Protect Your Gentle Skin From The Big-bad Sun!

Mr Sun – Mr Golden Sun – Don’t Shine Down on Me!

It has been taught to us right from the time we were little toddlers that being out in the sun is great for us. And it is partially true that moderate sun exposure is great for health. From being a great source for generating vitamin D in our body… to generating an overall sense of wellness when we are outdoor in the sun.

However we do easily forget that sun is a very powerful energy source and the energy it delivers in the form if UV rays tends to seriously affect our skin. UV rays (ultra violet rays) are energy rays from the sun which are not visible to us and yet constitute a huge amount of energy that our body is exposed to.

UV rays are also of two types – the UVA rays which affect the deeper layers of the skin and the UVB rays which affect the superficial layers of the skin.

Is sun really the big bad monster?

Well the effects of sun exposure are far reaching on our skin. Here are some of them. I would like you to decide.

  1. Tanning – This the most basic effect in which case the skin develops pigmented patches on the sun exposed areas.
  2. Sun Burn – Prolonged exposure to sun often creates itchy red patches on the skin with blistering or peeling of skin.
  3. Freckles & Lentigenes – These are tiny red to brown spots that form on sun exposed areas.
  4. Melasma – These are brownish patches seen on cheeks, nose and forehead.
  5. Premature ageing – Ageing of skin is caused by damage or reduction in the collagen and elastin under the skin. This results in fine lines and wrinkles. UV rays directly affect the collagen and elastin levels.
  6. Allergic reactions – Although rare these are seen on the sun exposed area of the skin as tiny red or skin coloured patches.
  7. Thinning of skin and capillaries – In this case the blood vessels under the skin become more prominent.
  8. Skin cancer – It is seen more in fair skin individuals with a positive family history. Sun exposure is a definite associated cause for the same.

OK now how do we ward off the monster?

Well as always prevention is better than cure and hence the focus always needs to be on reducing sun exposure by all possible ways.

  1. Cover your body

There is no substitute for the simplicity of covering up your body when out in the sun.  Remember the following

    1. Full Length Clothes
    2. Caps and Hats
    3. Sun Glasses
    4. Use Umbrellas

1.For exposed areas:  Use a Sun Screen

Sun screens are the best source of protection for exposed areas. Apply sun screen before leaving from home and refresh it every four to five hours if out in the sun continuously.

In selection of sun screen although the SPF value is important it is not the only criteria. A sun screen with SPF value greater that 15 is adequate. Most sunscreens are broad spectrum sun screens and protect against UV-A and UV-B rays.

Note that in recent times water proof sun screens are also available which can be used when going swimming or during rains. (Yes sunscreens are advised even during rains!)

The biggest myth about sun screens is that they are not safe to use on a daily basis. This is incorrect and as a matter of fact they are safe for children too.

But I already have sun damage. Can I reverse it?

If you find your skin showing any of the effects mentioned above. Please get in touch with a dermatologist. The doctor will treat the sun damaged skin with oral and topical medications.

Also it is interesting to note that quite a few signs of sun damage like pigmentation and the unevenness in skin tone can be handled by a trained dermatologist with some procedures like:

  • Chemical Peels – These are medications which work on controlled damage of the old skin and help in growth of fresh skin.
  • Laser Skin Toning and De-tanning – Lasers work by selectively affecting the pigmented area of the skin. Laser exposure results in lightening of skin and also evening out the skin overall.
  • Skin Polishing – Skin polishing again works by controlled abrasion of the old damaged skin and stimulating fresh skin growth.
  • Skin Rejuvenation with Iontophoresis – Iontophoresis is a mechanism by which the dermatologist uses the power of electrical ions to carry medication into the deeper layers of the skin.
  • Anti-Ageing treatments – Sometimes the damage to skin in the form of wrinkles and lines is near permanent. This can be helped by means of using botox and fillers.

As they say, it always pays to be in the know. I believe that it makes perfect sense for all individuals to be well informed on the single most common skin infection they will ever come across. Ringworm.

So where’s the worm?

Well in reality Ringworm is not caused by a worm. It is a misnomer; Ringworm is actually a fungal infection. It is also called tinea.

Hey, I thought ringworm only affects children…

Well unfortunately; you would be wrong. Ringworm can affect practically anyone at any point in their life. It is a highly contagious disease that can spread from one part of the body to another and also from one person to another. There are some groups which are at higher risk of contacting the infection like children, athletes and pet lovers, but what is important to know is that it spreads primarily by contact with the infected person or animal and by sharing of objects like clothes, bedding, towels etc.

So what does it look like? What happens with it? Where do I spot it?

Ringworm presents as red itchy patches in the form of rings in which the centre is clear and the borders are active (red in colour). It is accompanied by intense itching, so much so that it affects routine life and even sleep.

Although it is most commonly seen on closed areas like groin, buttocks and under arms, it can occur on any part of the body including but not limited to hands feet, scalp, nails and stomach.

When it occurs on groin, it is called jock itch. When it occurs on feet it is called athlete’s foot.

Why Me?

Well on an everyday level we are all exposed to various bacteria, fungus and viruses. The fungus that causes Ringworm flourishes in a hot and humid environment and we are after all in India.

Aggravators for Ringworm include increased sweating, diabetes, weak immune system, tight or restrictive clothing etc.

 Close contact sports or activities like wrestling, contact with infected pets are typically responsible for Ringworm infections.

Ok, so what should I do if I get Ringworm?

Well the first thing to know about Ringworm is what not to do.

