Definition


Persistent or recurrent inability to attain, or to maintain until completion of sexual activity, an adequate erection.

This is also know as erectile dysfunction.

The disorder may cause marked distress or interpersonal difficulty. There are different patterns. Some individuals report inability to obtain erection from the beginning of sexual experience ;while others report being able to experience erection only during masturbation or on awakening, but not during the coitus. Some experience adequate erection,lose it when attempting penetration. Still other report that have an erection that is sufficiently firm for penetration, but they lose erection before or during thrusting.

Male erectile disorder is frequently associated with anxiety, fear of failure, pressure of sexual performance, and decreased sexual excitement and pleasure. This can disrupt marital relationship and may be the cause of unconsummated marriage and infertility.

Stamp Test :A long strip of postal stamps in wound around the base of the penis at night before going to bed. Next morning if the perforations of the strip are found to be torn off, impotence is supposed to be of psychological in origin.

Peno-brachial index : The ratio of penile systolic blood pressure to that of brachial systolic blood pressure is normally 0.6. If found to be low, impotence is vascular in origin .

Papaverine Injection Test : Papaverine is a vasoactive drug. Using a 26 gauge needle, 30 mg papaverine is injected at the mid-shift of Corpus cavernosum of the penis. He is isolated, asked to stoke the penis and expose to erotic literature. An erection will occur in 10 to 15 minutes. If the erection is short-lived or partial, then impotence is considered as vasculogenic. If the erection is full, then impotence is considered to be neurogenic or psychogenic. In origin.

Caution : This test should be performed in a hospital setting or where the facilities for detumescence are available. The patient should be observed for next few hours till the erection subsides.

Rigiscan Test :

This is a gold standard for evaluation of Nocturnal Penile Tumescence and Rigidity (NPTR). This test is based on the physiological principle that a male gets erection 3 to 5 times during REM sleep. At night before going to sleep one ring of Rigiscan is slid over the base of the penis and the over the tip. Next morning, the tracing obtained are studied. This test can quantify erectile tumescence and rigidity. Rigiscan tracing indicates whether the impotence is organic or psychological in origin.

Arterial insufficiency and venous leaks can also be suspected on the basis of Rigiscan graphs. Patients with purely artery disease have low levels of rigidity but of adequate duration. Patients with venous leak will have varying rigidity levels with shortened duration.

Penile Ultrasound :

This test is for evaluation of the functioning of the penile arteries. A simple acoustic Doppler emits auditory signals or a colour Doppler can help visualization of arteries. Cavernosometry and caver nosography: this haemodynamic test is useful for diagnosing venous-occlusive dysfunction of the corpora. Biothesiometry, electromyography , nerve conduction studies: these are for evaluation of neurogenic impotence.

Treatment

Counseling

Sexual dysfunction is a marital unit problem and therefore both, husband and wife, should attend. Counselling is towards strengthening the marital relationship. Wife is requested to co-operate. The client is requested to quit smoking and alcohol. He is advised relaxation exercise, yoga. The couple is educated about anatomy, physiology of sexual organs sexuality are countered. Their myths and misconceptions about sexuality are countered. They are also explained that he does not have to do anything to have an erection. Erection is a physiological response to effective stimuli.

The sexual feelings that you are experiencing for ladies is NORMAL. But that just suggests that you have liking for female. It does not mean that for every sexual thought that you get with any lady, you must ask them for sex! NO lady will agree with you. Just like you have feelings, they must feel aroused by you too. It is for this that men and youth under go so much grooming and keep themselves fit and fashionable to be able to catch a lady's eye and then break into a conversation. You must learn skill of talking and trying to get the attention of the lady whom you like the most, this may get her to notice you. Good grooming, well manicured, hands and feet, good hair cut, trendy clothes, modest behaviour and gentlemanly approach is what the girls desire. Please do not mix your sexual feelings to them as a "natural" magnet. Girls do not agree for sex very easily. So make friendships with them first and then build your way.

The treatment depends on the cause of erectile dysfunction. Some of the common causes are, 1. Diabetes. 2. Prostate enlargement. 3. Heart problems. 4. Problems in low back affecting the low back nerves. 5. Low testosterone levels. In this regard I would like to advise you to get the following tests done and revert back with reports. Fasting and postprandial blood sugars, tsh, ultrasound pelvis and scrotum, serum testosterone levels. Once the cause is corrected erectile dysfunction will resolve automatically.

