I am suffering from Ocd and I take medicine from psychiatrist Dr. sharita Shah it has been 8 months I am pretty well now but she ask me to continue medicine but she doesn't told me when to stop medicine she doesn't answer my call what do to.

MD - Psychiatry
Psychiatrist, Chennai
OCD is a long term many times life long disorder, so kindly continue as per her advise. Rather than disturbing her over calls, consult her in her clinic. If all patients start asking doubts and disturbing docs with calls unless it is an emergency, it will be tough on the doctor's part, understand this.

People start smoking in teen age, how could we prevent them as we know it is an adolescence related problem.

Reparenting Technique, BA, BEd
Psychologist, Bangalore
It is not only an adolescent related problem. Though we can say that adolescents are vulnerable due to many factors. I see youngsters as little as 8 years smoking. It is the influence of peer group, upbringing, genetic issues, broken families, curiosity, hormonal influences, style, rebelliousness, availability of cigarettes, imitation of idols, feeling like an adult etc. There is poor education of the ill-effects of smoking, primarily. Children have not learned good coping skills and adjustability. Home lives are threatened by parental discord, addictions, broken relationships etc. The attraction for instant pleasure etc. Education is the best way to do preventive intervention. Creating awareness has reduced smoking in the West by 1% annually, but the lack of the same has increased the smoking in developing countries by 2% in the same period!

Any symptom in Breast, such as swelling, redness and especially pain shouldn't be taken lightly. I would suggest you to get a Mammogram of both the breasts and get a routine blood test (Hemoglobin, Total count, Differential Count), ultrasound of the abdomen and a chest x ray. Also consult a Gynecologist with your reports. In case of any doubts with your reports you can consult me. Do not delay your mammogram anymore.

Can activated charcoal be taken for cleansing of stomach? I have digestion problems due to anxiety, fear and stress.

C.S.C, D.C.H, M.B.B.S
General Physician,
Can activated charcoal be taken for cleansing of stomach? I have digestion problems due to anxiety, fear and stress.
Cleansing stomach is a misnomer believed by common men and quacks. STRESS CONTROL There are many ways to tame your stress and keep it at bay. Here are tips to tame your stress today, and keep the stress monsters at bay. 1. Perform diaphragmatic or “deep breathing” exercises. 2. Lie face down on the floor and begin breathing deeply and slowly, with your hands resting under your face. Do this for five minutes. 3. Sit in a reclining chair. Put a hand on your abdomen and a hand on your chest. As you breathe, make sure the hand on your abdomen is moving up and down rather than one on your chest. If the hand on your abdomen is moving you are breathing deeply and slowly. 4. Try progressive muscle relaxation or “deep muscle” relaxation. Progressively tense and relax each muscle group in your body. Learn the difference between muscle tension and relaxation. 5. Meditate. Use visualization or guided imagery to help you learn to be one with your thoughts. Sit quietly with your eyes closed, imagining the sights, sounds and smells of your favorite place, such as a beach or mountain retreat. 6. Exercise regularly or take up yoga.

CONCENTRATION TIPS Here’s some tips to get into a state of deep concentration where work / or studies flow easily so that you can do well in examinations/ and or do your work well. 1) Cut Off the Noise Getting into a state of concentration can take at least fifteen minutes. If you are getting distracted every five, you can’t possibly focus entirely on your work. Request that people don’t interrupt you when working on a big project. If you are required to answer phones and drop-in’s immediately, schedule work when the office is less busy. 2) Structure Your Environment The place you work can have an impact on your ability to focus. Try to locate yourself so you are facing potential distractions such as doors, phones or windows. This way you can take a glance to assess sounds that would otherwise break your focus. 3) Clarify Objectives Know what your goal is clearly before you start. If you aren’t sure what the end result is, the confusion will make it impossible to focus. 4) Divide Blobs. If you have a large project that needs work, clearly identify a path that you will use to get started working on it. If the sequence of actions isn’t obvious, it will be difficult to concentrate. Taking a few minutes to plan not only your end result, but the order you will complete any steps, can save hours in wasted thinking. 5) Know the Rules Get clear on what the guidelines are for the task ahead. What level of quality do you need? What standards do you need to follow? What constraints are there? If the rules aren’t clear from the outset, you will slip out of concentration as you ponder them later. 6) Set a Deadline Deadlines have both advantages and disadvantages when trying to force concentration. A deadline can make it easier to forget the non-essential and speed up your working time. If you give yourself only an hour to design a logo, you will keep it simple and avoid fiddling with extravagant designs. Time limits have disadvantages when they cause you to worry about the time you have left instead of the task itself. 7) Break down Roadblocks Roadblocks occur whenever you hit a tricky problem in your work. This can happen when you run out of ideas or your focus wavers. Break down roadblocks by brainstorming or planning on a piece of paper. Writing out your thought processes can keep you focused even if you might become frustrated. 8) Isolate Yourself Become a hermit and stay away from other people if you want to get work done. Unless your work is based on other people they will only break your focus. Create a private space and refuse to talk to anyone until your work is finished. Put a sign on your door to steer away drop-ins and don’t answer your phone. 9) Healthy Body, Sharper Mind What you put into your body affects the way you concentrate. Nobody would expect peak performance if they showed up drunk to work. But if you allow yourself to get chronic sleep deprivation, overuse stimulants like caffeine or eat dense, fatty foods your concentration will suffer. Try to cut out one of your unhealthy habits for just thirty days to see if there is a difference in your energy levels. I’ve found even small steps can create dramatic changes in my ability to focus. 10) Be Patient Before I write an article, I often sit at my desk for a fifteen or twenty minutes before I put finger on the keyboard. During this time I feel a strong urge to leave or do something else. But I know that if I am patient, I’ll stumble upon an idea to write about and enter a state of flow. Without a little patience, you can’t take advantage of flow when it rushed through you. If you need strong concentration I recommend periods of 90-120 minutes. Any less than that and you will waste too much time getting started before the flow can continue. More than this is possible to sustain focus, but you will probably benefit from a quick break. These are the general guidelines for concentration, studying well or doing your job. Regards.

