Exercise for Knee Health

I am Dr Rakesh Nair. I am an exclusive knee replacement surgeon. Today I am basically going to talk about how to prevent damage to the knees by doing the right type of exercises.  So, the topic today is going to be Osteoarthritis and Osteoporosis. How are they interconnected? How are both important in terms of treatment?

Let us start initially with Osteoarthritis. What do I mean by Osteoarthritis? 

Osteoarthritis means a worn out joint. The same knee which has souls start wearing out as you get older. Therefore, it is called a Primary. So, whether you have a problem at some age or not, this pain is going to happen to you, whether it happens at 25, 35, 40, 45 years. It might also happen at the age of 75 years. Most of my patients are going for surgery between the age group of 75-85 years. So here I am going to show you the right forms of exercises where your knee can last for the lifetime. So, we are looking at an average of 8 spans of 80-85.

So, will your knee last long?

The chances are very less because the present data says that the 80% of the general population has artificial knees because we are out living our knees. We are living much more than our knees can last.

What is Osteoarthritis? means what exactly happens to the joint?

The knees are made up of bones which are a thigh bone which is on the upper side and it is covered with the fine layer of the cartilage which is hardly 1 cm. The same thing is with the bone which is the Shin bone which is the lower part, again which is covered with the cartilage. Between these layers, we have a lubricant fluid connecting both the legs on the side we have strong ligaments which hold the joint together.

What exactly happen in Osteoarthritis?

In Osteoarthritis, the same cartilage which you see starts wearing out. There are small pieces which start wearing out. The joint which straight started getting bent which either like a bow like a deformity or a knock knee deformity depending on how strong or how weak your muscles are. When a  straight knee finally become bent then we have to look for some surgical options.

What are the symptoms of pain in the knee?

The first and the foremost symptoms are when a patient feels pain while climbing and getting down from stairs. So this is the first sign of Arthritis when you get to know that your knee is wearing out. Then they complain of pain, swelling. In the advanced stages, we will see the patients with bent knees, either bent inwards or outwards.

Today, I am going to tell you that can we prevent this damage?

For this, I am going to stress on exercise protocols for strengthening. Now, you need to understand that there is a difference between cardiac training and muscle strengthening. Most patients believe in walking as an exercise. But I would say that walking is the main form of exercise if not balanced properly with strengthening is going to cause wear and tear on the knees.

First, you need to strengthen the muscles which are there in the front and the back of the knees. So we are talking about strengthening the cardio steps and about strengthening the hamstrings. I would be giving you the practical demonstration of how we can do the same. 

These are the normal exercises which I ask 70 years old individuals to do.

  • Initially, the patient is lying down and push the knees downwards. Neeche dabaav daalo or thoda dheela karo. Push down and relax and do it till the count of 50. So, this is the basic first exercise you should do when you start any other exercise. 
  • The second exercise is leg race. This should also be from 5-50 counts. When you can comfortably do 50 counts at a time then we start the same in sitting posture.
  • Lie down with the face down and start bending the alternate knees upwards and then release. This is what you would do for a hamstring strength

So these are the exercises which you can do from the age of 20 years to 80 years. Depending on the muscle strength, you can gradually increase the intensity of an exercise and take it to the next level.

If you are able to strengthen your knees simultaneously as you increase your cardio activity, that would be the best form without any damage to your knees. Another thing is that if you can balance your cardio in terms of saying that if you could do cycling, swimming, walking or running and balance all 25% of your work out when you are working out in the gym. So when you are going to the gym, the first thing should be doing is not going to the tread mill, you should start with the cycling as it is non-weight bearing exercise. Do it for 10 minutes, gradually as you warmed up you can do cross stepper and lastly, you can go to the tread mill. That is the commonest mistake when we talk about cardio. Tread mill should not be the primary exercise when you go to the gym.

Maximum work can get by the trainers in the gym because most of the times you end up doing squats and crunches. Basically, your lower body is not that strong to take the whole grant of the weight. So, I would suggest that if you are 35, 40, 45 years and if you are little heavy in the upper body then please do not start with free hand squats.

First, do the machines. On machines, you can do leg extensions, hamstrings and press rings. And when you start feeling that your lower body is capable of taking weight then start doing squats and lunges. And if you are going to avoid it, it will not make much difference, you can increase your muscle strength in terms of your leg extension and hamstrings.

Running can cause irreversible damage to the knees after the age of 40-45. The 25-year-old individual can run because at the young age, the cartilage has the tendency for the recovery. But after the age of 35 years, there is degeneration. If you want your knees to stay healthy between the age of 35-80, then you need to take care good care of it.

