What is failure to thrive?

Children are diagnosed with failure to thrive when their weight or rate of weight gain is significantly below that of other children of similar age and sex. Infants or children that fail to thrive seem to be dramatically smaller or shorter than other children the same age. Teenagers may have short stature or appear to lack the usual changes that occur at puberty. However, there is a wide variation in what is considered normal growth and development.

Symptoms

In general, the rate of change in weight and height may be more important than the actual measurements.

Infants or children who fail to thrive have a height, weight and head circumference that do not match standard growth charts. The person's weight falls lower than the third percentile (as outlined in standard growth charts) or 20 percent below the ideal weight for their height. Growing may have slowed or stopped after a previously established growth curve.

The following are delayed or slow to develop:

  • Physical skills, such as rolling over, sitting, standing and walking
  • Mental and social skills
  • Secondary sexual characteristics (delayed in adolescents)

Diagnosis

  • It is important to determine whether failure to thrive results from medical problems or factors in the environment, such as abuse or neglect.
  • There are multiple medical causes of failure to thrive. These include:
  • Chromosome abnormalities, such as Down syndrome and Turner syndrome

Defects in major organ systems

  • Problems with the endocrine system, such as thyroid hormone deficiency, growth hormone deficiency or other hormone deficiencies
  • Damage to the brain or central nervous system, which may cause feeding difficulties in an infant
  • Heart or lung problems, which can affect how oxygen and nutrients move through the body
  • Anemia or other blood disorders
  • Gastrointestinal problems that result in malabsorption or a lack of digestive enzymes
  • Long-term gastroenteritis and gastroesophageal reflux (usually temporary)
  • Cerebral palsy

Long-term (chronic) infections

  • Metabolic disorders
  • Complications of pregnancy and low birth weight
  • Other factors that may lead to failure to thrive:
  • Emotional deprivation as a result of parental withdrawal, rejection or hostility
  • Economic problems that affect nutrition, living conditions and parental attitudes
  • Exposure to infections, parasites or toxins
  • Poor eating habits, such as eating in front of the television and not having formal meal times
  • Many times the cause cannot be determined.

Treatment

  • The treatment depends on the cause of the delayed growth and development. Delayed growth due to nutritional factors can be resolved by educating the parents to provide a well-balanced diet.
  • If psychosocial factors are involved, treatment should include improving the family dynamics and living conditions. Parental attitudes and behavior may contribute to a child's problems and need to be examined. In many cases, a child may need to be hospitalized initially to focus on implementation of a comprehensive medical, behavioral and psychosocial treatment plan.
  • Do not give your child dietary supplements without consulting your physician first.

It needs to be investigated. Investigations advised - hb, tlc, dlc, esr, sugar f & pp, uric acid, vit d3 urine r/e x ray of the affected part. Anyway you may try- sleep on a hard bed with soft bedding on it. Spring beds, folding beds or thick mattress are harmful use no pillow under the head. Do hot fomantation. Paracetamol 250mg od & sos x 5days. Caldikind plus 1tab od x10. Do neck back knee & general exercises. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take. If it does not give relief in 4-5days, inform.

Cause of Over sleeping a) Alcohol, smoking, drugs caffeine, sleeping pills b) stress, depression, fatigue inadequate/disturbed sleep, c) obesity, lack of physical activity d) Environmental factor like room is too hot/cold, excess light, noise, un cofortable bed e) Hypo thyroidism, diabetes, Heart disease f) sleeping dis order like sleep Apnoea (Breathing disorder where you stop breathing periodically in the night, due to blockage of Airway) , narcolepsy (day time sleep), Restless leg syndrome Treatment a) Have a proper sleep schedule-Get up and go to bed at same time everyday (body’s natural clock –cicardian rhythm will be accordingly activated) b) Good sleeping environment (Quiet, cool, dark, comfortable bed room without noise, light), c) Avoid caffeine/alcohol/nicotine 4-6 hours before bed time, d) Avoid day time sleep, daily exercise, light reading for 15 minutes daily before going to bed e).Avoid anxiety/stress/depression/fear/boredom/lack of motivation f) check your hemoglobin, BP,do a thyroid function test to rule out hypothyroidism g) consult physician for advice and treatment.

