I am 60 years, diabetic for five years, on Metaforte-500mg, 1-0-1, I do pranayama, sudarshan Kriya yoga, morning my breakfast will be invariably protein shake (herbalife) and my sugar is under control (98 -110) fasting and 140-150 PPBS. HBA1c is 6.5. My problem is foot neuropathy, there is no total numbness, but lot of talking and burning sensation. My doctor says HBA1c should be brought down to 6. My BP will vary from 135/90 to 140/95. Please suggest some remedy for foot neuropathy.

Vitamin b will definitely help in food neuropathy. Are you a member of herbal life? with diabetes you have to take more fibre, protein, variety of veg, 3-4 fruit servings in a day but in moderate amounts because of diabetes, very sweet fruits like banana, mango, sitaphal, cheeku etc are restricted. Eat dry fruits especially wall nuts & flex seeds for omega 3 & sprouts for fibre etc. In diabetes there are multiple nutrient deficiencies and food cravings. To correct them and feel better, you must take some dietary supplements as diabetic people have more nutritional needs. For in-depth understanding, planning optimum diet to deal with deficiencies & other concerns with weight gain

Ignorance to hearing loss z nt good. Most of the time sensorineural hearing loss are irreversible. So go for hearing test./. And share the reports with me as soon as possible puretone audiometry impedance audiometry n most important do not ignore.

PILLOWS AND MATRESSES.

“My neck pain started two days ago, Immediately I removed my pillow”

This is the very usual dialogue that I hear in my daily practice from my patients who are suffering from neck pain. Is pillow that important factor in causing neck pain or in relieving neck pain?? Let us know the facts.

Neck pain can be caused because of various reasons. Poor neck posture is one of the reasons for neck pain. So, what should be your sleeping posture? How can a pillow alter your normal sleeping posture?

Body needs rest and sleep gives our body rest so that body and mind work efficiently. It is very important that we maintain a normal sleeping posture so that muscles also get rest.

Ideally,

  • Spine has to be straight
  • Neck has to be in line with the body.
  • It is better to avoid sleeping on tummy, as you will be forced to turn your neck to one side to facilitate breathing. Whole length of your sleep is spent with the neck turned to one side, out of alignment with the body, forcing the brain to sense something wrong and you wake up with a stiff neck. Pillow has no role here!!!

So, what kind of pillow can be used?

  • Pillow can neither be too hard as it doesn’t absorb the shock/ weight of head, nor it can be too soft as the weight of the head can depress the pillow down and might take neck too low than body.
  • Ideally the pillow has to regain its original shape faster once you wake up from lying position. If a pillow takes too long to come back to its original shape or doesn’t come at all or remains in the same shape, its time for you to change your pillow.
  • If you are lying on your back, its better to use a thin pillow which supports your head and neck. Please do not use a thick pillow in this position, as it may raise or bring your head and neck forward or higher than body causing stress to your spine.

Not using a pillow when you lie down in this position can take your neck backwards and might cause trouble breathing.

Recommended to use a thin/ lighter pillow when you lie down on your back.

  • If you are lying on your side (right or left), its better to go with a thicker pillow. Ideally the pillow should be as thick as the distance between your shoulder and neck, so that when you lie down on your side the neck doesn’t go down or go up and stays in line with the body. Pillow has to be used!!

Recommended to use a thicker pillow when lying on side.

Pillow is not a major cause of neck pain. Its always better to use a pillow than not using at all. Removing the pillow the moment neck pain starts is definitely not a good solution for your neck pain. Chose the right pillow and more importantly consult your Physiotherapist.

Mattress:

As with the pillow for neck pain, mattress is not a major cause for back pain.

  • Mattress should not be too hard, should not be too soft either.
  • If it is too soft, the broader areas of our body like the hips and shoulders, might press the mattress down thereby reducing the support for other body parts, which strains them as they do not get proper support.
  • If its too hard, the mattress wont be able to support the various natural curvatures of our body (broader and narrower areas), forcing the muscles to work more and get strained.
  • Its better to change a sacked mattress or too depressed mattress.
  • Mattress should be uniform in contour.
  • Lying bare on floor wont work!!!!

And more importantly consult your Physiotherapist for back related issues.

World Alzheimer’s Day is celebrated on the 21st of September each year. A number of organizations, from all over the world concentrate on raising the awareness about Alzheimer’s disease and dementia. Alzheimer’s disease is a very common type of dementia. It falls under the group of disorders in which mental functioning gets impaired.

Life for an Alzheimer’s patients is quite difficult, but by following the following tips, you can support yourself and will also help you to cope with it. 

  1. You have to accept the changes and accept that life will continue getting difficult. Instead of trying to cover your difficulties for protection from embarrassment, you should face the fact. You also have to accept the changes in your physical abilities and adapt to skills, which are beneficial.
  2. You should develop some ideal coping strategies. Try to be occupied and involved and respond to the challenges you face in everyday life. By practicing the coping skills you will be able to gain a sense of control in your life.
  3. Try to identify stuff. Make a list of all the tasks which seem challenging to you and try to develop coping strategies for these tasks. Prioritize your tasks and determine whether it is really necessary to overcome certain tasks. You should also strategize solutions and come up with solutions which would suit you best.
  4. You should set realistic goals and expectations for yourself and use the coping skills to deal with difficult tasks. In case of very challenging tasks, do not hesitate to take help.
  5. A daily routine or plan is required for keeping a track of several tasks. A schedule reduces the amount of time you spend to figure out something, simplifying your task and also prevents mistakes.
  6. Always approach one task at a time and take enough time to complete tasks. Do not give up on challenging tasks and take breaks to figure things out.
  7. Always remember that you have more than a single chance to solve problems. If one chance fails, you should adopt new strategies and try again and again with better assessment.
  8. You should identify the triggers which cause anxiety and stress. Knowing the causes will allow you to plan in advance so that you can prevent them.
  9. Keep in mind that your family, friends, pets, will power and prayers are sources of great strength and will help you get past any obstacle.
  10. You should accept help from others and do not hesitate or think about being dependent on others.