Let me give you some tips on running.

It is important that you wear good running shoes while running and walk on a good walking track. Avoid walking on the roads. Please try to walk on the grass or a mud track. Because walking on the road may cause nerve stress. 

Before you start indulging into exercise, I would suggest you to have blood tests of  Calcium, Vit- D3 and Vit- B12. You will be surprised, when you are deficient in either of these and you are going to take the exercise to the next level, then you will face some damage. Only if the reports of these blood tests are normal then only take your exercise to the next level. Otherwise, there will always the chance of injury.

I always ask females to do basic bone density along with the blood tests which I told you previously. Maximum damage to the female body is done during menopause. But if your basic density of Vit- D3, Vit- B12 and calcium is normal then you will suffer from least damage even after the menopause.

What is sciatica? Please give the suggestion about sciatica and which are the symptoms of it. And how to be healthy from it? And how to prevent it?

Sciatica is pain in leg due to impingement of sciatic nerve in the lower back. Some time pain reaches to severe over the entire nerve root of sciatic nerve which is your lower back that causes buttock, thigh, lower leg and even foot numbness, pain, and sometimes even weakness. After looking to your pain I am recommending you something: Precautions: 1. Avoid forward bending, if required then bend your knees first and then bend forward. 2. Avoid weight lifting. 3. Sit in chairs or car seats with proper back support. 4. Place pillow under your legs while sleeping or can opt sidelying position for sleep. 5. Get enough vit d from sun as vit d keep bones strong including spine and other bones. 6. Drink plenty of water to enhance the intervertebral disks. 7. Avoid prolonged standing. 8. Change your sitting position every 15 minutes so that it does not pain. If possible visit some neuro-physio who can give you several modalities like SWD, IFT, traction etc to cure the pain. Along with it we have a special program designed in case of sciatica that is Butler’s mobilisation and Mckenzie exercises which a physio gives with the proper technique to get cure from disease. I understand your pain so I am suggesting you the following treatment that you can take at home and can get some relief from pain and discomfort. Treatment: 1. Use of Cold packs- Sometimes piriformis muscle gets tight which cause impingement of sciatic nerve leading to tingling and numbness sensation down into the leg. So ice massage can be given over the area of piriformis muscle or the entire back for 15 min. Once done with cold therapy take rest for 2-3 min to relax the muscles. 2. Since the pain is radiating you can do some below mentioned exercises which will give you immediate relief from pain if done correctly. A) Lie on bed on your stomach gently push up with your arms, resting on your forearms. Hold for 5 secs. Now slightly stretch your back muscles and count till 10 and relax. You might feel a slight pain initially but do not worry. Repeat it 15 times. B) Now that pain must have gone. And now you can push up onto your hands fully. Slowly lower down back to floor. Perform this atleast 15 times. Again a slight pain will be there initially. Once you will complete this exercise program your pain will be relieved right away. If pain is severe while doing the exercise then stop doing the exercise and immediately consult some physio to take care further. Other exercises that can be done in sciatica pain are: 1. Straight leg raising 2. Knee to chest. 3. Hamstring stretch. 4. Back extensors stretches and strengthening. 5. Gluteal stretch. All the above can be done for 10-15 times with the hold (stretch) time of 5 -6 secs Make sure to perform the exercises correctly which will give you relief from your pain and strengthen your muscles. Some times regular use of computer causes a group of problems called computer eye syndrome. Make sure don't use in dim light. Adjust the brightness of your computer well. Have the proper distace between eyes and computer atleast 30 cm. Correct your posture.

Knee pain. Quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Shoulder. Advice. Regular stretching of shoulder muscles. Warm up and cool down is a must. Hot fomentation.

I am 37 years, I have a flat foot, I can't stand for long, it pain very badly, and also burns which I can't bare. Shall I use any insole or is there any medicine to get relive and what shall I do?

Flat foot at times causes pain. But we must rule out other causes as well rule out diabetes & vit. D deficiency or any other metabolic disorder. Physiotherapy usually helps any way take paracetamol 250mg od & sos x 5days you will need other supportive medicines also. It may have to be further investigated do ask for detailed treatment plan. Do not ignore. It could be beginning of a serious problem. Must have x rays of your feet done. Make sure you are not allergic to any of the medicines you are going to take.

Rasoli operation is mild. Do not think about this. For back pain do regular back exercise. You can consult near by physiotherapist. Start daily walking--increase gradually to 40 minuets daily. Reduce oily, fatty diet, she can take b- complex daily one and any calcium tablet daily one. Drink good amount of water.

