MRI - LUMBAR spine 19,10.2016: Findings: Loss of normal lumbar lordosis seen? Due to muscle spasm. Lumbar vertebrae appears normal in alignment .No listhesis. Small Schmorl's nodes noted involving the endplates of multiple lumbar vertebra. Degeneration ofL1-L2, L2-L3 and L4-L5 lumbar intervertebral disc Posterior annular tear noted at L1-L2, L2-L3 and L4-LSlevels. At Ll-L2 level there is right paramidline protrusion, No foramina narrowing. At L2-L3, L3-L4 diffuse posterior disc bulge noted ,causing effacement of ventral thecal sac and inferior recess of bilateral neural foramina narrowing at L3-L4 level. At L4-L5 level, posterocentral and left forminal protrusion causing, significant canal stenosis and left neural foramina narrowing ,resulting in compression of exiting L4 and traversing LS nerve roots at this level. At LS-Sllevel, no disc bulge or herniation. Conus medullaris appear unremarkable. Impression: - Lumbar spondylosis degenerative changes as mentioned ,predominant at L4-L5 level. - At L4-LSlevel, posterocentral and left forminal protrusion causing, significant canal stenosis and left neural foramina narrowing ,resulting in compression of exiting left L4 and traversing Left LSnerve roots at this level. Gel means Disc (Fat) the Liquid which play between the bones gap back side.

Hi. Nerve root compression is there. It may require surgery by removing the protruding disc. But if no symptoms of nerve compression are present then you can wait but kindly visit a spine surgeon for expert opinion.

Try few things for your back pain: - Apply Ice and Heat Heating pads and cold packs can comfort tender trunks. Most doctors recommend using ice for the first 48 hours after an injury -- particularly if there is swelling — and then switching to heat. You can use whichever you find comforting as long as your skin is protected. - Limit Bed Rest Studies show that people with short-term low-back pain who rest feel more pain and have a harder time with daily tasks than those who stay active. -Keep Exercising Activity is often the best medicine for back pain. Simple exercises like walking can be very helpful. But remember to move in moderation, Stay away from strenuous activities like gardening and avoid whatever motion caused the pain in the first place. -Maintain Good Posture Most people have poor posture when going about their daily activities, putting unnecessary strain on their backs. -Strengthen Your Core Most people with chronic back pain would benefit from stronger abdominal muscles. If the abdominals are weak, other areas must pick up the slack. When we strengthen the abdominals, it often reduces the strain on the lower back. - Improve Flexibility Too much tension and tightness can cause back pain. By increasing flexibility you need to put an equal load throughout the body from the feet all the way up to the head. -Sleep the Right Way The amount of rest you get is important, and so is the position you get it in. Sleeping in a bad position or on a mattress without support can cause back pain. Back sleepers should put pillows under their knees. Side sleepers should place pillows between their knees to keep their spine in a neutral position. Stomach sleeping causes the neck and head to twist and can put undue stress on the back. -Use Relaxation Techniques Research shows that practices such as meditation, deep breathing and yoga, which help put the mind at rest, can do wonders for the back. -See a Specialist Developing an individualized exercise plan is essential to managing chronic back pain. Some patients need more core strengthening while others benefit mainly from stretching and improving flexibility. Please consult a Orthopedic Physiotherapist for the right exercise plan. Best wishes.

Hello lybrate user, most important thing in a hamstring injury is rest. Passive stretches till you can bear are great. Apply hot water bag fomentation to the local area a course of analgesics can be taken if pain is severe enough. Follow this and regain your movement god bless you.

Do contrast bath therapy Take hot water in a bowl & cold water in another & dip your hands upto the wrist in hot water for 3min then in cold for 1min, keep repeating for 20min, twice daily. Do wrist & hand stretchings

I am a 31 year old and I have left side lower back ache. I suffered with similar problem a year back but disappeared gradually. Now I am facing same problem for last 1 month.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. You have mentioned that this is your second episode of such kind of pain. If you have only localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Hi If you have localized back pain with - no neurological symptoms like numbness, paresthesia, weakness in limbs etc, my advice is: - intensive physical therapy initially under supervision of a physiotherapist and then at home. - avoid forward bending, lifting heavy weights etc - take an analgesic like diclofenac or something which suits your body for 5-7 days - take muscle relaxant for same time as analgesic to relieve muscle spasm - ice packs at the site of pain, muscle spasm - local analgesic spray can be used but no massage is required let me know how you are feeling. I will actually ask you to get x ray of involved region of spine ( ap & lateral) and send reports across to me so that I can advise further in a better manner. If you want treatment by your choice, then you can go to any road side bone setter. He will give you n number of exercises and manipulations, creams and lot many things. If you want proper treatment for your problem, then ask for treatment of choice. Locking of knee is mainly because of meniscus tear. Now ho ahead and get treatment from knee specialist or zola chhap. It's your choice.

You should prefer Homeopathic medicines....surgery is the last option. Give her Mag Phos 12 x - thrice daily for next 2 weeks. DO give hot and cold compress alternately for acute pain when required. For further queries get back to me.

you will get exact reason after doing MRI scan. in your case there is chances of ligament injury due to trauma . 1) Do massage with warm sesame oil or suitable oil for 15 min., afterwards take hot fomentation for 10 min. 2) Do regular stretching exercise Give feedback after 7 days.

Hi docs, i have severe pain in both my knees, and have been told its arthiritis. I am 67. I stya fit and am strengthening by legs, and all the relevant muscles.I swim play golf. Of late the pain also has increased. I have a high tolerance of pain. For medication I am taking Dolo 650 twice a day, and Glucosmine one a day. Can you suggest 1. A method to ease the pain. 2 . Is it ok to take DOLO650 TWICE A DAY. 3.Alternate medicines. For the pain . Thanks , and appreciate your effort in going digital.

Hi thanks for your query. Low back pain is usually not a result of delivery(whether normal or by a operation). It is due to weakness of back muscles, discogenic and sometimes deficiency of calcium/vitamin d levels. Please tell me her complaints elaborately with all attached investigations and I should be able to revert back to you with some solution for it. What is bmd in bones? what are side effects of excessive calcium supplements?

Dear, (bmd) is bone mineral density which calculates mineral matter per square cm in bones. It also means whether the bones have enough calcium in the matrix. Bones store calcium which is taken from the diet we consume. Normally calcium supplements will not have any side effects but are to be used in caution with people having high calcium levels in their blood or with hypertension. Excessive calcium supplementation is said to be associated with the risks of kidney stones, prostate cancer, constipation, calcium buildup in your blood vessels (calcified plaques), impaired absorption of iron and zinc (due to ionic competition). Calcium metabolism is complex involving the parathyroid gland, the kidneys, bones intestine and is regulated with vitamin d. Maintaining proper calcium homeostasis is the key function of the parathyroid glands and vitamin d. So you need to check calcium supplementation along with vitamin d. Please consult with your vitamin d levels.

First of all you need proper awareness regarding posture that how to sit, stand and work properly. Secondly check out your mattress which should be little bit firm not so hard or soft. Thirdly take towel then roll it sharply and put at your back (lumbar area) horizontally while lying in supine position for 15 minutes. You can go for hot water fomentation as well for 15 minutes too.

For leg pain, please follow the below advise: keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.
