugenia Jambolana whether eaten raw or juice extract has been shown to have beneficial effects in managing sugar levels and cholesterol.
  2. Gymnema sylvestre: Used to manage diabetes for over 2000 years, it reduces sugar cravings and is being touted as the future for diabetes treatment.
  3. Bitter gourd (Momordica charantia): It has 3 components that give it strong antidiabetic properties. The charantin reduces blood sugar levels; the polypeptide has insulin-like effects; and lectin that again has hypoglycemic effects.
  4. Bel (Aegle marmelos): Also known as wood apple, leaves of the plant are shown to have antidiabetic properties. 5 to 10 leaves can be chewed on a daily basis to help control blood sugar levels.
  5. Fenugreek (Trigonella foenum graecum): Consuming 10 g of fenugreek seeds soaked in water increases the amount of insulin produced, thereby increasing sugar breakdown.
  6. Neem: Chewing about 4 to 5 leaves in the morning on an empty stomach helps to control blood sugar levels. Alternately, neem leaves powder is available which can be dissolved in water and consumed. If you wish to discuss about any specific problem, you can consult an ayurveda.

Hi, thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You are very young to actually have persistent back pains. I can as an orthopaedic surgeon advise you management of low back ache but you will have to consult a general physician for your issues with gas trouble. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc and with no other associated constitutional symptoms like evening rise in temperature, loss of weight, excessive sweating at night etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. KNOW ABOUT Nose Bleeding

Nosebleed is a common problem and is often because of a respiratory illness or dry conditions. Nasal drying is common in the hot summer months because of the extreme temperature and dry air is due to use of air conditioners. Here are some typical reasons for nosebleeds:

• Nasal allergies
• Blowing your nose too hard or trying to remove something from inside the nose
• A result of “popping” the ear
• Nasal exposure to chemicals
• Frequent sneezing or having an upper respiratory infection
• Use of nasal spray or a blood-thinning drug, such as aspirin
• Inhaling air that is extremely dry or cold
• Having recent surgery on the nose or elsewhere on the face
• Breaking the nose or a similar injury
• Uncontrolled blood pressure

Bleeding can be controlled by direct pressure i.e. compression of the nostrils grasping the alae distally so all mucosal surfaces are opposed. Direct pressure should be applied continuously for at least 5 minutes, and for up to 20 minutes. The patient should be encouraged not to check for active bleeding. Patients who are properly instructed may control their bleeding while the evaluation gets underway.

Other maneuvers include bending forward at the waist while sitting up (to avoid swallowing blood), placing a plug of cotton wool or gauze into the bleeding nostril (sometimes coated with antibiotic ointment), expectorating out blood that accumulates in the pharynx and a cold compress applied to the bridge of the nose.

These maneuvers also should be taught to high-risk patients for use at home. Initial treatment with two puffs of oxymetazoline may hasten hemostasis

Dear lybrate user. I can understand. Penis curved towards right or left or downward is always normal and this can be seen with majority of people. This mostly happen due to masturbation. People who use right hand for masturbation, their penis will be curved to the left and the opposite with left handlers. There are other reasons too. This phenomenon is quite normal and there is nothing to worry. In fact, curved penis will give you more pleasure when you engage in vaginal intercourse than straight penis simply because curvature provides more friction. Enjoy your curve. Hello, There are many ways to get straight and silky hair, but the curls with which we are born with, can’t be totally get rid off. - Regular hot oil treatments is one the best and easiest ways to keep your hair smooth and well hydrated. The heat and saturation from the warm oil will relax the light wavy and curly pattern of your hair. For hot oil treatment, the best options are coconut, olive, almond or sesame oil. - Coconut milk is believed to help straighten hair. It has a moisturizing ability and also gives your hair a beautiful shine and luster. In a glass jar, mix one cup of fresh coconut milk and the juice of one lemon. Place the glass jar in the refrigerator for a few hours, until there is a creamy layer on the top. Massage your hair and scalp with this cream, and then leave it on for 20 minutes.Cover your head with a shower cap or swimming cap and wrap a warm moist towel around the cap. Leave it on for about 30 minutes, and then shampoo and condition your hair. - Mix together one-half cup each of milk and water. Store the mixture in a spray bottle. Comb out any tangles in your hair, spray the mixture over your hair and comb it again. Leave it on for 30 minutes so that the milk is absorbed by your hair. Then wash your hair with shampoo and apply conditioner. - Beat two eggs properly and mix in four tablespoons of olive oil. Apply this mixture to your hair and scalp, then comb your hair with a wide-toothed comb. Wear a shower cap and leave it on for 30 to 45 minutes. Rinse it off and wash your hair with a mild shampoo. Management: Take diet rich in protein like non-veg, eggs (it's loaded with essential nutrients such as proteins, vitamin b12, iron, zinc and omega 6 fatty acids in large amounts and biotin), different pulses & legumes, sprouts, nuts (sp. Walnuts and almonds); iron-rich food like green vegetables, dates, carrots, prunes, green peas; also oats, shrimp, take flaxseeds, sunflower seeds, sesame seeds, green tea, soyabean, fruits, salads, etc. Take low-fat dairy products (they also contain whey and casein, two high-quality protein sources) take proper 6-9hrs of sound sleep. Give up the habit of smoking and excessive alcohol intake. Avoid junk food and any kind of beverages (tea, coffee, aerated drinks, cold drinks, etc.) avoid canned/ tinned food or food that contains preservatives (eat fresh ones). Avoid use of hard water to wash the head rather use boiled water for head wash. Avoid use of harsh hair colour, hair treatment like rebonding, straightening, perming, etc. Avoid vigorous exercises or any supplements provided by the gym! cover your head with scarf or cap while stepping outside. Use rose wood/ any wooden hair comb to brush your hair. Regularly massage head and hair with coconut oil, almond oil (depending upon your scalp type) 1 to 3 times a week. Regular exercising is also one of the important point, but should be done under some good, qualified yoga teacher or instructor for proper guidance; it also releases the stress/ tension in the mind and the body and keeps you fit and young!

