As you have mentioned that your pain is activity related, then my advise will be: - ice packs at home 3-4 times in a day with limb elevation at night - local analgesic spray like dynapar qps spray can be used after fomentation - take a short course of anti-inflammatory medication as per your body's suitability - stop your physical activity till pain completely subsides - intensive physical therapy, training and proper muscle conditioning once pain has subsided - slow resumption of physical activities with proper pre activity warm up and stretching of muscles and short increments in duration of physical activity each day as per pain tolerance. Chronic Shoulder & Neck Pain - Ways It Can Be Diagnosed!

The neck and shoulder have a huge range of movement and thus they are more prone to injury. The ball and socket joint at the upper arm is the shoulder and both the neck and the shoulder are made up of soft tissues, muscles and ligaments.

Reasons behind pain Injured rotator cuffs: 

Rotator cuffs are the tendons supporting the shoulder. If they are injured it might lead to a chronic loss of the range of movement. This is more commonly known as a frozen shoulder. 

  1. Dislocation: Dislocated disc of the neck or shoulder due to an accident would cause severe pain in the affected area. 
  2. Fractures: Fractured collarbone or the upper arm bone may result in chronic pain. 
  3. Tendonitis: This occurs due to the over usage of the arms or neck or strains due to over exertion like in sports. 
  4. An unstable shoulder joint: If there is instability in the ball and socket joint of the upper arm, chances of having a chronic pain is high. Others: Degenerative arthritis, herniated disc, and even heart attacks can be the other possible causes of chronic neck and shoulder pain. 

When to go to the doctor? 

When the patient experiences a pain due to a severe blow or injury, the neck pain radiates down the legs and arms, a feeling of numbness and tingling sensation, or has a stiff neck with fever or vomiting, it is essential to seek the advice of a Doctor. 

Diagnosis for shoulder and neck pain X-ray: 

The main form of diagnosing a pain is an x-ray. It helps to reveal diseases from arthritis, tumour, slipped disc, narrowing spinal cord, fractures and spinal cord instability. 

  1. MRI: Magnetic Resonance Imaging or simply the MRI reveals the detail of nerve-related issues along with the problems in the ligaments and tendons. 
  2. CT scan: There may be instances when the injuries are difficult to be diagnosed. And, in the case of a fracture, a CT Scan is advised for more details. It is also an alternative to MRI. 
  3. ECG: An Electrocardiogram is a way of diagnosis if shoulder and neck pain continues along with chest pain or shortness of breath. If there is a risk of an individual having a heart attack, then an ECG is recommended. 
  4. Electro Diagnosis: Electromyography referred to as EMG and Nerve Conduction Velocity (NCV) are used to diagnose pain in the arm, neck, shoulder followed by a numb feeling on the affected areas. 

Treatment for chronic shoulder and neck pain 
A simple sprain on the neck and shoulder would demand a rest for a few days along with pain medication. If the pain is due to muscle spasms application of dry or moist heat may provide relief. In the case of a swelling due to injuries, a cold ice pack 4 to 8 times a day will provide immense relief. Patients might be advised a cast or sling for the shoulder pain. The soft tissues in the neck and shoulder can be treated with pain relievers taken for a few days. Muscle relaxants are also prescribed depending on the severity of the pain. Local corticosteroid injections are helpful for shoulder arthritis. Platelet rich plasma injection is another modern treatment for arthritis or tendinopathy. In some situation, radio frequency procedures are very helpful. Exercises are very effective for both neck and shoulder pain movement. Surgery might be an option if nerve roots are affected.

Backache after delivery is quite common and mainly because of wrong posture and calcium deficiency. So take proper back support while sitting and feeding your baby. Continue taking calcium tabs and vit d.

Although there can be many causes for Heel pain like spurs, fascial inflammation, infection, trauma etc but normally Plantar Fascitis is the most common cause especially if there is no history of any injury or any s/s of inflammation or infection like swelling, skin redness etc. This kind of pain is usually due to inflammation of your heel fascia. It is chronic pain which get aggravated especially when you walk more and is better with rest. Treatment modalities are aimed at relieving inflammation of the fascia, like ice packs, physical therapy , footwear modification and anti-inflammatory therapy. Foot and finger exercises. Do the following exercises barefoot: •1. First write digits 1 to 10 using your toes raised up in the air. •2. Sit on a chair, raise your toes and curl them •3. Pick up a small piece of cloth off the floor using your toes. •4. Still in a sitting position, point your toes and circle your ankles in a slow counter and clockwise motion. •5. Stand up from the sitting position and go on tip toe for 5 seconds.

