Sleep Wake Disorder - Irregular Sleeping Patterns

Sleep-Wake Disorder is a very uncommon syndrome which causes irregular sleeping patterns without any real schedule.

Causes
Persons with brain malfunctioning and those who do not have a regular routine during the day experience such disorders. People with changing work shifts often experience such disorder. Also, travellers who often move from one country to another with different time zones may also have these symptoms.
There's no cure for this disorder, but treatments, including hormones, medicine, and light therapy, can help get you closer to a normal sleeping pattern.

Symptoms

Symptoms may include any of the following:

  1. Sleeping or napping more than usual during the day
  2. Trouble falling asleep and staying asleep at night
  3. Waking up often during the night

Exams and Tests
A person must have at least three abnormal sleep-wake episodes during a 24-hour period to be diagnosed with this problem. The time between each such episode varies from 1 to 4 hours. The doctor may advise you to go ahead with a device called actigraph in case the diagnosis is unclear. The device looks like a wristwatch, and it can tell when a person is sleeping or awake.

Treatment
Though there is no simple cure for irregular sleep-wake syndrome, however, several therapies and lifestyle changes may help a person return to a normal sleep-wake cycle. These include:

  1. A regular daytime schedule of activities and mealtimes need to be fixed.
  2. You should not stay in bed during the day for long.
  3. Use bright light therapy in the morning like those from computer screens and television. The period of exposure should also be increased. Exposure to blue light should be minimized at night.
  4. You should take melatonin at bedtime. This is a hormone that controls the sleep-wake cycle. By taking doses at the right times, you might be able to shift your body clock earlier or later.
  5. Make sure to make the room dark, quiet, and as comfortable as possible at night.
  6. Also one should try to minimize the amount of noise in the sleep environment.
  7. Add more structure to the patient’s day by scheduling social interaction, exercise, and other activities.
  8. Your doctor may prescribe drug to control the timing of the sleep-wake cycle.

Finally, you should take proper care of yourself. Make positive changes in your lifestyle. You can even ask your boss to make your work schedule a bit flexible.

The ultimate goal of these lifestyle changes is to help the person sleep longer at night and make him feel active and awake during the day. Most people have sleep disturbances on occasion. But you need to see your doctor if such sleep disorders occur regularly.

Hello, My mother is suffering from Bell's palsy since 20th nov16, as she is diabetic Dr. did not have her prednisone steroid within 72 hours after the attack. Now it been 3moths over Dr. asked us to stop physiotherapy now but it's not cured. We have consulted couple of neurologist but they are saying nothing can be done now after 3 months. My mother become too upset and me too. Should I go anywhere outside of Kolkata for further treatment, can it be cured, can she smile again. Please help me with your valuable suggestion .Thanks.

Best is she has been though physiotherapy how about she continuing her on daily basis, then blowing the baloon exercise ,blowing the same side of the face without baloon it will significantly improve over a period of time. It may not be 100 percent nrmal but will be close. Age is also one of the factors thats what I understand and experience. 2 warnings keep her away from TV, Stress, cold weather cover uself well these might aggrevate it.

Neurological Disorder - How Panchkarma Therapies Can Help?

Neurological disorders may creep in due to many reasons. It may be due to degenerative diseases, accidents, other health conditions, birth defects or genetics. The treatment is carried on for 45 minutes to 60 minutes by therapists or experts. Another form of the treatment requires the rice to be made into a paste, and then the paste is applied all over the body. But this is more preferred for beautification.

Causes of insomnia
There can be varied reasons for different people that can cause insomnia.

  • Unhealthy lifestyle i.e. improper sleep cycle, improper food habits, untimely naps, substance abuse etc.

Do you have any symptoms like snoring, interrupted sleep, unrefreshened sleep, morning headaches, daytime sleepiness etc, if yes it further supports the diagnosis.
It is a condition characterised by obstruction of upper airway especially during sleep due to lax airway and narrowing of airway due to obesity.
Basically, obstruction of upper airway leads to temporary stoppage of respiration, increased sympathetic drive and increased pulse rate. Also, other reason for pulse rate rise is deconditioning as you are not accustomed to activities.
Investigation
ENT examination if you have any symptoms of nasal stuffiness, recurrent tonsillitis to rule nasal causes of obstruction.

Polysomnography which is sleep study to confirm the diagnosis and extent of disease, pressure of cpap needed.
In treatment, first thing weight loss. You need to have low fat, calorie diet in order to loose weight. Cure for this is weight loss.
Secondly, cpap which is mask and device, and pressure during inspiration and expiration during sleep in order keep your airways patent and avoid this from happening.
Also, regular exercises is very important. Chest physiotherapy and yoga will be helpful.

