you can switch over to warfarin 5 mg tab twice daily . minimum duration of therapy is 3 months. you need to check PT and INR regularly to keep it within 2-3. thanx.

Actually it depends on the conditions for which you r taking, acitrom has better anticoagulant effect which is required when you have valvular problem+atrial fibrillation and in that case we have to ignore side effect also.
On the other hand xarelto does not require monitoring and has less side effect.
Anticoagulant effect if you ask acitrom >xarelto
it depend upon condition for which you r taking .tell in detail condition so that I can guide you better.

Xarelto is a newer oral anti coagulant, specifically it is anti factor xa. What was the reason for which xarelto was prescribed because it is now recommended for heart and peripheral vascular problems and has also been used for dvt. It will be best to seek another consultant for additional information in person but best to follow an expert.

Xarelto dosing can be adjusted if there are kidney problems but certainly not contraindicated. However, exact dosing needs to be calculated based on his creatinine clearance (a measurement used to define kidney function). Since your creatinine is at higher level inform your doctor, xarelto is unsafe to use in renal desease. Do not use analgesics along with this, increases the risk of bleeding.

Usually bleeding stops. But take care that the blood clots inside canal should be cleaned. You can clean ear canal with ear bud dipped in betadine liquid.