You should take the advice from your psychiatrist or from Alzheimer Association National Early-Stage Advisors in order to understand the challenges such as medications, management of schedules and dealing with emotional or relationship changes.

Understanding The Traumatic Brain Injury Better!

Traumatic brain injury (brain injury triggered by an external event such as a fall or a road accident) can be quite a harrowing experience for the patient and their loved ones. While most people recover from the injury, the duration and extent of recovery may vary from individual to individual.

Some patients may simply suffer from a temporary amnesia, while others may remain comatose for months. The recovery time and extent of neurological deficits in traumatic brain injury is determined by the severity and location of the brain injury. The traumatic brain injury can be

Open or closed

  • Open injury: There is an opening up of the skin and other layers leading to direct contamination of the brain from external debris and the risk of brain infection is high in such a case.
  • Closed injury: here, the brain is not contaminated, but the impact may cause various degrees of injury to parts of the brain.

Primary or secondary injury:

Primary injury: it is damage caused to the brain at the time of the injury. The damage can be

  • A crack or fracture in the skull.
  • Bleeding into the layers between skull and brain, like a EDH (Extra Dural Hematoma), SDH (Subdural Hematoma) or SAH (Sub-Arachnoid hemorrhage). These are potential emergencies, especially an EDH, where timely Neurosurgical intervention can decide between life and death.
  • Due to violent shaking of the brain inside the skull cavity, there can be a damages to the entire brain including the axons and neurons without any visible hemorrhage on CT scans (Diffuse Axonal Injury). The outcome can be quiet unpredictable. It is dangerous if the brain starts swelling up excessively after a DAI.
  • A person can also suffer from brain hemorrhages and contusions within the brain matter. Large contusions may need surgical evacuation to control brain pressure. Smaller ones are treated without surgery. Deep seated contusions towards the centre of the brain cause more morbidity even if small in size.

The secondary brain injury may develop gradually (within few hours or days from the primary brain injury), further aggravating the brain tissues. This is brought about by inflammation, altered blood supply, high brain pressure, brain edema or swelling, and programmed cell death. Sometimes, even though the primary insult is small, patients may succumb due to secondary injuries which are highly unpredictable and difficult to treat.

Estimating the extent of the brain damage

The damage inflicted to the brain is assessed by the combined assessment of the following:

1. The mode of injury, especially velocity of impact.

2. How long the person has been unconscious.

3. How bad are the visible external injuries. Is it an open or closed injury?

4. Whether there was any convulsions, vomiting, ear or nose bleeding?

5. Assessment of eye opening, verbal response and movement. This is assessed and recorded by the GCS (Glasgow coma scale).A score of 15 implies the person is fully conscious, a score of 9 indicates the person is in a semi-alert state, while a score of 3 indicates a deep state of coma.

6. Brain imaging through CT or MRI. This gives a more direct information about the physical damages to the brain, and guides the decision about Neurosurgical intervention.

Treatment of traumatic brain injury:

  • Medical : medicines are administered to control brain pressure and limit the secondary injuries. It also attempts to control events like convulsions and infections.
  • Surgical: surgery becomes necessary when there are depressed fractures impinging into the brain, or large hematoma or brain swelling causing rise of brain pressure or a shift of the brain to one side. Other indications are uncontrolled CSF (cerebrospinal fluid) leakage from nose. Sometimes shunts need to be put to drain out CSF in hydrocephalus (accumulation of excess water in the brain ventricles). During surgery, parts of the skull may need to be removed (Craniectomy) temporarily to allow brain expansion and control brain pressure.
  • Accessory surgeries: smaller procedures like a tracheostomy (bypass of the windpipe) or PEG (stomach tube) may need to be done for long term care in comatose patients.
  • Rehabilitation: tireless care by nursing and physiotherapy are necessary to help patients recover faster and avoid complications like bed sore, deep vein thrombosis, spasticity and weakness of limbs.
  • Recovery: Recovery from traumatic brain injury may be time taking (weeks, months to even years). In addition to the initial treatment and medications, patients may need Brain Injury Rehabilitation. The rehabilitation aims at enabling the person to lead a normal life as early as possible. There are medications, activities, and exercises to improve one's cognitive power.

According to a recent study conducted, researchers found that a healthy lifestyle can help fight dementia. As physical activity will help in stimulating the blood circulation in areas that are at risk. 

Alice Hollamby, study’s lead author stated, "We understand that living with dementia poses many challenges to individuals and their families and the idea of improving their physical fitness may seem like an unachievable target."

And, Dr. Eddy J Davelaar, study co-author, mentioned, "We all know we should embrace a healthy lifestyle to strengthen our physical and mental wellbeing. However, this is not to say that when one develops dementia, all hope is lost."

Also, as per previously conducted studies, it was found that physical fitness may help prevent dementia, the current study further showed that it can ward off further memory loss in patients suffering from it.
