Causes and Treatment for Infertility

My mother is 68 years old Type 2 diabetic / hypertension patient. Latest results are: HbA1c is 8.2. Hb is 10.4, Creatine is 3.7. She is currently on H.Mixtard 30 (26-0-10 ), telmikind (0-0-1), Thyronorm 12.5 (1-0-0). Thyronorm was started just 2 days back as we discovered she has TSH of 11. She is currently very weak, mentally confused, unable to walk without assistance. We believe that the weakness could be because of high TSH, and anaemia. We have started treating her for TSH, any ideas on how to treat anaemia considering she has CKD? Also, any opinions on what could be causing muscle weakness/ mental confusion etc? Thanks in advance.

Mr. lybrate-user, Thanks for the query. For a person with diabetes and hypertension, if both conditions are not well controlled, general weakness is always possible. Plus presence of hypothyroidism will add to it. With HbA1c% of 8.2, her diabetes is not in good control. It also has affected her kidney functions (high creatinine). No mention of her BP readings, so can not comment. TSH 11 mU/L is high, hence the dose of 12.5 mcg is bit too low. Hemoglobin 10.4 is unlikely to cause such a severe weakness. It is the effect of uncontrolled diabetes, hypothyroidism, hypertension and anemia. Hence first thing is to control her blood glucose and BP. Plus TSH should also should be brought to around 5 mU/L. It is also necessary to bring down creatinine level. This kind of a case has to be handled by the competent local physician. Thanks.

Thyroid Diet Chart In Hindi -

Thyroid Diet Chart In Hindi -

I have a thyroid problem from 13 years I am using thyronorm 100 mg tablets. If I decrease the dose then my thyroid level is increasing. Now I am married I want to know that in pregnancy I will face any problem and also I want to know that I can eat everything or else some of the food items I should not prefer.

If you keep your TSH level in the normal range in pregnancy, there will not be any problems. But, otherwise it can cause complication to the pregnancy or the fetus. Try to avoid raw food items like (cauliflower, cabbage). You can eat them in cooked form.

You are correct .The headache may be of different cause. Get her BP cjhecked daily at home and when the headache occurs get an eye Dr. -opthalmologist consultation since diabetics are more prone for glaucoma, cataracts as well as Retinal detachment yearly eye exam with a senior Dr. is needed. If these are normal get a ENT Dr. consult. Change her diet and lifestyle If you want specific dietary and lifestyle advise for sugar and cholesterol control take a consult.

Mr. lybrate-user, Thanks for the query. TSH 8.9 mU/L is high. This can lead to increase in weight plus there may be other symptoms of hypothyroidism. Usually for most patients the advise is to maintain TSH < 3mU/L to remain free from any symptoms of hypothyroidism. But if there are heart related issues slightly higher level of TSH is good enough. Because of reduction of dosage of thyroxine from 150 to 125 mcg, TSH has gone up. My suggestion is increase the dose to 137.5 mcg per day and then check TSH after 6 to 8 weeks. Thanks.

Hello, Madam, Thanks for the query. Hypothyroidism if not well controlled, can lead to subfertility or infertility. So it is essential to maintin TSH <3 mU/L. Then if you are looking for conception then TSH should be maintained around 2 mU/L. Otherwise conception may not occur. If occurs, fetal development is affected and later pregnancy outcome is also affected. So please check TSH level and if needed increase the dose of thyroxine tablets. Thanks.

Don't Drink Sugar Calories

MBBS, FIAGES-MInimal Access Surgery

Don't Drink Sugar Calories

Sugary drinks are the most fattening things you can put into your body.

  • This is because liquid sugar calories don't get registered by the brain in the same way as calories from solid foods.
  • For this reason, when you drink soda, you end up eating more total calories.
  • Sugary drinks are strongly associated with obesity, type 2 diabetes, heart disease and all sorts of health problems.
  • Keep in mind that fruit juices are almost as bad as soda in this regard. They contain just as much sugar, and the small amounts of antioxidants do NOT negate the harmful effects of the sugar.

Sir I am 42 years old and I am diabetic patient and when I was going to sex that time I am discharge with in second.
Hello- Premature ejaculation (PE) is a highly prevalent sexual dysfunction among patients with diabetes mellitus (DM). Microvascular complications of diabetes such as diabetic neuropathy is common and can justify the presence of PE due to the fact that ejaculation depend on the integrity of the autonomic nervous system and its central and peripheral neurotransmitters. Fortunately the condition is curable with the help of proper sugar control, dietary modification, exercise and reduction of weight and consumption of herbal aphrodisiacs.

Hello, Thanks for the query. Long standing diabetes whether very well controlled all throughout or not is likely to affect sexual function. This happens due to smaller blood vessels being damaged (supplying penile muscle). So it is better to consult a sexologist and follow his advise. Sometime drugs like PDE-V Inhibitors do help, to be taken just before the sexual act (about 1 hour earlier). Thanks.

Recently conducted thyroid test at thyrocare, T3 and T4 in normal range. TSH IS 7.51. What should I do next.

MBBS, CCEBDM, Diploma in Diabetology
Endocrinologist, Hubli-Dharwad
Mr. lybrate-user, Thanks for the query. TSH 7.51 mU/L, is above normal, but does not warrant any treatment as it is considered as sub-clinical hypothyroidism. In such situation treatment is given only if one has any of the troublesome symptoms of hypothyroidism. Otherwise you have to check TSH every 6 months to see if it goes beyond 10 mU/L, then treatment is needed. You should avoid soy beans and soy containing foods. Thanks.
