Sir, my son 2 years old is suffering with heavy fever from 22 feb 2017, after treatment of oral based on our doctor's advise on 26 feb 2017 we admitted my son in children hospital. CRP level is 64. After two days of treatment the CRP level is decreased to 16. The fever also comes down from 104 F to 97 F. After one day of discharging from hospital my son again suffering with fever with 100 F temperature. In case sheet LRTI WITH SEPSIS. Now medicines are cefbact 750 mg through IV, linezolid, crocin DS, ascoril, maxtra and also nebulization. I search on the internet for this problem, there are many articles on this problem, I am fearing about my son future after the reading those articles. Rise in CRP alone can not diagnose a patient having sepsis. CRP can be rised in simple cold with high grade fever also and once the fever resolved and decreases CRP came down as you have mentioned. LRTI can be diagnosed by examining patients chest. It can be viral or bacterial and CRP can be rised in any of them. This much antibiotics or medication is not required in LRTI and none in viral. Sepsis is related to complications if it is in real present. Blood culture and patients symptoms both if looked in a 2 year child can confirm sepsis. Chest X-ray with examination of patient will confirm LRTI. In any case though if such big injections are started they have to be given for atleast 5 days if really needed. But problem what happened here is that antibiotics has been already started so blood culture if done at this stage may grow no organism. In this case patient can be looked symptomatically and has to be examined, get a blood culture if not done, Chest x ray is a must if some one said LRTI. All things need to be evaluated.

Are there any long-term effects associated with taking ADHD (attention deficit hyperactivity disorder) medications? If so, what are they and what medications are implicated?

MBBS
General Physician, Mumbai
Use of stimulant decreases the symptoms to a much good effect and if we don't give stimulant medication there will be more of conduct disorder and progress in disease and for side effects we have to monitor the person and advice them accordingly.

Crp is a indirect test for infection and inflammation. No need to worry if the baby is feeding well and active. However your pediatrician will examine and tell you what to do. Above value is abnormal. Baby needs follow up with pediatric / neonatal consultant.

My baby is 22 days old on his 21st day we had a naming ceremony in which I was served with normal diet which everyone is eating not special diet for new mother since then my baby is passing stool several times and it is smelling foul, but he is taking feeding normally and passing stool every time he is fed what is the reason is it there any problem.

MD - Paediatrics, MBBS
Pediatrician, Tumkur
There's no special diet for a new mother. You must eat normal food with extra calories as you are feeding. If you're exclusively breastfeeding, your baby cannot get diarrhoea. Some babies do have frequent semisolid stools as soon as fed due to exaggerated gastrocolic reflex. You must continue breastfeeding.

It is a protozoa infecting gut which may remain silent or may cause abdominal disturbances like pain in abdomen, loose motion etc. It is not so serious directly but may interfere in absorption of nutrients.

Hi my son is almost 6 yrs old and has been suffering from fever on and off for the last 2 and half months. We took all tests including urine test, blood test, chest x ray, ultrasound on the abdomen. All tests were normal but the blood test alone showed that crp count was raising every time the test was taken. He has been on antibiotics (amoxicillin, pencilin, augmentin, pencilin iv) for the last 2 months and still falling sick almost every alternate week. While the doctors say there is some infection that is causing the crp count to increase they are not able to say what is causing it. Should I be worried? should I take him for a full medical checkup for thorough examination? please note that for the last 2 months they were diagnosing as tonsillitis, viral infection, bladder infection, possible pneumonia but nothing came out positive in the tests. He does complain of eye irritation and pain in testicles sometimes. Please provide your insights.

Fever on and off since 2 months. Means for how many days each episode lasts? your history suggests it is not infectious cause. Measure the temp. With thermometer and record it. Fever charting is very imp. For diagnosis what is crp value any time esr has been done? what are the tests has been done till now? need to see in details. Child is not hospitalised that means in between fever he is fine. Any joint pain, joint swelling? history of tb in family or close contact? what has happened to weight in last 2 months. Most probably fever is due to inflammatory condition. Need to be worked up.

At 12 months hepatitis a first dose has to be given. But this is a optional vaccine. It helps in preventing water borne cases of hepatitis. So decreasing morbidity and school absenteeism. Next dose will b at 15 month of age which will b mmr and chickenpox. At 18 month dpt booster and polio booster will b there.

Asthma in Children: Diagnosis and Treatment

MAMC, MRCPCH, MD - Paediatrics, MBBS
Pediatrician, Noida
Asthma in Children: Diagnosis and Treatment

Has your child been coughing frequently? Is the cough chronic in nature, making your child breathe rapidly and does he/she complain about a tightened chest? These symptoms signify that your child is having asthma. Asthma is a medical condition characterized by paroxysmal wheezing respiration dyspnoea. It is common in children and an affected child experiences difficulty in breathing, and a whizzing sound is produced, especially during expiration. Asthma may lead to severe health complications and needs immediate diagnosis and treatment.

Diagnosis: The diagnosis of asthma is based on the symptoms, medical history and a physical examination of the child.

The different modes of asthma diagnosis are as follows:

  1. Medical history and symptoms: You must tell the doctor about any history of breathing trouble with your child or whether there are chances of other inherited health conditions. You must explain your child's symptoms properly, which may include coughing, wheezing, chest pain or tightness and others if observed.
  2. Physical examination: A physical exam will be carried out in your child where the doctor will listen to his heart and lungs, and look for eye or nose allergies.
  3. Medical tests: A chest X-ray of the child has to be carried out, along with a simple lung function test known as spirometry. This test measures the amount of air present in the lungs and determines how fast it can be exhaled. Spirometry enables a doctor to determine the severity of the asthma. Some other tests are also carried out for the identification of asthma triggers. They include allergy skin testing, blood tests and X-rays to know if sinus infections are affecting the asthma. An asthma test determines the amount of nitric oxide in your child's breath.

Treatment: Based on your child's severity of asthma symptoms and his medical history, the doctor will provide you with an action plan to treat the same. This action plan explains all the medications your child requires, the dosage and schedule of the medicines. The plan also includes points on what to do when the asthma worsens and when an emergency treatment is required. Anti-inflammatory drugs are prescribed to children who require bronchodilator medication. All asthma medicines used by adults can be used in case of children but in lower dosages.

You should give the asthma medications to your child using a home nebulizer or a breathing machine. A nebulizer delivers asthma drugs by transforming them from liquid to a mist. The child gets the drug by breathing it via a face mask.

In order to control and manage asthma in children, they must avoid the triggers and should keep away from any source of smoke. A doctor must be consulted to know about the best diagnosis and treatment methods.

Please let me know what's your baby is doing, if you mean baby is holding breath for longer time turns whitish/blue, if baby is becoming pale and remains playful , then it's breath holding spells, but if blue then she need immediate medical attention.

My son is 12 year old. He shakes his head frequently. Sometimes his shoulders also. Whats the reason and treatment?

M.D.( Pediatrics), DCH
Pediatrician,
Probably it is a tic, without any significance, but it's diagnosis by exclusion. One needs to physically examine.
