Any nicotine that is swallowed with saliva or washed down with eating or drinking will not be effective. If a substantial amount of nicotine is swallowed, it may cause such side effects as heartburn, upset stomach or hiccups. Thus, do not eat or drink while you have the gum in your mouth. Difficulty in Swallowing - Are You at Risk of Dysphagia?

MBBS, MS - ENT, DNB (ENT)

Difficulty in Swallowing - Are You at Risk of Dysphagia?

Swallowing food comes naturally to human beings. But when there is a problem, it is usually called Dysphagia. Esophagus, a muscular tube-like organ located at the back of our throat, usually helps in swallowing food and transferring them to our stomach. When esophagus does not function properly, dysphagia happens. Patients suffering from brain or nerve disorder, seniors and babies might have this problem.

People with dysphagia might witness the following issues:

  • Difficulty in swallowing solids and liquids.
  • Gaging, choking or coughing usually occurs when trying to swallow.
  • Experience pain when trying to swallow and also heartburn.
  • Swallowed food might come back up either through mouth or nose.
  • A chronic problem might result in weight loss.

Causes:
There are 2 main reasons, why the Esophagus might become dysfunctional.

  1. Due to some medical condition the muscles and nerves that help the esophagus work have stopped working.
  2. The esophagus is blocked by something.

There can be a number of reasons for both the condition. Here are the reasons why the muscles and nerves might not work.
Certain diseases can create problems with your nervous system, which in turn can affect the esophagus. These diseases are polio, multiple sclerosis, muscular dystrophy, Parkinson’s disease.

  1. A brain stroke, spinal cord injury or brain injury can also affect swallowing.
  2. Polymyositis or Dermatomyositis are immune system problems that can cause swelling or weakness.
  3. The muscles of esophagus suddenly squeeze, which is called esophagus spasm.
  4. Scleroderma causes the esophagus to become thin and weak.

The esophagus might be blocked because of these reasons:

  1. Esophagus might have malignant or non-malignant tumours.
  2. Esophagitis is a medical condition when the esophagus is infected, got some allergy or even if a pill got stuck on it.
  3. People suffering from reflux diseases often experience the acid that backs up into the esophagus. This can cause an ulcer on it resulting in scars. Scars make Esophagus narrower, making it difficult to swallow.
  4. There are small sacs called Diverticula on the esophagus or the throat, often making it difficult to swallow.
  5. Lymph nodes, tumours, bone spurs can also obstruct esophagus and create difficulty in swallowing. If you wish to discuss about any specific problem, you can consult an ENT specialist.

Constant Ringing in Ears - Causes + Treatment

Constant Ringing in Ears - Causes + Treatment

Tinnitus is a disorder which is characterized by a constant perception of a ringing noise in the ears. This is not the condition in itself, rather it signals some other underlying condition such as an ear injury or age related hearing loss. It is not a serious condition, although the symptoms can worsen with age. If you are affected by this disorder, you might experience or ‘hear’ a constant ringing sensation in the ears, even if there are no external sources of noise. These sounds could range from being a buzzing, ringing, to a hissing sound. This condition has mighty chances of interfering with your daily routine.

It is classified into two types:

  1. Objective tinnitus: This disorder is caused by muscle contractions or a blood vessel problem.
  2. Subjective tinnitus: The more commonly occurring type, it is caused by damage to the auditory nerves or regions of the brain that interpret sound.

The causes of this disorder are:

  1. Exposure to loud noise: If you are exposed to loud noises such as the ones emanating from firearms or heavy machinery over a prolonged period, it can lead to tinnitus.
  2. Aging: Aging can cause progressive loss of hearing, thus triggering tinnitus.
  3. Blockage of the ear: There is earwax present in the ears, the function of which is to trap dirt and bacteria. Excessive earwax accumulation leads to loss of hearing, resulting in tinnitus.
  4. Modifications of the ear bone: Any stiffening of the middle bone in the ear could impair your hearing and result in this condition.