Hi lybrate-user, It appears to me that you are suffering through depression, it need not have a cause or origin. It can happen to anyone with our without any reason. Feeling down or blue is a natural part of life. People let us down, things go wrong, we lose people we love or the dreams we've valued. When feeling down stretches over weeks or months, is occurring frequently and interfering with your ability to interact with people and enjoy your life, it is likely that you're experiencing some form of depression. Provided that you're able to access information and have a good doctor and support network around you, even severe depression is a highly treatable condition. Along with medication psychotherapy from psychologist is a important adjunct. Firstly, you will have to make few lifestyle changes: Sleep well. Sleep is essential to a healthy, balanced body. Lack of sleep can aggravate negative thinking and easily becomes a vicious cycle whereby your negative thoughts keep you awake and disable your ability to get enough sleep. Waking unrefreshed and feeling tired is a common complaint during depression, and even too much sleep can leave depressed persons feeling tired. Breaking this cycle requires enforcing a strict sleep routine of the same bedtime and waking time every day, avoiding caffeine and alcohol, not exercising three hours prior to bed, removing anything distracting from your bedroom, and keeping your room at a suitable temperature. Exercise releases a natural anti-depressant chemical in your brain and gets you into doing something active. Start small with a simple walk to the local store or around the block, or to your garden gate might be the way to begin. Gradually work up to a routine that fits with your needs and enjoyment. Look for friends or group exercise sessions since having a partner will keep you more motivated. You can also look for activities that will allow you to release some of the pent-up emotions that may be built up, such as kickboxing. Playing sports is a great way to get regular exercise, stay occupied, focus on self-improvement and meet new people. Some studies have found that people who participate in sports have somewhat fewer symptoms of depression. Choose a sport that is exhausting to quiet the chatter in your mind and leave you feeling wrung out—just don't overdo it. Join a team or class in your area and commit to showing up to as many of these meetings as possible, even if you may not feel like going some days. Eat healthy. Reduce your intake of sugar, high fructose corn syrup, fast foods, and processed foods. Eat more fruits, vegetables and whole foods. Drink plenty of water and do some research on foods that are said to improve your state of mind and well-being. Improving your diet can be a positive project to keep you constructively occupied and focused when you're working through your depression. Plus, many healthy foods are effective at improving mood Talk with friends, family and colleagues who make you feel good and who are good to be around. Spend time with people who see the world in a positive way and ask them to share their visions, ideas, and approaches to life with you. Most positive people will be more than happy to reveal the things that help them keep upbeat and happy about their lives. Learn from them. Remember that misery loves company. It can be incredibly hard to keep away from negative people as down and out as you're feeling, but do your best to avoid them. You won't be doing either of you a favor by confirming each other's fears that the rest of the world is terrible. Keep occupied. Being busy is a way to prevent negative thoughts from going around your head repeatedly. For depressed persons, the first step is often the hardest, so making yourself do things can be a huge difference in your day and getting you started Start a journal of your journey through your depression. Document your feelings somewhere personal and completely private. This will be the place where you let out your darkest thoughts — no holds barred — because you don't need to worry that anyone will judge you for them. A diary can become your collaborator in the struggle against your depression because it eventually provides you with great evidence of what improves your mood as well as what brings it down. Try to write in it daily if possible. Change your thinking. As part of progressing, recognizing and defeating negative thinking patterns is a very important aspect to concentrate on. Cognitive behavioral therapy, psychotherapy, or other forms of psychological therapy are helpful when you're unlearning negative thinking and establishing patterns of thinking that support your self-esteem and increase your self confidence. While it is best to read up in this area and speak with someone qualified to help guide you through the ways to change your thinking, there are some important things to keep in mind. Acknowledge the feeling will pass. This can be a very difficult step, but it's vital because it helps you to start banishing thoughts of hopelessness. List all of your good points. When you're depressed, it's easy to understate the positive things about yourself. Turn this around by listing everything that is good about you. Include achievements from the past and hopes for the future, however few or random they may seem. If you can't write this list, have a trusted friend or family member start one for you. This is a list to keep building upon as you work through your depression. Self-acceptance is a vital part of recovering from depression because you acknowledge that there are good things about yourself, but also that you have challenges to overcome. This will help you stop judging yourself more harshly than anyone else. Make decisions, however small, and act on them. Again, while this is very difficult to do during depression, it is a vital element in facing the sense of helplessness that tends to overwhelm depressed persons. Small decisions like getting out of bed, calling friends, or cleaning up the kitchen all add up. Once you act on them, they become achievements. Learn how to replace faulty or negative thinking by focusing on it. Ask questions such as: Am I assuming the worst? Am I condemning myself because something bad has happened? Am I focused on my weaknesses rather than my strengths? It is helpful to arrange the negative thought in one column and a rationalization in another column, so that you can confront and undo the negative thinking. In one column you may write the thought" I'm a failure, and in the other you challenge that thought with" I made a mistake. I've made mistakes in the past and things have turned out okay. I've also had many triumphs. Learn assertiveness techniques once you've challenged the harder aspects of your negative thinking processes. Assertive techniques will allow you to find a pathway to standing up for yourself without giving in to feelings of anger, fear, or powerlessness. Knowing how to assert yourself is an important part of not falling back into depressive patterns in the future. Remember, Always keep yourself busy, or doing something positive. Sitting alone, or thinking about all the bad events in your life without expressing them to someone is going to make your depression worsen. Also pratice yoga or meditation. All the best.
