For a diabetic it is best advised to have a diet planned as per the sugar levels, lifestyle and history. Though you can follow some guidelines - have small frequent meals. Do not fast for more than 2-3 hours. Avoid a very heavy meal at one time. Avoid fruit juices, aerated drinks or sugarcane juice, honey, jaggery, sugar. Avoid junk foods. Exercise and stay active. Do not attempt to try out too many remedies as you could go into hypoglycaemia. Sleep well. It's all a matter of discipline.

I have very low vitamin d and increased uric acid and slightly increased thyroid. I came to know this last week. From that time I am taking medicine. What should I have to eat or drink and what I dont.

MBBS, MD - Internal Medicine
Internal Medicine Specialist, Faridabad
A high uric acid level can be caused when your body either produces too much uric acid or your kidneys don't eliminate uric acid rapidly enough. A high uric acid level may cause increasingly frequent attacks of gout, or it may never cause problems. A high uric acid level may also cause some people to develop kidney stones or kidney failure. And some people with a high uric acid level also develop high blood pressure, heart disease or chronic kidney disease, but it's often unclear whether this is a direct cause or merely an early warning sign of these conditions. Factors that may cause a high uric acid level in your blood include: Diuretic medications (water pills),Drinking too much alcohol,Genetics (inherited tendencies),hypothyroidism (underactive thyroid),Immune-suppressing drugs,Niacin, or vitamin B-3, obesity, psoriasis, Purine-rich diet — organ meat, game meat, anchovies, herring, gravy, dried beans, driedpeas, mushrooms and other foods Renal insufficiency — inability of the kidneys to filter waste Tumor lysis syndrome — a rapid release of cells into the blood caused by certain cancers or by chemotherapy for those cancers,Also, you may be monitored for high uric acid levels when undergoing chemotherapy or radiation treatment for cancer. Avoid protein diets, avoid more milk products. bad habits , fast food/fat/oily,salty foods. Coffee, smoking, alcohol intake. Non-veg., take whole grain, oats, green veg.,plenty of water in day time, 1 glass of water at night before bed.Take tab. Zyloric 100 mg once a day.Take tab. Bencid 500 mg once a day. Tab. Ulracet sos or tab. Naprosyn 500 mg once a day or sos for pain in joints.

if your pp sugar level is 300 mg/100 ml, then diabetes is not in control. You need to add some more drugs with the advise from your doctor. What is the hba1c%, is it below 7%? also how active you are, because exercise ad weight reduction helps in better control of blood sugar.

Hi doctor
My age is 36 but I am suffering from diabetes I have more stress in my job.
I am have sex problem pls help me.
Mr. Lybrate-user, in patients with diabetes erectile dysfunction is quite often seen complication. Most important thing is to have very strict blood glucose control, like fasting closer to 100 mg, pp 150 to 160 mg & hba1c% <6.8. Hence, you will have to work to wards achieving the above levels. As regards stress in the job, well you will have to learn to relax and reduce that stress level. There are multiple methods or ways to learn relaxation techniques. As regards relieving sex problems, there are a number of drugs available, which can be used.

What causes Hashimoto's thyroiditis, and what is the best method of treatment? Can iodine help this condition?

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. Lybrate-user, hashimoto's thyroiditis (ht) is an autoimmune disease with lymphocytic infiltration of thyroid gland, with anti bodidies agaist thyroid peroxidase enzyme (tpo abs) which destroy thyroid secreting cells, leading to deficiency of thyroxine - with resultant hypothyroidism. Thyroid gland is infiltrated with lymphocytes and may appear firm or lobulated. Anti tpo antibodies are detected in serum in patients with ht. One of the causes for ht is reported to be excessive consumption of iodine in salt. Other causes are infectious diseases may be as a trigger. Patients with ht usually have other family members also having some autoimmune disorders like t1 dm or rheumatoid arthritis, or they themselves may have some other autoimmune condition. Treatment ofr ht is giving thyroxine hormone tablets for life time. Iodine per say has no role in the treatment of this condition. In us this is the most common cause of hypothyroidism and is more common in women compared to men.