Shoulder Dislocation and its Common Causes

A shoulder dislocation occurs when the bones of the upper arm snap out of your shoulder blade, generally after suffering a major injury. The joints of the shoulder are the most flexible body joints, thereby, making them more prone to dislocation. In most of the cases, a dislocated shoulder can be fixed within a few weeks’ time with the help of proper diagnosis. There is a high chance of suffering from dislocations over and over again. A dislocated shoulder, if not treated immediately, can lead to further complications such as damage to the blood vessels and nerve around the affected area or tearing of the associated muscles or ligaments.

What can cause a shoulder dislocation

You can experience a dislocation in your shoulder joints due to the following reasons:

  1. Shoulder dislocation can occur if you play any kind of contact sports such as football or hockey. You are more susceptible to a dislocated shoulder if the sports or physical activities you engage in regularly involve the risk of suffering from falls such as gymnastics or volleyball.
  2. A dislocated shoulder is a common occurrence if you have suffered a major motor vehicle accident that might have affected your shoulder.
  3. You can also suffer from a dislocated shoulder if you have survived a fall in the recent past such as tripping from a ladder or similar injuries.

Symptoms of a dislocated shoulder

You are suffering from a dislocated shoulder if you exhibit the following signs and symptoms:

  1. Abnormal enlargement of the affected area or bruising
  2. Severe pain around the shoulder area due to a recurrent muscle spasm in the shoulder joints
  3. Extreme pain while moving the joint
  4. A sense of numbness or a tingling sensation near the affected area, for example, in areas around the neck or the arm
  5. Visibly deformed shoulder joints

Treatment for shoulder dislocation

The treatment for shoulder dislocation depends on the severity of the injury. The following treatments can be implemented with suggestion from the doctors:

  1. If you had recurrent instances of dislocated shoulders, your doctor might suggest a surgery to remedy the damaged blood vessels and nerves around the area.
  2. Depending upon the intensity of the pain, the doctor might implement a general manoeuvre procedure to relocate your shoulder bones. After the procedure is conducted, you will feel your pain significantly reducing.
  3. Your doctor can also recommend you to wear a sling around your shoulder area to regulate the movement of the shoulder joints for the first few days to about 3 weeks, depending upon the intensity of the pain.

Most leg pain results from wear and tear, overuse, or injuries in joints or bones or in muscles, ligaments, tendons or other soft tissues. Leg pain can also be caused by blood clots, varicose veins or poor circulation. To relieve mild pain: stay off your leg as much as possible apply an ice pack or bag of frozen peas to the affected area for 15 to 20 minutes three times a day elevate your leg whenever you sit or lie down please go to a physiotherapist if: you have pain during or after walking you have swelling in both legs your pain gets worse your symptoms don't improve after a few days of home treatment you have painful varicose veins if you feel the pain is from muscle cramp then you need to drink a lot of water because poor fluid intake (dehydration) may predispose to cramps also low blood levels of either calcium or magnesium directly cause cramps. Kindly show me a photograph & digital x rays of affected part. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick matress are harmful use no pillow under the head. Do hot fomantation. Ibuprofen 200mg od & sos x 5days. Bio d3 max 1tab od x10. Do neck, back & general exercises. It may have to be further investigated. You will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take. Do reply back for detailed treatment plan. Do not ignore. It could be beginning of a serious problem.

Identify the mid point between the ankle bone and tendons and give pressure with a blunt edge on either side of your foot for 2 minutes. Also give pressure in the web between 2nd and 3rd finger from your toe for 2 minutes. Finally give a good massage for both feet in hot water. Identify the dipping portion on the back of your knee folding. Press this dip with both middle fingers and hold your knee with your thumbs for support. Please swing 20 times morning and evenings. This will relieve your problem. Consult after 10 days.
