Dear lybrate-user In pregnancy, You can safely use routine remedies like ENO, buttermilk, decreased amount of spices in food and having small frequent meals to deal with gas problems. If it doesn't settle then do try antacid syrups instead of tablets. They don't have any negative impact on the baby. You can even try home remedies like isaphgol. Good luck.

REPORTING PLATELET COUNTS IN DENGUE – MANUAL OR AUTOMATED ? Which is more reliable ?

 Platelets are the smallest type of blood cells. They are important in blood clotting. When bleeding occurs, the platelets swell, clump together, and form a sticky plug that helps stop the bleeding. If there are too few platelets, uncontrolled bleeding may be a problem.
In the current season, dengue is not the only cause of decrease in platelet counts. Other causes like malaria, other viral infections, Vitamin B12 and folate defiiency etc can also cause reduction in platelet counts. That said, dengue patients may also clinically present without any decrease in platelet counts.

It is well-known that thrombocytopenia ( reduction in platelet counts) is one of the critical parameters in patient management. Therefore, it is very important that laboratories assess platelet counts with utmost accuracy. The normal range of platelet count in a healthy adult individual is 150000 - 400000/μL. Babies and children have different reference ranges.compared to adults Therefore please check on the lab report for normal age wize reference range. A count of 1.5 lacs would be considered as normal in adults but would qualify as decreased count in a child.

Platelets can be counted either on automated machines( automated method) or on manual blood smear by pathologist.(manual method).

Generally hospitals and laboratories measure platelet counts on automated hematology analyzers- as .these are simple to use and give fast results.However, they suffer from a very big disadvantage. This disadvantage is based on the principle on which these machines work.The machines are programmed in such a manner that any blood particle falling within a predermined size range is counted as platelet and above this range is counted as red cell(RBC – which contains hemoglobin and is responsible for red colour of blood).Although this concept works well in majority of cases, but machine readings are seldom reliable , especially when platelet counts are below 30,000/ cmm. Giant platelets will be counted as RBCs and the machine will give a factitiously low reading of platelets. - Again , if the sample has not been properly mixed at the time of collection, platelets will stick and form clumps. Again the size of these clumps will be more than that of individual platelets and machine will count these as RBCs , thus give a falsely low platelet count. Also, if the sample is collected in a periphery and takes a lot of time to reach the main lab , by this time the platelets would have swelled up due to presence of additives in the blood tube, and not be counted in platelets.but as RBCs instead. (due to size factor)
So what is the solution, ?
All such cases, where platelets are reported low on analyzers, must be screened on peripheral smear by pathologist. Only on looking at the peripheral smear , will the pathologist be able to confirm whether the platelet counts are actually low or not.

Summary : The analyzer is reliable in majority of cases . However, for all cases with low platelets, manual screening by a pathologist is must for confirmation and to avoid unnecessary panic and unrequired platelet transfusions.

Gym exercises are not recommended during pregnancy. But yes, a few yogic asanas can be done during pregnancy. These asanas are different in each trimester of pregnancy and must be strictly done under expert guidance.

Treating Spinal Stenosis the Right Way

What is spinal stenosis: The narrowing of the gaps in the spine or the backbone is called as spinal stenosis. This leads to pressure on the nerves and the spinal cord. 5% of cases of spinal stenosis are seen in the low back of the lumbar spine. In some cases the narrowing of the spine leads to the compressing of the nerve root, which leads to pain in the leg.

Causes of spinal stenosis: It can be caused by aging, arthritis, trauma to the spine, spondylolisthesis and tumor of the spine

Symptoms of spinal stenosis: There is constant pain in the back and legs as stenosis pinches the nerve responsible for sensation and muscle power. Other symptoms are numbness and tingling in the legs, frequent falling and pain while walking.

Treatment of spinal stenosis:
- A routine program with exercises and physical therapy are the best treatment for spinal stenosis. The exercises are not a complete cure but they are beneficial in keeping the patient active. A physical therapy can be supervised and once the period of the same is over people easily transition to the exercise program. The key for exercise is starting slowly in order to build strength.

