I am 23 years old male and recently encountered a fracture in my palm. The bone connected to my little finger got fractured in angular manner. Shall I go for a POP or a surgery where a wire will be inserted?

Fellowship in Neurological Rehabilitation, BPTh/BPT
Physiotherapist, Gurgaon
Hi, all dislocations or fracture dislocations should to be put back in joint (reduced) and held in joint until healed. Sometimes the injury will pop back into joint and be stable enough to hold in a plaster cast. If so they need careful supervision with repeat x-rays for 2-3 weeks from injury to make sure they do not slip out again. If the joint is not that stable it will need to be held with 1-3 wires passed through the skin known as k wires. These usually protrude through the skin so they can easily be removed in the outpatient's clinic. Some surgeons will bury the wires under the skin. Only occasionally is open surgery required i. E. A formal cut is made on the back of the hand over the injury. The wires typically remain in place for 4-5 weeks. During this time the fracture and wires will need support in a plaster cast either a full cast or a partial cast. Any fingers or parts of fingers not held by the plaster should be moved to reduce stiffness. Most patients do very well following these injuries. After an immobilization period, rehabilitative exercises should be performed to regain strength and mobility and to minimize chronic pain and stiffness. Thanks.

I am 14 years old. I used to have back pain while studying and doing homework. What should I do?

I am 14 years old. I used to have back pain while studying and doing homework. What should I do?
Hello. Maintain a right standing and sitting posture, keep your back always straight, avoid carrying weight on back. Hold or carry weight divided equally in both the hands or shoulders (to avoid over straining on one particular hand or shoulder). Lie down on bed on your back without pillow. Exercise: Practice Shavasana or Corpse Pose as it looks like a dead body. If practiced correctly and with full efforts, can relax every body part, relaxing not only the body but also the mind: Spread the legs one to two feet apart, the toes are turned outwards, the heels facing each other, a comfortable distance apart. Bring the arms a little away from the body, palms turned upward. Relax the neck and allow it to turn to the side if it is more comfortable. Close the eyes and focus the attention on the body, breathing normally. Medication: Take homoeopathic medicines - Arnica 200 and Rhus Tox. 200 - take both of them thrice daily for 1 week. Also, apply Schwabe's Topi-Arnica and Topi MP Gel on the affected area in circular motions gently thrice a day. Get back to me thereafter.

Compartment Syndrome: What Should You Know About It?

Compartment Syndrome: What Should You Know About It?

What is Compartment Syndrome?
Compartment syndrome is a type of condition that occurs when pressure builds up inside a muscle tissue in the body. Compartment syndrome usually occurs due to swelling or internal bleeding after an injury. The swelling should go away once the wound heals. However in some cases, if the swelling doesn't improve quickly, it leads to the accumulation of pressure. Let us have a look at the symptoms and the causes in depth.

Understanding Compartment Syndrome:
Compartment syndrome is not exactly a disease, but it hampers the flow of blood to and from the affected tissues. Groups of organs/muscles are organized into zones called compartments. During an injury, blood may start accumulating in these compartments, as a result of which the pressure inside the chamber rises restricting smooth blood flows. Severe tissue damage can result, with a loss of body function as well. Compartment syndrome can be fatal in some cases if not treated immediately. The common areas where this occurs are the legs, arms, and abdomen.

Compartment Syndrome Causes:
Compartment syndrome can occur immediately after an injury or they can happen when the treatment plan for the injury is under way. The biggest risk is that while some compartment syndrome takes days to form, few cases occur within hours. The acute form of the condition occurs when there is damage to the bone like a fracture to the hand and limbs. Other causes can include crush injuries, over-tight bandaging, burns and prolonged compression of an arm or leg. Blood clots also cause this condition in some cases, and so does strenuous physical exercises. A simple physical exam is used to diagnose compartment syndrome along with an X-Ray.

Compartment Syndrome Treatment:
Treatment for compartment syndrome works on the fundamental principle of reducing the pressure to the affected region. Dressings and casts that hamper the blood flow have to be removed immediately. Movement and bodily functions of the affected area have to be monitored vigorously. Surgery is one of the foolproof ways to ease the pressure. It is done by making long incisions into the skin underneath the affected area and releasing the pressure. The surgeries are done as minimally invasive procedures and can be clubbed together with the treatment plan the person is undergoing. In some cases, if the pressure is weak and not restricting the flow, then physical therapies can be undertaken. Medications can also help to an extent along with strength exercises.

With the advances made in medical science, doctors can identify potential compartment syndrome symptoms more quickly. Since these mostly occur when undergoing the treatment, the condition has a better chance of getting identified at its root. They can be avoided mostly with the experience of the physician.

For leg pain, please follow the below advise: keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise. Back. Extension exercises x 15 times x twice daily. Bhujang asana. Core strengthening exercises. Back stretchingstretch do the cat/cow avoid bending in front. Postural correction- sit tall, walk tall. Apply hot fomentation.

Hi I can understand your problem. Back pain is related to poor posture rather than height # relax & stay positive. # take hot and cold fomentation. # learn posture care exercise. # use lumbar support. # take break every 20 minutes. # do strengthening exercise of back and core muscles not all back pains are same.do share investigation reports if you have any.
