Hello. If you have arthritis since childhood, it would be Juvenile idiopathic arthritis (jia). It needs to be treated with same drugs as for adult arthritis. You are doing wrong by taking steroids for prolonged periods without any other disease modifying drug. Steroids should be taken only as a bridge therapy till the effect of other potent arthritis drugs sets in. So see a Rheumatologist urgently and take proper medicines.

Hi as you know the diagnosis, you need to be seen by rheumatologist. As the medications needs regular reviews because of steroids. Better to keep regular follow ups. Rheumatoid arthritis is life time problem, unfortunately it needs patience and regular checkups. Vitamin d and calcium supplements need to take along with your medications.

First of all it is important to know the cause of macular edema. You have not mentioned that in you query. Second important factor is type of macular edema and cause of its persistence. Only giving anti vegf injection does not cure macular edema. One needs to find out the cause behind it and that should be targeted. Also nepafenac is not needed after anti vegf injections. Nephrotic syndrome usually minimal change disease which are common in children usually relapse whenever there is any sought of infection.
You can go in for stress dose steroid, that means whenever you feel he is developing some throat or chest infection, give full dose steroid as per his wt (40-60 mg of prednisolone) for 3 days, so that he tide over that crisis period.
Still relapses are common and part and parcel of this disease.