PHYSIOTHERAPY TREATMENT OF HEAD INJURY

The treatment may comprise of the following measures:

IMPROVES ALERTNESS OR AROUSAL THROUGH SENSORY STIMULATION:

The patient who is drowsy or confused need to be stimulated by makes them more alert and awake. The therapist should encourage the patient’s cooperation during the treatment. The main aim is to stimulate the reticular activating system by making the patient sit or even stand in the tilt table.

The therapist should provide tactile, visual, auditory and Proprioceptive stimulation to the patient that will send facilitatory signals to the brain and will enable the alert response to be provoked. Auditory stimulation can be given by speaking to the patient during the course of treatment. Visual stimulation is given by showing familiar faces, objects or movement in the visual field of the patient. 

Proprioceptive stimulation by giving traction and approximation at joint structures is very helpful in stimulating the arousal response in the patient.

PREVENTION OF SPASTICITY:

As hyper tonicity generally sets in almost all head injury cases various measures need to be taken to keep them under control. Gentle passive movement, gradual rhythmic sustained stretch, prolonged icing for 20 minutes over the muscles, biofeedback, proper positioning are certain measures that needs to be employed for controlling spasticity.

MAXIMISE THE PATIENT’S FUNCTIONAL CAPACITY:

The main aim of this management is to improve the ROM, improve the control of voluntary movement, strengthening paretic muscles, improve the coordination, balance and teach various safety measures.

The treatment should be wide spread over the periods of time as the patient’s attention span and endurance is very less.

NEUROMUSCULAR TRAINING can be given through the development sequence by inhibiting abnormal movement pattern and by facilitating normal movement pattern.

The patient may give activities like bridging, prone on elbow, on all fours, side lying to sitting, sitting, kneeling, half kneeling, standing and walking.

PROPER DOCUMENTATION is necessary of the entire event through- out the day. Infact the routine of the patient should be maintained in the register and the patient need to be reminded of various activities especially if the patient has memory problems. The patient may be given register with photo and names of various health professional visiting him so that each day’s program can be entered. This will benefit both the patient and his acquaintance to know regarding the activities given to the patient.

USE OF VESTIBULAR BALL while training the patient for crawling, bridging, sitting, balance helps in building the Proprioceptive stimulation and teaches proper control to the patient.

Each task has various subtasks which need to be mastered by the patient so that he learns the actual activity using normal movement combination and performs it with precision. Like for training the patient to get up from bed, he may be taught to do asymmetrical push up with the trunk in partial rotation, then lower leg patterns are incorporated and finally the whole task of get up from sidelying is practiced.

REPETITION ACTIVITIES is key like any other neurological disorders. Ambulation training should always be done in upright position training the patient in each and every phase of the gait cycle. If the patient’s balance is poor then assistance may be used.

FUNCTIONAL ELECTRICAL STIMULATION has been shown more effective than kinetic joint training in certain types of cases. The upper extremity also appeared to use specific synergies for hand use in different positions. Clients often can opens hand in out stretched arm position but will be unable to perform the same action when the elbow is flexed. Some patient with minimal functional deficit in th upeer limb may be given some assistive devices or support for the hand so that they can perform some basic activity like eating, combing, writing, etc. this technique helps the shoulder and other proximal structures to produce appropriate movement sequences for hand use but does not facilitate hand function. The treatment however does provide whole task practice even though some basic component of the function is substituted by other means.

REVERSING TASKS in some patients helps in developing increased control by modifying a task or synergy as well as making the muscle work both eccentrically and concentrically. For instance lowering a glass of water on the table may help the patient in getting th glass close to the mouth by improving motor control of biceps during eccentric contraction.

Dear Mahipal, numbness is usually caused due to nerve compression and vit b deficiency. you need to add vit b complex in your diet either as supplementary or via natural food stuffs . Apart from this use a medicine Hypericum 200 once daily for 3 days and consult back for proper treatment.

Sleep is the best reliever of all stress and physical pain. Not only is it important to get sleep, but also to sleep soundly and without any disturbance. The quality matters. Here are a few tips to get better sleep: 1.It is important to maintain the same sleep schedule every single day. This helps to naturalize your body clock and get the best quality sleep. 2.Switch off all mobile phones, laptops and other electronic devices. Their artificial glow not only hampers your eyesight, but also prolongs the amount of time your body takes to relax and fall off to sleep. 3.If your back pain causes you to continually wake up in the middle of the night in discomfort, then sleep with a pillow or cushion between your legs so as to straighten out your back and relieve pent up stress. 4.The importance of your pillow can never be stressed upon enough. Your pillow should be not too fat, hard or soft. It should just about support the natural curve of your neck, so that it is perfectly aligned with the centre of your body. Correct sleeping postures are extremely important. 5.Make sure your room is nice and dark so as to stimulate the secretion of a hormone called melatonin that is regulated by light exposure.