The fact that you have been taking anti anxiety drugs for more than 10 years and still suffer from migraine, anxiety and depression, means your problems are more due to your personality factors than physical causes. You seem to have a very low self esteem and are low on self confidence. You will need to go through intensive psychotherapy. Consult a good psychologist/ psychotherapist, who can help you understand your fears, mental blocks and help you overcome your low self confidence. Medicine can give you temporary relief, unless you change your thought process, you will continue to have difficulties. To begin with, focus on what'you' want in - stop thinking about what people expect you to do. Have a routine for yourself- healthy diet, lots of water (at least 8-10 glasses a day), some physical exercise- yoga, walking, aerobics, dancing- whatever is to your liking. Think positive and set daily goals for yourself.

Stress Management

Psychologist, Bangalore
Stress Management

What is stress?

There are situations in life when a person is required to perform beyond his/her normal capacity, or situations when some expectations are not met. This creates anxiety, fear and anger. This is a situation where a person feels inadequate, under-confident and experiences loss of control. This affects a person's physical and mental wellbeing. This feeling of fear, anxiety, restlessness is commonly known as "tension or stress".

Symptoms of Stress :

  1. Anger and Aggression: While under stress a person starts doubting his/her own capacity and efficiency because he/she feels inadequate or incapable of performing certain tasks. This creates a lot of anger or frustration and sometime, may result into aggressive behaviour.
  2. Irritability: Under stress people get irritated for no reason. There is loss of patience, loss of sense of humour.
  3. Sleep Disorders: A person under stress may not be able to sleep at all or in some cases sleep excessively.
  4. Eating Disorders: A person under stress may lose appetite or in some cases, tend to overeat resulting in harmful effects on body.
  5. Anxiety: Stress may cause a lot of anxiety and restlessness. The person under stress may not to able to concentrate and unable to focus on any activity, making things more complicated.
  6. Depression: In a few cases a person under a lot of stress may go into depression.
  7. High BP: Body and mind are closely connected. Hence stress in mind creates stress in the body and under stress, blood pressure may rise.
  8. High Sugar Level: Stress may increase the blood sugar levels.
  9. Headache or Unexplained Body Pains: Stress may cause headache or body aches in different parts of the body, without any physical reason for it.

Sometimes, there may not be any symptoms for stress but the trauma manifests in the form to heart attack, Stroke etc.

What causes stress ?

  1. Relationship Issues - Conflicts in relationships is a major cause of stress.
  2. Life style - Working for long hours, sedentary life style, unhealthy food habits, along with insatiable urge for money causes a lot of stress.
  3. High expectations from oneself and from others - The peer pressure, the pressure of constant deadlines builds up a huge stress. When a person has very high expectations from himself/herself without corresponding efforts, there is a fear of failure which causes lot of tension. At the same time, having unrealistic expectations from others also is a definite reason for disappointments, resulting in stress.
  4. Physical and Emotional Abuse - When a person is physically or emotionally abused and unable to defend himself/herself for various reasons, it causes frustration. This results in anger and stress.
  5. Competition - Unhealthy competitions with others create a feeling of inadequency and result in lower self esteem and sadness, thereby causing stress.
  6. Health Issues - Chronic and serious health issues cause lot of tension.
  7. Suppressed Emotions - Lack of freedom to express own emotions and feelings, amounts to stress.
  8. Lack of Assertiveness - Inability to say "No", when wanting to say so, inability to fight for one's own rights, causes tension and anger.
  9. Boredom - Lack of creative/interesting occupation may cause stress especially in old people or people with physical disabilities.
  10. Low self esteem, feeling hopeless and helpless - If a person has low self esteem and does not value oneself, it causes lots of negativity and stress.

Why stress management is important?
Body and mind affect each other's functioning. When the mind is stressed out, it gets reflected in the body in the form of high blood pressure, high sugar levels, indigestion, acidity, heartburn, ulcers, skin rashes, headache, nausea, cancer, eating disorders, psychosomatic pains etc. It may trigger asthma, migraine etc.

