I had problem of piles last 2 years so please suggest me better treatment for my good health.

Piles are haemorrhoids that become inflamed. Haemorrhoids are masses, clumps, cushions of tissue in the anal canal - they are full of blood vessels, support tissue, muscle and elastic fibers. Piles can be of various sizes and may be internal (inside the anus) or external ones (outside the anus). Typically, internal piles occur from 2 to 4cm above the opening of the anus. External piles (perianal hematoma) occur on the outside edge of the anus. The internal ones are much more common. Symptomatic haemorrhoids affect at least half the population at some time in their lives before the age of 50. In the majority of cases, piles are effectively treated with over-the-counter medications, a good fluid intake, and by following a diet high in fibre. In severe cases, the piles may have to be surgically removed. About 10% of patients who go and see their doctor about piles eventually require surgical intervention. Internal haemorrhoids are classified into four grades.  Grade 1 - there are small inflammations, usually inside the lining of the anus. They are not visible  Grade 2 - larger than grade 1 haemorrhoids, but also inside the anus. When passing a stool, they may get pushed out, but return unaided  Grade 3 - often called 'prolapsed haemorrhoids'; these appear outside the anus. The patient may feel them hanging out. They can be pushed back in if the patient presses with their finger  Grade 4 - these cannot be pushed back in and need to be treated by a doctor. They are large and stay outside the anus all the time. External haemorrhoids are called perianal hematoma. These are small lumps that are located on the outside edge of the anus. They are very itchy and can be painful if a blood clot forms inside (thrombosed external haemorrhoid). Thrombosed external haemorrhoid requires medical treatment straight away PILES OCCUR when the blood vessels around the anus and in the rectum will stretch under pressure and may swell or bulge. Inflamed veins (haemorrhoids) can develop when pressure increases in the lower rectum. This may be due to:  Chronic constipation9  Chronic diarrhea10  Lifting heavy weights11  Pregnancy12  Straining when passing a stool.13 The tendency to develop haemorrhoids may also be inherited. The risk of developing piles grows with age Treatments for piles In the majority of cases, piles resolve on their own without the need for any treatment. Treatments can help significantly reduce the discomfort and itching that many patients experience. Diet - piles can be caused by too much straining when doing bowel movements, which is the result of constipation. A change in diet can help keep the stools regular and soft. This involves eating more fiber, such as fruit and vegetables, or switching your cereal breakfast to bran. Water is the best drink, and the patient may be advised to increase his/her water consumption. Some experts say too much caffeine is not good. Simple things you can do yourself to help prevent piles:10  Try not to strain when you go to the toilet  Avoid laxatives  Exercise. Ointments, creams, pads and other OTC medications - there are some over-the-counter (OTC) medications which help soothe the redness and swelling around the anus area. Some of them contain witch hazel, hydrocortisone, or some other active ingredient which can relieve symptoms of itching and pain Surgery - used for particularly large piles, or grades 3 or 4 haemorrhoids. Generally, surgery is used if other procedures were not effective. Sometimes surgery is done on an outpatient basis - the patient goes home after the procedure.

Hi. I am 16 years old male. I have stomach pain on regular intervals (once or twice every week) and loose stools also for last 2 years. Whenever I eat any junk food for even once. My stomach pains. Pls help. What should I do other than taking medicines for lifetime. I also suffer from lactose intolerance also. please help.

Stop junk food first of all. Always foment your stomach with hot water bag after applying castor oil. Daily in between meals (i.e half meals--this medicine--remaining meals) take 1tsf Ajwain (carom seeds) + 1tsf ghee + warm water.

Hi doc I had my last period on dt 17.04. 16 we had protected sex but at time of intercourse condom had stuck but liquid was not spread not cut either tired yesterday I had red spot and cramps in my stomach still there is a chance of pregnancy?

Dear Lybrate user. Phimosis is a very common problem. In most of the cases You can clear the problem yourself. Apply oil on your foreskin and on the glans. Move the foreskin forward and back slowly without hurting you much. Because the oil is lubricant, it will be easier than without oil. Do this forward and back movement 20 times several times in a day. Repeat the same every day for around one month. You will get the elasticity and you will be able to retract the foreskin completely. Take care.

What are condiments that helps a ckd patient to improve the functioning of kidneys, whose serum creatinine is above 1.5 and below 2.5.

Few tips on Diet for kidney disease - have a low protein diet (decrease in milk and its products and also decrease in the intake of pulses and nuts etc)- can use corn oil, sunflower oil, olive oil for cooking -decrease the intake of butter, ghee, cream,coconut etc - avoid farsan food items - whole grain cereals are best for carbohydrate intake - get your blood checked for electrolyte levels regularly and adjust salt in diet as per the report values.

Dear lybrate-user, we recommend a proper planned diets for diabetics you need to maintain the gi (glycemic index of your food to maintain the glucose levels in your blood. Please have small meals in your day (place at 2 hourly gap) take more of complex cards like roasted grains. Balance the sugar of your fruits by combining it with salads and soups. Take multigrain chapatti to reduce the gluten in your diet. Drink plenty of water and include some, cinnamon in your dairy preparation for maintaining the lactose of milk.