Hi there ~ Recommended foods in diabetes: Make your calories count with these nutritious foods: Healthy carbohydrates. During digestion, sugars (simple carbohydrates) and starches (complex carbohydrates) break down into blood glucose. Focus on the healthiest carbohydrates, such as fruits, vegetables, whole grains, legumes (beans, peas and lentils) and low-fat dairy products. Fiber-rich foods. Dietary fiber includes all parts of plant foods that your body can't digest or absorb. Fiber can decrease the risk of heart disease and help control blood sugar levels. Foods high in fiber include vegetables, fruits, nuts, legumes (beans, peas and lentils), whole-wheat flour and wheat bran. Heart-healthy fish. Eat heart-healthy fish at least twice a week. Fish can be a good alternative to high-fat meats. For example, cod, tuna and halibut have less total fat, saturated fat and cholesterol than do meat and poultry. Fish such as salmon, mackerel, tuna, sardines and bluefish are rich in omega-3 fatty acids, which promote heart health by lowering blood fats called triglycerides. However, avoid fried fish and fish with high levels of mercury, such as tilefish, swordfish and king mackerel.'Good' fats. Foods containing monounsaturated and polyunsaturated fats — such as avocados, almonds, pecans, walnuts, olives, and canola, olive and peanut oils — can help lower your cholesterol levels. Eat them sparingly, however, as all fats are high in calories. Foods to avoid Diabetes increases your risk of heart disease and stroke by accelerating the development of clogged and hardened arteries. Foods containing the following can work against your goal of a heart-healthy diet. Saturated fats. High-fat dairy products and animal proteins such as beef, hot dogs, sausage and bacon contain saturated fats. Get no more than 7 percent of your daily calories from saturated fat. Trans fats. These types of fats are found in processed snacks, baked goods, shortening and stick margarines and should be avoided completely. Cholesterol. Sources of cholesterol include high-fat dairy products and high-fat animal proteins, egg yolks, shellfish, liver, and other organ meats. Aim for no more than 300 milligrams (mg) of cholesterol a day. Sodium. Aim for less than 2, 300 mg of sodium a day. Take the help of a nutritionist if you feel like the above was helpful but you need more support. Hope this helps. Take care and have a lovely da

Hi, It would have been useful if you mentioned the reasons why Thyronorm was not suiting you? Further you did not mention the results of your thyroid function tests. I feel that based on your symptoms your Thyroid function tests may be abnormal. The symptoms that you are describing are of hypothyroidism, so looks like the dose of Thyronorm was not sufficient. Plan: Please get your TSH, fT3 and fT4 done. See an endocrinologist who can prescribe alternative brands of the thyroxine hormone drug. Thank you.

Hello Lybrate user, You should incorporate protein and fiber rich foods to your diet and limit the intake of simple sugars and carbs. Avoid fatty foods, fried, avoid too much of carbohydrate foods especially simple carbs like- table sugar, sweets, refined flour etc. Try to incorporate whole grain foods to your diet like- multi-grain flour, multi-grain bread, oats, dalia etc. Avoid saturated fat foods and have good fat foods like 5-6 nuts (walnuts, almonds). Have low fat milk and milk products. Will help keep your sugars in range and eat small and frequent meals. Good luck!

Causes, symptoms and treatment of Infertility

Hi, I am Dr. Jayanti Kamany. I am an IVF Consultant practicing at Srishti Fertility Centre and Women's Clinic in Mumbai. So, today i'll be talking about Infertility. The problem of infertility is on a steep rise. Some of the causes for the rise in inferility are increased age at the time of marriage for women, postponement of child bearing. Even if a women gets married earlier she postpones her child bearing for various purposes like her career.

Increased stress levels today, increased levels of sexually transmitted diseases and of course Tuberculosis, which is a very common disease in our country. So, who is to be blamed for the infertility. We have found that 3 out of 10 couples are battling with this problem of infertility. 30% of the faults is found in the female, another 30% is found in the male partner, again 30% is found in both partners and about 10-15% cannot be explained because inspite of all the tests being normal, there is no problem which can be found and we call it unexplained infertility.

There are two types of infertility:

1. Primary infertility- Primary infertility is when the couple has never conceived

2. Secondary infertility-  Secondary infertility is when the couple had a child but cannot conceive again, or the couple has conceived, has had an abortion but there is no conception again.

When should a couple consult or take treatment for infertility?

Usually if the ovum is young, if she is below 28 years she can wait for one year after trying to consult an Infertility Specialist. Even after one year if the couple doesn't conceive inspite of having regular sexual intercourse without having birth control measures they should see an Infertility specialist.

Now there are certain conditions where a couple irrespective of the duration of marriage or age should see a doctor immediately. So, what are those conditions? In the female, if she is having any problems regarding her menstrual cycle, suppose her menstrual cycles are irregular or she is getting very scanty bleeding or scantimensis or she has very severe pain during mensis or otherwise if she had a surgery where her one ovary is removed or a problem of polycystic ovarian disease which is very common today the she needs to see a Gynaec immediately. If she has recurrent abortions, that is, she can conceive but cannot carry the pregnancy to 9 months then definitely there is something wrong with her so she has to consult an Infertility specialist.

What are the causes in a male?

A male, if he has had a surgery in the testes or if he has a history of a disease called mumps in his childhood, or if he has a low sperm count or he cannot feel his testis in his scrotum then he needs to see a Fertility specialist.

For further information on this topic of Infertility or if you are planning to conceive, you can consult me on Lybrate.com.

You are having hypothyroidism and it is recommended that you consider taking thyroid medicine to correct that. Thyroid hormones are essential for the body metabolism and brain function. If deficiency is prolonged it can have wide range of problems from dry scaly skin to memory loss of brain. Consult with your doctor for treatment or contact me on lybrate. The kidney stones are not related to it. Try to drink plenty of water.

The gall stones are in no way related to the prostate. It is better to treat silent gall stones when they are silent and not during an acute attack. This is done laparoscopically. The prostate needs to be investigated in detail. I do lap choley and TUR prostate together at one sittingif the patient is fit as it avoids another operation. Both procedures are minimally invasive