Certain factors such as smoking, age, sex (men are prone to this disorder) and heart related disorders increase the risks of being affected by this condition.

Treatment
The treatment of this condition begins with the identification of the underlying condition, if any. The various treatment options are –

  1. Removal of earwax: Excess earwax has gotten rid of which can relieve symptoms of tinnitus.
  2. Suppression of noise: Certain machines, known as white noise machines, produce random sounds such as the sound of rain or the sounds of waves, thus eliminating the ‘hearing’ of sounds which are common to this condition.
  3. Medications: Certain medications such as alprazolam and nortriptyline can help reduce the severity of the symptoms. If you wish to discuss about any specific problem, you can consult an ent-specialist.

My 5 years old son is having frequent nose bleed for last one month. Though the amount of blood is quite less but the onset can be as frequent as twice a day. We consulted ENT doctor about 15 days back and he suggested its anterior and suggested paraffin drops along with Saline. This reduced the frequency but not entirely cured the problem. For last few days the frequency has again increased. We stay in Pune and the weather has been dry but changing now. He is taking Oleptal DT 150 twice a day for childhood epilepsy. Please suggest if we should revisit a specialist.

Children has tendency of nose picking. So nose bleeding (epistaxis) is very common in this age group. Inn summer there is dry air which causes dryness in nose which also lead to nose bleed. I suggest you that you apply small amount of ointment neomycin in anterior part of nose along with saline nasal drops 3 drops three times a day for minimum 5 days. And also encourage child to avoid nose pricking.

Olive oil can provide fast relief from an earache. It serves as a lubricant and helps get rid of an infection in the ear. Olive oil can also be helpful in stopping buzzing sensations inside the ears. Put 3-4 drops of lukewarm olive oil as ear drops into the ear canal. Alternatively, you can dip a cotton bud in the oil and put it firmly into the ear. You can also use mustard oil in the same way. For further queries get back to me.

I am snoring heavily at night. And even at evening nap also I am snoring I know I am snoring at evening but I could nt stop snorr. Due to heavy snore my friends and family are not able to sleep properly. But I am sleeping very well. But I seriously want to stop this snoring.

MBBS, DNB (Respiratory Diseases)
Pulmonologist, Mumbai
Hi I know that it can be very embarrassing to face snoring issues. That said, embarrasement should be least of your concern. You can have sleep disordered breathing or may be sleep apnoea. But don't worry you can get it easily assessed with a chest physician. Lifestyle changes like exercising and sleeping on your side can reduce snoring. ENT visit may be required to look into some nasal problem. Sleep apnoea, If ignored can lead to several issues.

Suicidal thoughts are indicative of severe depression, kindly consult a psychiatrist immediately .The word depressed is a common everyday word. People might say" i'm depressed" when in fact they mean" i'm fed up because i've had a row, or failed an exam, or lost my job" etc. These ups and downs of life are common and normal. Most people recover quite quickly. With true depression, you have a low mood and other symptoms each day for at least two weeks The common symptoms are (few might be present in a person with depression) Feelings of sadness, tearfulness, emptiness or hopelessness Angry outbursts, irritability or frustration, even over small matters Loss of interest or pleasure in most or all normal activities, such as sex, hobbies or sports Sleep disturbances, including insomnia or sleeping too much Tiredness and lack of energy, so even small tasks take extra effort Changes in appetite — often reduced appetite and weight loss, but increased cravings for food and weight gain in some people Anxiety, agitation or restlessness Slowed thinking, speaking or body movements Feelings of worthlessness or guilt, fixating on past failures or blaming yourself for things that aren't your responsibility Trouble thinking, concentrating, making decisions and remembering things Frequent or recurrent thoughts of death, suicidal thoughts, suicide attempts or suicide Unexplained physical problems, such as back pain or headaches General medical conditions and substances causing depression also needs to be ruled out.