- Generally, the patients are counseled not to perform strenuous activities as that might worsen the symptoms of spinal stenosis. In case of lumbar stenosis, patients find it comfortable when they are flexed forward. The doctor might recommend walking while being bent over and leaning when using a walker if not walking upright; sitting in recliners is also recommended instead of straight back chairs.

- Injections of cortisone are usually given in the epidural space. This is believed to relieve the symptoms of spinal stenosis temporarily. Though there have been studies which question the use of these injections as they lead to the risk of osteoporosis related fractures. This is still being pondered over by the doctors as a suitable treatment option as it has its own benefits and risks.

- Medications such as the anti inflammatory medicines like ibuprofen or aspirin are helpful in controlling the symptoms of spinal stenosis. Narcotic drugs might also prove to be helpful if used with care and for a short while. This is helpful for severe pain related to the nerves. Muscle relaxants like gabapentins might also be recommended. In some case antidepressants might also be given to provide relief from pain.
Though side effects are a concern when using these medications and the patient should let the doctor knows about their allergies.

Agar 2 din bad fir se test krti hu or result negative aata hai to periods start krne k kya krna hoga. By chance agar test ka result positive aaya to tab periods start krne k liye kya krna chahiye. please help me out.

I'm almost 8 weeks pregnant now. Had a miscarriage in July due to chemical pregnancy and this September 6th I came to I'm 6 weeks pregnant. I feel nauseating alot and vomiting feeling only throughout day and morning sickness with vomiting. Wanted to ask what precautions should I take as eating wise. As of now I can't bear any cooked food smell. So I'm sticking more into sandwiches and juices would it be enough? And by when I can start prenatal yoga exercise. And do I have to be very careful with this pregnancy? but doc did say that it looks healthy pregnancy. Is it normal and safe to get immediate pregnant so fast after the last miscarriage?

Yeah you should be very careful with this pregnancy. In 1st trimester mostly ladies complain of nausea and vomiting. Dont panick. Eat whatever you like. After 12weeks problem will subside. After1st trimester you can start your prenatal yoga. Have small frequent diets. See a nearby gynaec, and do all the necessary investigations done so that if there is any problem, it can be sorted out at the early stage of conception. She will advise you some medicine for nausea and vomting.

Hello Doctor, My Mother's menstruation cycle is not getting stopped. her age is 43 yrs, She got her periods January 1st 2015 and then it continued for almost 8 days, later it stopped for 5-6 days and again started, this continued till date. Due to this her haemoglobin reduced to 4.5. So our family doctor suggested to get her admitted in hospital and get the blood transfusion done. So we admitted her on 28/01/2015 and continuous 3 days the blood transfusion was done. In total there were 3 bottles blood transfused in her body. Along with the blood transfusion Pause-MF tablets were given just to stop her bleeding, as her bleeding was heavy. Also through saline Pause injection was given. This procedure made her HB rise till 10 and also bleeding stopped, but as the injection course was over the bleeding started again in heavy manner. We consulted one Gynaecologist who gave us solution to insert Mirena Device which would stop her bleeding and other Gynaecologist suggested to remove the Uterus. So my question is which procedure would sense effective in case of this problem? Will Mirena insertion be suffice? Will it have any side effects in future on my Mother's body? Also when we are supposed to remove the uterus? Does it have any side effects on post removal? If yes then please let me know. Below are the details of the sonography reports and blood reports. HB previously: 4.5 HB current : 10 Pap Smear Test : Normal Sonography details: Clinical profile: Menorrhagia Uterus is anteverted and measures 120*59*83 mm The endometrail echo is central and is thick (17.9mm hperplasia) No focal/diffuse uterine abnormality seen. A well defined this walled cyst with clear contents is seen in the right ovary. It measures 52*39mm. The left Ovary is (32*17mm) looks normal. No andnexal abnormality is seen. There is no free fluid see in the cul-de-sac. IMPRESSION : Bulky uterus with a thick endometrial echo. For further avaluation. Right ovarian cyst. Tablets given to stop her bleeding: Xamic (2 times a day) course for 3 days. Just need a avdvise on which would be the right procedure to be followed? Mirena or Uterus removal and also its side effects ans future complications if any? Thanks