Because of these harmful effects of stress, the body's capacity to function reduces. At times, it may cause serious threat to one's life. Hence, it is very important to learn to relieve and also to manage stress, in order to have a long and healthy life.

There are a number of ways to relieve stress:

  1. Sleep, take rest, relax.
  2. Take a break from daily routine, do something different, take a short vacation.
  3. Go for a walk/jog/run/cycling. Practice Yoga, meditation, deep muscle relaxation, etc.
  4. Spend time with friends and family.
  5. Play with children.
  6. Spend time with pets.
  7. Pursue a hobby like reading, gardening, painting, music, dance, singing, cooking, etc.
  8. Recreation - Watch a movie, play, picnic, go for a drive or shopping.
  9. Pamper yourself with beauty treatments, massage, or your favourite food/drinks.

How to manage Stress?
Stress is inevitable part of life now. Right from a kid going to Pre-Nursery school to senior citizens staying alone at homes and having severe health problems; all are under lot of stress. No one can avoid stress. But there are certain ways to manage your stress-

  1. Self-awareness. Learn more about your own self. Understand what induces stress in you, what tasks are difficult for you, what situations are difficult for you and then try to figure out the reasons why it is so. Try to resolve or modify your thoughts and attitude.
  2. Identify stressors and explore options. Once you identify what is causing your stress, try to find more options to resolve your issues.
  3. Reasonable Expectations and Feasible Goals. Have reasonable expectations from yourself and from others. Have goals which are possible for you to achieve.
  4. Learn assertive techniques. Learn to express your feelings, learn to fight for your own rights, understand and respect other's rights.

Try to minimise stress in life and lead a happy healthy life.

What you are going through is greaving. Loosing loved one is not easy to deal with emotionally. I am sorry for your loss. When you are greaving, your mind is continuously engaged in sad and disturbing thoughts and feeling of loss and perhaps guilt. When sadness over powers you it is difficult to feel any other feelings. Even feeling happy makes you feel guilty and uneasy. Slow but effective way to over cone sadness is to engage yourself in some neutral activities such as cleaning. Walking or readjng a light book. Once you try that for few days you may be ready for following things. There are few things I would like you to try. 1. When you feel sad. Feel the feeling of sadness fully. You may close your eyes and let that sadness flow through your mind and body. If tears flow let them flow. Cry it out. Feeling what you feel fully and totally is healing. Once you feel your sadness fully. You might feel the lightness in your heart. Feel that too and say out loud. How wonderful! and engage your mind remembering some good qualities and good times you spent with the person you are missing. 2. Regular simple breath awareness meditation also help manage your loss. 3. You may also l write daily 3 to 5 things you are grateful to every morning. These things could be as simple as having a bed to sleep and food to eat. Affirm what you write and add new thing in your list for 7 to10 days. If you are not feeling any change over next 2 weeks or if you lack motivation to initiate and maintain above activities feel free contact me and we could work together for few weeks to months to reverse your cycle and help you appreciate what you have. Hope this helps wishing you very best.

You sound like a depressed person from anger turned inwards. Your withdrawal from people is quite typical of a person who is depressed. Suicide is no solution and is a cowardly act. Like is very precious and must be looked after very well. You may be suffering from the rejection issue and sometimes, out of frustration, you will reject yourself in the form of suicide. When depressed suicide will look like an option but believe me it is not. You get only one chance in this life and you must make the most of it. If you think that you cannot do anything with the anger (which incidentally is not true, but you believe so), then you will tend to turn the anger inwards toward yourself and blame yourself, and start to beat yourself black and blue. Anger, being energy, and when turned around will stifle your natural energy to make you feel lethargic and listless. Can you identify with the anger? If so, express it to someone, preferably to a counselor. Once this anger comes out, it will seem like a heavy weight has been lifted off of your back and your mood will return to normal. Don’t submit to it. If you find the fight is too much, then go for counseling first and then for medication. The combination is ideal to combat this condition. This is the only condition were you are required to literally do the opposite of what you feel like doing. Depression makes you want to withdraw, lie down, sleep, become lethargic, slow down, make you too lazy to even think etc. So when I suggested doing the opposite, you will need to fight this disease. You must become active; stay upright during the daylight time; meet people; never sleep during the day, and wake up by 6 am every day, play some active games, especially contact games, do physical exercises, talk to people and join some social clubs, eat more of proteins and vegetables, attend Yoga classes etc. Watch a lot of sitcoms on TV or comedies and cheer yourself up. Go for excursions in groups, for outings, camps, conferences, and religious conventions. Get a pet dog and spend time training it, exercising it and relating to it. Expose yourself to some sunlight every day, at least 30 minutes but not in the scotching heat. I hope you can find someone to love and be loved too, if you are not already married. Whatever happens please incorporate these three important adaptations in your life: always be responsible, be respectful, and be functional. If you did these three things, lots of things will go well in life. Please pray and have faith in God to alleviate your sufferings. Do not live with lies because you will feel guilty and the depression will continue to plague you. Face the truth, always.
