I am 33 yrs old male I have partial tear of anterior cruciate ligament, joint effusion noted, mucoid degeneration /tear of posterior horn of medial meniscus. Is there any treatment without surgery.

MS - Orthopaedics, MBBS
Orthopedist, Delhi
How does it effect u, partial tear, if it less then grade 2, then it can be left alone, rather the medial meniscus tear which you have is more problematic, if you can near the pain, and your knee doesn't lock while standing from sitting. Medical treatment can be sought, otherwise go for surgery.

What You Should Know About Spondyloarthritis?

What You Should Know About Spondyloarthritis?

The bones and joints orchestrate very well with each other for smooth movements of various joints and the spine. However, due to various reasons, some identified and others unknown, there could be inflammation of the areas where the bones join the muscles leading to impaired movement. The range of motion could be reduced or it could be painful, thereby, limiting movement. Spondyloarthritis includes a group of inflammatory diseases that involves the bones and joints and enthuses which are areas where the ligaments and tendons attach to the bones. This causes conditions like plantar fasciitis, ankylosing spondylitis, psoriatic arthritis and reactive arthritis. The term spondyloarthritis is a blanket term to describe any of the above-mentioned conditions.

Read on to know a little more in detail about it:

Causes: While the exact etiology is not identified for sure, most patients with spondyloarthritis have an autosomal dominant gene (HLA-B27). Chronic infections could trigger the onset of the disease too.

Symptoms:

1. In all forms of spondyloarthritis, there is always inflammation of the spine, joints and entheses. If the fingers or toes are involved, there could be swelling leading to what are typically known as 'sausage digits'.

2. There is sacroiliitis which is inflammation of the joint between the pelvis and the spine, and spondylitis which is inflammation of the joints between the spines. 3. Family history and autosomal dominant gene.

Diagnosis:

A detailed medical history and physical examination form the cornerstone of a confirmed diagnosis. In addition, radiographic examination and diagnostic blood tests also could be done to confirm the diagnosis. Radiographs, especially, of the spine and the low back show typical changes including sacroilitis in suspected cases of spondyloarthritis. If the spine does not show these changes, but there is a high likelihood of spondyloarthritis, then MRI (magnetic resonance imaging) can be used to confirm the diagnosis.

As a final step, a blood test for the HLA-B27 gene can also be confirmatory. However, it is not true that a person with the gene will definitely develop the disease.

Treatment:

  1. The most important aspect of treatment is the use of pain medications and anti-inflammatory drugs (NSAIDs) which help manage pain and inflammation.
  2. Corticosteroid injections may sometimes be given directly into the joint spaces or the membranes to provide quick relief of these symptoms.
  3. ESWT / RSWT has a definite role to cure enthesopathy. Plantar fasciitis, and achillodynia can be cured with ESWT. Even small joints of hand and feet are amenable to treatment with RSWT. It is nonoperative and noninvasive treatment and has no side effects.
  4. Very rarely, stronger drugs such as methotrexate or sulfasalazine may be used.
  5. Another set of drugs known as TNF-blockers (tumor necrotizing factor blockers) are also shown to be highly effective in controlling the pain of the spine and joints.
  6. In very severe cases, surgery may be required to correct joint damages or correct severe degenerative changes.
  7. Exercise plays a very vital role in controlling the issue and improving range of motion. If you wish to discuss about any specific problem, you can consult a doctor and ask a free question.

7 Best Homeopathic Remedies For Fibromyalgia!

In the recent years, with the building work pressure and the increasing stress in our everyday activities, our regular lives have become choc-a-bloc with all sorts of engagements. This often renders our bodies prone to fatigue and listlessness. This phenomenon can often be observed in an intensified form by way of a disorder called Fibromyalgia or as it is deemed in common parlance, "rheumatism".

Fibromyalgia is an ailment, which causes pain in the body tissues. The tender and fibrous tissues like muscles, ligaments and tendons are often affected by this disease. As a consequence of Fibromyalgia, one feels incapacitated to work or perform any physical activity. Sometimes Fibromyalgia is accompanied by extreme headaches, sleep disturbances, muscle pain and consistent dizziness

The degree of chronicity of the disease may vary but the underlying cause remains the same. Given the high incidence of the disease, the treatment for the same is also widely available. However, for a more effective and lasting cure, many resort to homeopathy.

Homeopathy offers several remedies to Fibromyalgia, classified on the basis of the discrete symptoms of the disease:

  1. Rhus Tox: Rhus Tox is an excellent antidote for rheumatism induced stillness and muscle pain. When paired with suitable exercises, it can afford a lot of relief.
  2. Ruta Grav: Ruta Grav aids in relieving tendon pains and sustained usage helps in restoring muscle strength in case of Fibromyalgia.
  3. Rhododendron: Often the Fibromyalgia pain gets aggravated with a marked change in the season. Rhododendron mitigates the intensity of the pain.
  4. Arnica: For any injury induced body ache, Arnica is an extremely effective cure. It also functions as the foremost option, when the precise causes or the symptoms are unknown.
  5. Causticum: Certain forms of Fibromyalgia are accompanied with soreness, weakness and a feeling of stillness. Causticum soothes such ailments.
  6. Bryonia: Bryonia alleviates chronic pain in the soft tissues of the body. Exposure to heat needs to be avoided mandatorily in order to optimize the effect of Bryonia.
  7. Kalmia Latifolia: When besides the obverse symptoms of Fibromyalgia, the pain in the muscle is accompanied with a feeling of cold and numbness, one of the best homeopathic cures is Kalmia Latifolia.