Follow these instructions to get relief from back pain: first of all check your posture when you are lifting any material like heavy bucket. And try to lift the heavy bucket without bending your back. You may lift the bucket after bending your knees only. This will definitely help you. * gentle massage with lukewarm mustard oil of your waist area will also give you relief. * surya namaskar is only exercise which you need to do. * in diet vegetable protein of cereal, b complex supplements and half teaspoon of haldi in a cup of milk for seven days will give you enough relief.

You must have a balanced high protein diet. Increase fresh fruits and vegetables in your diet. Protein sources like egg whites and soya. However if weakness persists get your thytoid, blood sugar & vitamin d levels tested and review.

I am 52 years well built male. Have low back pain above my left side hip joint if I walk for a long time or stand for some time. Was into weight lifting over 20 years.

This could be due to lumbar spondylosis with nerve root compression. Rule out diabetes & vit. D deficiency or any other metabolic disorder. Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Any way take paracetamol 250mg od & sos you will need other supportive medicines also. Do back (spine) exercises contact me again if need be. Make sure you are not allergic to any of the medicines you are going to take you may have to use lumbosacral support for some time. Avoid painful act/activities. Take medicines as prescribed it may have to be further investigated. Do reply back for private consultation and detailed treatment plan.

You may have to add some healthy fat. Include 1 gel of seven seas - sea cod oil every day during mid morning. Also include 5 almonds and 5 walnuts daily, 1 tsp flax seed powder with curd daily and 1 tsp olive oil on your subzi daily. Include 1 tbsp of ghee daily. Healthy fats can help to lubricate the joints and will help reduce the pain. Please start this and look for good results after following it regularly for at least 2-3 weeks. Please keep me informed. Avoid strain do rest. Tk heat fermentation and apply pain relief ointment. Sit straight maintain your posture. Avoid long sitting and toward bending. Avoid all exercise at time. If feel better then ok otherwise tk physiotherapy treatment for few days after relief start stretching exercise of spine and legs.

there is no specific “diet” that people with rheumatoid arthritis.but certain foods can help control inflammation like sprouts, pulses, fish, fiber rich foods etc. Yes you can do yoga.

Hand exercise dear you can do it easly. 1, Pinch Strengthener This exercise helps strengthen the muscles of your fingers and thumb. It can help you turn keys, open food packages, and use the gas pump more easily. •Pinch a soft foam ball or some putty between the tips of your fingers and your thumb. •Hold for 30 to 60 seconds. •Repeat 10 to 15 times on both hands. Do this exercise two to three times a week, but rest your hands for 48 hours in between sessions. Don't do this exercise if your thumb joint is damage Finger Lift Use this exercise to help increase the range of motion and flexibility in your fingers. •Place your hand flat, palm down, on a table or other surface. •Gently lift one finger at a time off of the table and then lower it. •You can also lift all your fingers and thumb at once, and then lower. •Repeat eight to 12 times on each han Thumb Extension Strengthening the muscles of your thumbs can help you grab and lift heavy things like cans and bottles. •Put your hand flat on a table. Wrap a rubber band around your hand at the base of your finger joints. •Gently move your thumb away from your fingers as far as you can. •Hold for 30 to 60 seconds and release. •Repeat 10 to 15 times with both hands. You can do this exercise two to three times a week, but rest your hands for 48 hours in between sessions. Thumb Touch This exercise helps increase the range of motion in your thumbs, which helps with activities like picking up your toothbrush, fork and spoon, and pens when you write. •Hold your hand out in front of you, with your wrist straight. •Gently touch your thumb to each of your four fingertips, one at a time, making the shape of an" O. •Hold each stretch for 30 to 60 seconds. Apply Hot Fomentation twice daily. Avoid bending in front. Postural Correction- Sit Tall, Walk Tall. Extension Exercises x 15 times x twice daily – Lying on tummy, take left arm up for 3 seconds, then bring it down, right arm up for 3 seconds, bring down. Bring right leg up, hold for 3 seconds, bring it down. Then right leg up and hold for 3 seconds and bring it down. Repeat twice a day- 10 times. Bhujang Asana? Lie flat on your stomach, keeping the palms out, bend the neck backward, take a deep breath and while holding it for 6 seconds, raise the chest up. Release breath and relax your body. Repeat the exercise 15 times twice daily. Core Strengthening Exercises- Straight Leg Raised With Toes Turned Outward, repeat 10 times, twice a day.
