Vertigo can be central or peripheral. Central cause due to occlusion of blood vessels of brain. Peripheral cause usually due to ear problem. Vertigo on lying down, mild and prolonged vertigo can be central type especially if there is underlying diabetes and hypertension.

Please consult an experienced physician. Cervical spondylosis rarely causes headache. Sleeping of feet could be because of neuralgic pain, may be due to vitamin b1, b6,& b12 deficiency or diabetes mellitus or orthopaedic cause in back or neck. Please exclude high blood pressure also besides diabetes and thyroid dysfunction.

bppv can be treated with epleys manoeuvre and vestibular rehabilitation exercises.
With regular vestibular exercise you can be symptom free and lead a normal life.
Also if you still ha e symptoms with your current dose of vertin the dose will gs e to be adjusted.
*tips for starting vestibular exercise:
no. 1: sit on your bed with legs hanging.
Keep your head straight and just by moving your pupils first look to the right. Hold this position. Count to 10.more

Avoid lifting heavy weights
if not relieved by medications and rest. Next options are applying head traction to relieve compression at the indenting site. And later if still it persists you may require a steroid infiltration after seeing the mri.