Fibryomalgia is an ailment, which not only manifests physical troubles but also affects one's emotional status adversely. Therefore, to mete out the best remedy, homeopathy carefully assess both the physical and the psychosocial bearing of the patient before recommending a particular cure.

Most shoulder problems fall into four major categories: Tendon inflammation (bursitis or tendinitis) or tendon tear Instability Arthritis Fracture (broken bone) Other much less common causes of shoulder pain are tumors, infection, and nerve-related problems. Treatment generally involves rest, altering your activities, and physical therapy to help you improve shoulder strength and flexibility. Common sense solutions such as avoiding overexertion or overdoing activities in which you normally do not participate can help to prevent shoulder pain. Remedies... Cold Compress..... The simplest yet most effective tip in the list of home remedies for shoulder pain is cold compress as it can provide you with relief almost instantly. The cold will be able to help you numb the affected area, thus alleviate pain and inflammation..... Take some ice cubes into a bag of plastic and have the bag wrapped in a towel. Apply the pack directly onto the affected area for about 15 minutes. Perform couples of time every day in several days. Hot Compress.......... Similar to cold compress, it is also an extremely effective tip from the list of home remedies for shoulder pain which can bring you instant relief from swelling, inflammation and pain. It is recommended to use hot compress after the injury has occurred for 48 hours. This compress is also effective at easing the pain from muscles which are stretched out.Take a hot bag and fill it with hot water then apply directly onto the shoulder for about 15 minutes in a comfortable lying position. Perform couples of time per day. Also you can use the shower to run some slightly hot water over the shoulder for about 10 minutes. Maintain a still standing position during this time. Additionally, you might enjoy some hot shower twice every day. COMPRESSION.. You can use some kinds of elastic bandage or even a warm wrap which is easily found at the market. Have your shoulder wrapped for couples of day until the swelling and pain are subsided. Additionally, you should keep the shoulder elevated using the pillow in order to improve the healing. Note: Do not wrap excessively tight as it might lead to inefficiency in circulation and make the condition worse. Take 2 cups of Epsom salt into a warm bath and dissolve. Soak your whole body including the shoulder under the water for at least 20 minutes. Perform 3 times per week Take some oil and warm it up then apply directly onto the shoulder. Have the muscles at the shoulder squeezed and pressure gently in order to alleviate pain as well as encourage flow of blood. After massaging for 10 minutes, take a warm towel to apply onto the shoulder for maximized effect. Perform couples of time every day. Avoid massaging the affected area in case it incurs pain. Take 2 cups of vinegar into a warm bath. Soak your body including the shoulder under the water for at least 20 minutes. Perform once every day in couples of day. Also you can consume 1 tablespoon of vinegar mixed in warm water with some honey for twice every day in 1 week. The use of blackstrap molasses to reduce shoulder pain has been tested for many years. Even though there has not been a final conclusion yet, it is highly possible that its magnesium content is responsible for its benefits. You will speed up shoulder pain treatments at home very well. Another way to maximize the results for bruished shoulder is to use molasses with apple cider vinegar. How to do: You would need molasses (1/4 cup), apple cider vinegar (half a cup), honey (half a cup) and ground ginger (1 teaspoon). Mix all of them in water Consume it regularly every day USE NO PAIN OIL

Hot packs are very effective as they dilate blood vessels and increase blood flow, delivering needed nutrients and oxygen to cells in the area being heated, aiding the removal of cell waste and promoting healing.

For most low back pain treatments, active exercise is the focus of the physical therapy programDepending upon the patient's specific diagnosis and level of pain, the back pain exercises andrehabilitation programs will be very different, so it is important for patients to see a spine specialist trained to develop an individualized program of back exercises and to provide instruction on using the correct form and techniqu. Most of us will suffer from back or neck pain at some stage during our lives. The good news is, however, in most cases the pain is not serious in nature and can be remedied. Postural related back pain generally stems from sitting for long periods with overloaded or tight back and pelvic muscles. For example, if your rear thigh muscles (hamstrings) are tight, when you sit they will pull the pelvis out of position and increase tension on the lower back muscles. This in turn overstretches your back muscles and with lengthy periods of sitting will cause pain. This is one example of many variations of postural related back pain. However, one thing will be in common, progressively developing back pain from long periods of overstretched and overloaded back muscles and ligaments. When you first start to develop postural back pain it manifests itself as a low grade ache which tends to be relieved with light exercise or avoiding sitting. But after a while the pain increases and tends to be there most of the time regardless of what you do. It should be noted that a small percentage of back pain is caused by more serious conditions like a prolapsed disc or nerve compression. Symptoms that suggest a more serious back problem are leg pains, pins and needles or weakness in your leg or foot. These symptoms need to be properly assessed by a Chartered Physiotherapist.