Erectile dysfunction can be completely cured with homoeopathic medicines if properly taken under the guidance of a qualified homoeopath. What is effective way of sleeping! Like having at a stretch 7 hrs of sleep is good or taking short naps like 2 hrs 3 times a day is good. Good in the sense making your memory and improving.

Dear Doctor, How do we find out which type of diabetes we are having. What are the safe sugar levels for fasting blood test and post meal test? David.

Type 2 diabetes usually occurs after. 20 years of age and type 1 diabetes occurs below 20 years of age. But that is not hard rule. Only blood test confirms which type. Normal fasting levels are below 100 mg%and post meal level below 140 mg%

If stool mixed with blood or mucous, watery and multiple times, persist for more than 14 days or associated with fever ; then only its a concern, otherwise leave it. Take home based ORS (Rice water +salt, Cocunut water, butter water (mattha) etc) as much as possible plus curd

A stinging pain in the neck that makes you feel broken from the neck joint could be a reason to worry. It is often accompanied by an aching head or a throbbing shoulder joint. Neck pain, such as this occurs due to an injury in the area resulting from sudden back and forth movement of the neck. It is referred to as whiplash as the nature of the pain and subsequent discomfort resembles the snapping of a whip. The pain is more like a sprain, but not exactly so. This can affect athletes or even any person in general who has faced an accident in recent past. Whiplash is characterized by a tearing of ligaments and tendons that connect one bone to another.

Causes of Whiplash:

  1. Sports that involve contact: Any extracurricular activity involving collisions or other forms of physical contact can cause neck injuries. Football and rugby players frequently suffer from Whiplash.
  2. Road accidents: Public transport has a host of disadvantages. Reckless driving can make the tendons of your neck tear. Auto accidents cause immense strain on your neck; it can also lead to Whiplash.
  3. Any kind of physical abuse: If you have gotten into a violent fight with someone your neck could suffer from a Whiplash. Instances of physical abuse where the neck is jerked or punched at can also result in neck strains. Domestic violence often leads to instances of neck strains.

Symptoms related to this condition:

  1. You may or may not suffer from tinnitus. Tinnitus is a physical disorder where you hear ringing sounds. It is quite distracting.
  2. Your vision might get blurry.
  3. You can experience irregular sleep patterns.
  4. You can find it hard to concentrate on any one thing for more than a while.
  5. You can grow irritable at the drop of a hat.
  6. Your memory may fail you. You might find it next to impossible to trace back a day's activity.

Some of the other direct symptoms are:

  1. Your neck might turn stiff preventing you from moving it left, right, up or down.
  2. Pangs of pain can be felt in and around your shoulders.
  3. Your upper arms can grow numb and tender.
  4. Your head can spin repeatedly or you could feel dizzy.
  5. Whiplash can also result in serious fatigue.

  • Fadogia agrestis: This herb can help in increasing the testosterone level, which is responsible for better sexual stimulation and other sexual functions in male patients. 
  • Lepidium meyenii: This is a herb that has originated in Peru and is also known as Maca. This herb can help in fixing infertility in male and female patients. It contains various elements like sterols, campesterol and sitosterol among many others that can improve fertility.
  • Saffron: This herb is known to boost the libido by leaps and bounds and is even used to garnish a hot glass of milk just before bedtime, as it acts as a good aphrodisiac when combined with milk. A strand or two can work wonders, as per Ayurvedic practices. 
  • Passion Flower: This is considered as a good aphrodisiac because it helps in reducing anxiety and stress, which can soothe a person and create a better environment for stimulation and excitement. It can be used by male and female patients alike. 

There are many herbs as per Ayurveda, which can be used as aphrodisiacs in order to cure sexual disorders. These can be taken on a regular basis, in moderation.

Eat green veg. More fiber foods. Avoid fatty foods, junk foods, oily meal, drink plenty of water. Take digestive meal. Syp. Aristozyme 10 ml thrice a day. Take tab. Charcol at night. Syp. Livoluk 10 ml at night. Tests for constipation. Stool c/s, examination of the rectum and lower, or sigmoid, colon (sigmoidoscopy) examination of the rectum and entire colon (colonoscopy). An x-ray of the rectum during defecation (defecography). Avoid smoking/alcohol if you take.

I am 73 Yrs old male. I had a brain surgery for sudural hematoma in 2004. And become alright soon. Of late I am having muscle pull (cramps) when I turn left of right side or when I bend down and during sneezing. Also my fingers and toes become stiff. What are they due to and what are the remedies? Physiotherapy is of no use. Kindly suggest some medicines.

You are most likely having cramps of muscles due to weekness. During acute stage you should apply some muscle relaxant like volini ointment or spray. Go for regular morning walk followed by free hand exercises. Have nutritious balanced diet. And plenty of water by mouth cramps of muscles mostly due to lack of water also. You will be all right.