Dear user. I can understand. Increased intake of saturated fat boosts blood cholesterol levels. Such conditions cause a reduction in the sperm counts. Conditions promoting an increase in LDL levels in the circulation can also produce a decrease in the number of sperms produced and therefore, in male fertility. When cholesterol levels fall, testosterone synthesis would also be depressed. So high and low cholesteron can affect male fertility. Take care.

You are concerned about premature ejacaultion. Ejaculation of semen during sexual intercourse before or immediately after penetration. Premature ejaculation occurs when a man ejaculates sooner during sexual intercourse than he or his partner would like. Premature ejaculation is a common sexual complaint. Estimates vary, but as many as 1 out of 3 men say they experience this problem at some time. As long as it happens infrequently, it's not cause for concern. The primary symptom of premature ejaculation is the inability to delay ejaculation for more than one minute after penetration. However, the problem may occur in all sexual situations, even during masturbation. Premature ejaculation can be classified as lifelong (primary) or acquired (secondary). Lifelong premature ejaculation occurs all or nearly all of the time beginning with your first sexual encounters. Acquired premature ejaculation has the same symptoms but develops after you've had previous sexual experiences without ejaculatory problems. Many men feel that they have symptoms of premature ejaculation, but the symptoms do not meet the diagnostic criteria for premature ejaculation. Instead these may have natural variable premature ejaculation, which is characterized by periods of rapid ejaculation as well as periods of normal ejaculation. Psychological causes Some doctors believe that early sexual experiences may establish a pattern that can be difficult to change later in life, such as: •Situations in which you may have hurried to reach climax in order to avoid being discovered •Guilty feelings that increase your tendency to rush through sexual encounters Other factors that can play a role in causing premature ejaculation include: •Erectile dysfunction. Men who are anxious about obtaining or maintaining an erection during sexual intercourse may form a pattern of rushing to ejaculate, which can be difficult to change. •Anxiety. Many men with premature ejaculation also have problems with anxiety — either specifically about sexual performance or related to other issues. •Relationship problems. If you have had satisfying sexual relationships with other partners in which premature ejaculation happened infrequently or not at all, it's possible that interpersonal issues between you and your current partner are contributing to the problem. Biological causes A number of biological factors may contribute to premature ejaculation, including: •Abnormal hormone levels •Abnormal levels of brain chemicals called neurotransmitters •Abnormal reflex activity of the ejaculatory system •Certain thyroid problems •Inflammation and infection of the prostate or urethra •Inherited traits •Nerve damage from surgery or trauma (rare) Common treatment options for premature ejaculation include behavioural techniques, topical anaesthetics, oral medications and counselling. Keep in mind that it may take a little time to find the treatment or combination of treatments that will work for you. Behavioural techniques In some cases, therapy for premature ejaculation may involve taking simple steps, such as masturbating an hour or two before intercourse so that you're able to delay ejaculation during sex. You may try avoiding intercourse for a period of time and focusing on other types of sexual play so that pressure is removed from your sexual encounters. You can also try diverting your mind before ejaculation is ready to come and proceeding after a brief break, thereby prolonging duration of intercourse / action The pause-squeeze technique You and your partner can use of a method called the pause-squeeze technique. This method works as follows: 1.Begin sexual activity as usual, including stimulation of the penis, until you feel almost ready to ejaculate. 2.Have your partner squeeze the end of your penis, at the point where the head (glans) joins the shaft, and maintain the squeeze for several seconds, until the urge to ejaculate passes. 3.After the squeeze is released, wait for about 30 seconds, then go back to foreplay. You may notice that squeezing the penis causes it to become less erect, but when sexual stimulation is resumed, it soon regains full erection. 4.If you again feel you're about to ejaculate, have your partner repeat the squeeze process. By repeating this as many times as necessary, you can reach the point of entering your partner without ejaculating. After a few practice sessions, the feeling of knowing how to delay ejaculation may become a habit that no longer requires the pause-squeeze technique. Topical anaesthetics Anaesthetic creams and sprays that contain a numbing agent, such as lidocaine or procaine, are sometimes used to treat premature ejaculation. These products are applied to the penis a short time before sex to reduce sensation and thus help delay ejaculation. A lidocaine spray for premature ejaculation is available Although topical anaesthetic agents are effective and well-tolerated, they have potential side effects. For example, some men report temporary loss of sensitivity and decreased sexual pleasure. In some cases, female partners also have reported these effects. In rare cases, lidocaine or procaine can cause an allergic reaction. Oral medications Many medications may delay orgasm. Although none of these drugs is specifically worthwhile as people believe, approved medications may be prescribed for either on-demand or daily use, and may be prescribed alone or in combination with other treatments to treat premature ejaculation, some are used for this purpose, including antidepressants, analgesics and phosphodiesterase-5 inhibitors, like Sildenafil or Tadalafil You can ask me personally if you want these tablets with your full health details.

