Abdominal Pain Should Never Be Neglected!

MBBS Bachelor of Medicine and Bachelor of Surgery, MS - General Surgery, Fellowship in Minimal Access Surgery(FMAS) & Reproductive Medicine, PGDHHM ( Post Graduate Diploma Hospital & Healthcare Management)
General Surgeon, Vadodara
Abdominal Pain Should Never Be Neglected!

There are several Causes of abdominal Pain.It is a dangerous symptom and should never be neglected.Abdominal pain can be either Acute pain or it can be Chronic Pain.

Commonest Causes of Abdominal pain we see in day to day practice are:

1) Urinary tract stones

2) Gastroenteritis

3) Acid peptic Disorders, Reflux Esophagitis

4) Appendicitis

5) Cholecystitis

6) Systemic Causes: Sickle cell crisi,Metabolic Causes

7) Referred pain: Myocardial infarction, Pleurisy

8) Hernia: Ventral Hernia, Inguinal Hernia, Umbilical Hernia,

9) Malignancies (Cancer) and Tumours of GI Tract, Urinary tract,Retroperitoneum.

10) Infections: Tuberculous, Mesentric adenitis, Liver abscess,

11) Pelvic Causes ( Torsion Ovarian cyst, Ovarian tumours), torsion Testis,Rutured Ectopic Pregnancy.

12) Pancreatitis (Alcoholic, Gall stone)

13) Intestinal Obstruction

One should visit an Experienced General Surgeon who will first take detailed history, and then do a thorough Clinical Examination. You should properly give all history to the Surgeon including all past diseases( Take with you all old records, Investigation reports),Personal History, Family History, History of addictions if any,Appetite, Bowel, Bladder, Sleep, etc.

If required one may be subjected to any of the following Investigations:

Routine Laboratory tests

USG Abdomen ( Ultrasound)

Xray Abdomen

Sometimes CT Scan or MRI

Endoscopy (Upper GI or Colonoscopy)

Other Specialised investigations

Management:

Either Medical or Surgical:After a definite diagnosis is reached, Many causes are managed by Medications alone. Some of them requires Surgical Intervention, which can be either Immediate / Emergency Surgery or planned (Elective).

Examples of Emergency Surgical interventions:

1) Acute Appendicitis

2) Strangulated Hernia

3) Peritonitis due to any Cause.

4) Severe Hydronephrosis with large obstructing Urinary tract Stone

5) Acute Cholecystitis

6) Ruptured Ectopic Pregnancy

7) Torsion of Ovarian Cyst.

Surgeries of the Abdomen can be Either an Open Surgery or more commonly nowadays by minimal Invasive technique called the Laparoscopic Surgery.

The Surgeon should be left to take a call regarding method of surgery as per his/her experience, expertise and equipment available at the centre and patient condition. Not all surgeries are possible laparoscopically in all patients.

Advantages of Laparoscopic Surgery:

1) Minimal Invasive: Small Key Holes only on the abdominal wall. No big incisions,or injury to the body. Therefore minimal local and systemic inflammtory response.MInimal collateral damage to the body

2) Small scar: Better Cosmesis

3) Faster Recovery and back to routine work and life.

4) Less Pain

Good afternoon lybrate-user As per your information you are taking Mebex and Ivoral forte and you are finding better for 15 days and again worms are seen in stool. I feel necessary to inform you that 15 days is not the waiting period for total disppearance of worms from stool through the medicines you are taking. The adult worms will get killed in 2 weeks but the eggs do not get killed and they get hatched to become adults. As per my knowledge it takes 6 weeks of time including maintaining hygeine.

I have Gilbert syndrome and always feel anxiety and took alprazolam for many years but now doctor have me clonazepam sos and escitalopram 10 mg for two weeks and after that will increase to 20 mg. But I now wake up in morning with heavy head and have to take ambulax in morning with bed tea.

MD - Psychiatry
Psychiatrist, Chennai
Alprazolam on a longer run is dangerous and causes serious side effects. It is good to switch to escitalopram and clonazepam, but some develop heaviness as you said. You can discuss with your psychiatrist and change to better medications, which control anxiety well without heavy head. All the best.

It is important that you follow the following advice: Firstly, don’t miss breakfast, lunch or dinner. ... Secondly, avoid aerated drinks, tea, and coffee. ... Also, include milk and diary products in your diet. ... Don’t remain hungry for long periods of time. ... Raise the head of your bed if symptoms interfere with sleep. If your bed allows for it, raise the head of it by 6 to 8 inches. Gravity will keep the acid in your stomach. Don't just pile up pillows, though. These tend to bend your neck and body in such a way that increases the pressure. It will make the hyperacidity worse Consider losing weight. Losing weight may reduce some of the pressure on your lower esophageal sphincter, keeping stomach acid from leaking through Eat smaller meals. Decrease the amount of food you eat at any one time. This may reduce the amount of stress and pressure on your stomach For medication consult online in private.

Sir my husband is suffering from alcoholic fatty liver. Dr. suggested him sirrosam 400 mg. Is it good for him. What type of diet he should have?

Hello lybrate-user, Cirrosam 400 is a combination tablet of S-AdenosylMethionine, this compound/enzyme is naturally produced in a healthy liver. However due to repeated consumption of large quantities of alcohol the production of this enzyme is reduced. This enzyme normally helps by converting toxic free radicals called proOxidants, glutathione is one part of the three mechanisms that exists in our human body to protect us from free radicals/pro oxidants. S-AdenosylMethionine supplementation for an individual with alcoholic liver cirrhosis helps the liver do its job! In hope that it allows some recovery! Again the final definitive therapy would be a liver resection and or transplantation. Depending on severity of the disease process. Diet Low fat diet is advisable, as the liver produces something called bile to digest fat, you may want to cut down of fatty meals. Also support your diet with natural enzyme tablets. Please meet a clinical nutritionist and ask for a low fat moderate carbohydrate diet plan. This will be most beneficial for your husband. Hope this helps.