Hi,to overcome this weakness, take Alfavena malt, 2 tsf, twice daily for 1 month. BC no. 27, 5 tabs twice daily. Take Biofungin syrup, 2 tsp twice daily. Drink plenty of water. Eat a well balanced nutritious diet. Take Sound sleep of 7-8 hours. Avoid stress and anger, try to be happy, have a positive attitude towards life. Revert me after 15 days.

I'm 37 years old lady. I'm a science teacher. I'm suffering from hypothyroid along with asthma. Sometimes I get asthma attack and continuous coughing with breathlessness. At that time I need oxygen and nebulizer. I'm taking medicines thyrox, primosa, quogress tm, thyrocal for hypothyroid and odimont fx, hicope25, close ND, doxiflo650, formonide200 inhaler as prescribed by my doctors. But now a days I'm feeling trembling in my hands, legs and throat too. What may be the reason? And guide me properly for all above mentioned.

Hello Miss lybrate-user, I think there are two issues here 1. The recurrent asthma attacks. You need tp consult a chest physician and remain under close supervision. The drug has to be adjusted depending on the severity of your symptoms and frequency of asthma attacks. 2. The tremor is due to the formonide and any other drug (beta agonist) that you may be adding to the nebuliser. This is a common side effect and will go once the dose is adequately adjusted. Apply bhringraj oil on your scalp then give hot water fomentation. 1-1 drop of cow ghee in both nostrils at night time before sleep. Take pranacharya allergin capsule 1-1 twice a day. Take lots of proteins like soyabean. Egg white. Etc. Avoid tea. Coffee. Spicy food. Fermented food. Junk food. Apply only ayurvedic shampoo on your scalp. Do pranayama early morning.

Hi. As height can be gained till 21yrs. Of age in males and 18yrs. In females, so you do have 3yrs. More to grow further! start taking homoeopathic medicines which are safe to take without any side-effects - bakson's hite-aid and five phos. - both thrice daily and thuja 1m/ one dose once a week - take them for 4 months. Management: take milk and milk products which contains more calcium. More of protein (for cell multiplication) like pulses, legumes, sprouts, nuts, flaxseeds, non-veg, eggs, etc. Also, don't forget to take green vegetables, yellow veggies and fruits, seasonal fruits, salads, etc. Do skip rope daily, initially 10-20 times later gradually and slowly increase the frequency as per your capacity. Do get indulge in stretch exercises, and sports like basketball, volleyball. Last but not least, do take enough of rest and sleep (which helps in repairing of cells and cell rejuvenation). Get back to me after 4 months for further guidance. All the best!

There are some instructions are given below 1. Eat high fiber foods such as whole grain, oats and fruits such as sweet potatoes, pears, apple, plums, beans, broccoli or spinach help and make easy to passage of stool. *. Soak a few dates in milk, squash it to make a paste and take it with milk for a week to relieve constipation. If you are diabetic do not eat this. *triphala churna (a mixture of amla, haritaki, and vibhitaki) taken with honey twice a day is a great remedy for constipation. If you are diabetic so take this with warm water. *taking a teaspoon of ghee mixed in a glass of hot milk before going to bed. * boil a few figs (fresh and ripe or dried) in a glass of milk, and drink this mixture at night before going to bed. Figs have high fiber content and provide quick relief from constipation. *. Drink at least 3 liters of water every day to keep your body hydrated and improve bowel movement. * cut down on alcohol, tea, and coffee since they can worsen constipation. * regular physical exercise is a must for proper bowel movement. Pranayama and yoga are also beneficial. Yoga such as pavan muktasana, balasana, ardha matsyendrasana, and suptavajrasana are among the many yoga asanas that help relieve constipation. * take proper sleep of 6 to 8 hrs daily.