I removed my implant on Friday September 30 2016 at about 12 mid day and started the microgynon contraceptive pill at 11 pm the same Friday. I had sex 2 days after. Was it 2 early to have sex without condom since it was only 2 days since I switched to the pill? Am I at risk of getting pregnant?

Sores and Lumps on Genitals - How Can They be Treated?

Are you observing bumps and lesions in and around your vagina? These are female genital sores, which may be painful, itchy, produce a discharge, and are very tender. These lumps and sores may occur due to skin disorders. Female genital sores mostly appear as a symptom of STIs or sexually transmitted infections. STIs are considered to be a hidden epidemic, which many women leave untreated because of embarrassment or fear.

Diagnosis of female genital sores
A physical examination is conducted for determining the cause of genital sores and lumps in females. A pelvic exam test will be undertaken and you will have to provide reports of your medical history. Certain tests such as blood work and a culture of the sore may be required as well. A culture involves taking a swab sample from the affected sore and testing it for detecting bacteria. After the cause of your genital sores is determined, an ideal treatment method is prescribed.

Self-care methods
On observing the symptoms of genital sores, you can apply some self-care measures to treat them before you get to visit a doctor. You can try a sitz bath, which is effective for pain and discomfort relief. A sitz bath can be prepared at home by filling your bathtub with warm water, going up to your hips when you get immersed. You should add a mild saline solution or baking soda to the bathtub water. A small basin for a sitz bath can be purchased from a drug store.

Treatment

  1. The method or treatment for genital sores and lumps depends on the cause.
  2. Certain topical oral medicines are used for treatment of the sores and for relieving pain.
  3. Other medications such as antibiotics, corticosteroids, pain relievers, antiviral medicines and several anti-itch drugs are also used.
  4. Some types of genital sores do not need treatment, but you should get them removed if they bother you. A noncancerous cyst is an example of such type of a sore.
  5. The long term outlook for genital sores in females depends on the cause, once again. Sores and lumps caused because of chronic skin conditions and genital herpes are likely to recur. The long term outlook also depends on how fast you get an existing STI treated.

For the prevention of female genital sores, you should practice protected sex by using condoms. This prevents the infection from spreading to the people you have sex with. It is also important for you to open up about your condition and seek proper treatment, if you experience symptoms.

Hello, premature ejaculation occurs when a man experiences orgasm and expels semen soon after sexual activity and with minimal penile stimulation. Men with premature ejaculation often report emotional and relationship distress. Medication: take homoeopathic medicine - schwabe's damiaplant/ thrice daily for 4 weeks. Dear Lybrate user. Sex is always enjoyed if it is done with love, passion and affection. The love making time is important. It should be within 30 minutes. The more you try to extent it, the more you and your partner will be stressed and therefore the pleasure associated with orgasm will be simultaneously reduced. Therefore, limit the sexual performance to less than 30 minutes. Sex is energy consuming and your body needs proper rest and nutrition replenishment after every sex. Also keep in mind that you should provide 24 hours gap between two sexual intercourses. Sexual intercourse is not advised in the cases of stress, anxiety, physical fatigue, cough, cold etc. Take care.

The average incubation period for HIV (Antibody) is 25 days to 2 months. If you get tested before this time has elapsed, it is recommended that you test again to confirm your results once the incubation period has passed. Body fluids if in kiss also is infective.