  • Do not reach for that itching cream you have seen advertised. Do not ask a chemist to prescribe a cream. Most over the counter creams only supress the itching and rashes temporarily without eradicating the fungus. Hence the infection bounces back on stopping the usage of the creams.
  • Quite a few OTC creams often tend to have a combination of various drugs which can harm the situation more than help it.
  • Please do not use home remedies. I have come across people applying garlic, turmeric, vinegar, oil etc… These are great for cooking, let’s leave them in the kitchen.
  • Then there is ‘Sapat Lotion’, this can seriously harm the affected area. It often burns the skin but fails to affect the infection.

How is it treated?

Visit a dermatologist. Do not self-medicate. The treatment will include oral and topical anti-fungal medication. And it will include medication that can control the itching.

Ha! I have been through it,  I am not getting it again.

Well unfortunately it’s not true. The infection can recur due to external factors. It is best to know how to prevent it. Here are some thoughts.

  • Stay dry
    •  Dry the skin completely after bathing.
    • Do not wear damp clothes.
    • Do not wear clothes on wet body
  • Wear loose clothing
  • Avoid sharing clothes, towels, napkins etc.
  • If you have pets, on any sign of infection in the pet get the same treated immediately.
  • Bathe immediately after exercise or change clothes after swimming
  • In case you have an infected person in the family, wash the clothes and linen of the infected person separately.

The word 'acne' is Greek in origin, which means'the highest point'. Unfortunately the condition causes more of ‘low points’ in people who suffer from it.

The Great Indian Jugaad does not work.

Almost – anyone & everyone – will go through this condition at some point in their life. And being a wide spread condition there are enough myths associated with it.

In my career I have come across crazy beliefs ranging from ‘irregular bowel movements’ to intake of ‘heat causing foods like mango fruit’ to consumption of ‘dairy products’ being blamed for acne. And of course every ‘ma’ and ‘dadi-ma’ have a ‘ghar-ka-nuska’(home remedy) for it, from applying ‘multani-mitti’ (mud / clay) to applying garlic.

Unfortunately not a lot of these ‘reasons’ and ‘solutions’are based on science and facts and in each of these case I have seen the patient’s condition being aggravated by these crazy ideas. 

So what’s the truth?

Acne is dominantly hormonal in nature and is caused by change in hormones which cause an increase in production of sebum (oil).This sebum along with dead skin cells blocks hair follicles. This environment is great for naturally occurring bacteria in skin to multiply, causing inflammation and acne.

Acne is primarily seen in adolescents; but can occur between 12 to 45 years of age. It is sometimes even seen in neonates (babies) due to circulating maternal hormones. It can get aggravated by certain conditions like

a.      Stress

b.     Hot and humid environment

c.      Hormonal Imbalance Conditions

d.     Application of oil or oil based products

e.     Certain oral medications

Acne is commonly seen on face, chest, back and upper arms.

ShortTerm and Long Term Effects:

In addition to an immediate and obvious deterioration in cosmetic appearance, cystic and hard acne leaves behind scars and marks. Even picking or squeezing of acne causes more marks and scarring. I have seen that this deterioration in cosmetic appearance often leads to a lower self esteem and lack of confidence in young patients.

So what do I do?

Here are some helpful dos and donts:

Wash your face twice a day

Remove make up before sleeping

DON'Ts

Don’t scrub or rub the face

Don’t squeeze the acne

Don’t apply oil on face or scalp

Don’t use home remedies or over the counter medicines.

7 Habits of Effective Skin Care

Remember these 7 Habits to have a healthy and glowing skin:

  1. Wash Up : It is important to keep your skin clean with simple washing up of exposed parts of the body a few times a day. However never over-do it and never scrub your skin. Skin is a delicate organ and needs gentle cleansing. Use soaps and cleansers which are mild and do not leave your skin feeling dry.
  1. Moisturize : You can never over-do this. Do not count the number of times you apply a moisturizer, the idea is to keep your skin healthy and soft. There is only one rule in moisturizers : Do not use oil. Oils do not act the same way moisturizing creams do. Also all reputed brands of moisturizers are safe and there is no substantial difference between expensive and in-expensive moisturizing creams.
  1. Sun Protection : Exposure to moderate sunlight has its health benefits, however prolonged exposure can cause substantial damage to the skin. Remember to always protect yourself adequately when going out in the sun. Always wear sunscreen when going out into the sun as it guards you from the harmful UV Rays of the sun. SPF factor of a sunscreen is a number to look at, higher the better, however it is not the only factor. As long as the sunscreen is from a reputed company it should be safe to use.
  1. Diet : A good healthy diet is all that matters. Get adequate proteins, vitamins, minerals and carbohydrates from your food and of-course hydrate with water as needed. Things that do not matter are the myths about foods that produce ‘heat’ in the body. Non-Vegetarian and Sugary foods are not your skin’s enemy, however like all things in life consume all foods in moderation.
  1. Exercise : Moderate exercise of your choice on a daily basis is proven to release ‘endorphins’ in your body and also it helps in increasing the blood circulation. These factors definitely affect the health of your skin and keep you looking young.
  1. Don’t Stress: Stress is now medically proven to affect your health adversely. It is also an established fact that stress causes aggravation of skin ailments and also makes you look older !
  1. Catch your ZZZs : Sleep is a big factor in overall health and also the health of your skin. In addition to getting 7 to 8 hours of sleep also try establishing consistency in the time you go to bed and wake up. Sleep increases collagen in your skin, the component that helps keep your skin looking younger.

The 8th Habit : Know when you need a doctor. Never ever ignore any skin ailment, a small thing today can blow out of proportion tomorrow. Use your sensibilities and approach a dermatologist whenever in doubt.
