" hi there, I am a medical doctor myself and an mbbs graduate, who is preparing for neet-pg myself, so do not have much time rather two months left before exam ,so this is highly directed towards only senior psychologists (preferred) and senior psychiatrists, I have a psychiatric history of using medication since 4 years and right now I am having occasional crying spells and inability to feel like reading which I used to enjoy before although I am put on bupropion 300 mg and lamotrigine 200 mg and amisulpride 100 mg and alprazolam 0.5 mg ,sleeping meds are just to counteract modafinil which I take them occasionally to decrease fatigue and increase motivation to study, I will be thankful if someone who is well experienced can provide me with counselling to decrease to cope up with stress and crying spells and any one who can alter medication to the circumstance I am in right now can just give a suggestion, I want to a consult with a psychologist or a psychiatrist and I do not have enough time to maintain a mood journal so any coping skills within short time against crying spells can just drop in a suggestion ,i want to take a consult, thank u.

Unfortunately, therapy cannot be hastened in any manner but it is possible to bring about some functionality to begin with and gradually you can develop good coping skills and recovery over time. You need to consult a psychiatrist for the medication part and follow some therapy along with it. Hello, 10 year ho gye is anxiety and depression ko. Day by day aur kharab ho rha. Aisa lgta hai ki din kam bche h mere lekin suicidal thoughts nahi aate wo nahi krna. Daily jindagi ko bojh ki trh jita hu mai. Pura din soye soye nikal deta hu. Kaam krta hu office me but na hi mujhe khali baithna psnd na hi job krna. Koi bhi job me man ni lgta. Dosto ya ghrwalo se baat krne ka bhi mn ni hota infact mujhe yaad hi ni rhta call krna. Saara kaam hadbadi me krta hu. Agr call bhi krta hu office me client ko to bina dhang se baat kiye 2-3 min me nipta deta hu call. Dher saari chize puchna bhul jata hu. Hmesha overeating krta hu aur itni tezi se khata hu ki haath rukte ni. Job ki soch ke bechaini hoti rhti. Koi bhi kaam me confidence ni h mera. Kya krna h life me, kaun si job krni, kis chiz ke layak hu kuch ni pta. Kuch bhi krne ka mood ni. Engagement tod chuka hu 2 baar wo bhi an instant pr jb 1 month rh gye the shadi ko. Koi feeling ni h mere andar. Kisi ko bhi khone se dar lgta. Dwa chl rhi anxiety ki 9 months se pr kuch khas frk ni pda. Every month 7000 rs ke aas pas dwa aur appointment me chle jate. Phir bhi manage kr rha. But ab himmat haar rha. Kahi phir se dwa leni band na krdu kyuki aisa 2 baar hua h jb maine doctor ki dwa bnd kr di ho. Ek psychologist se session chl rha tha. 2 session attend kiya uske baad ni gya. Ek saath psychiatrist aur psychologist ka financially manage ni ho pa rha. Gharwalo ko ye bimari ni lgti unke hisab se kuch ni hota ye sb. Mt socho. Itna to mujhe bhi pta h ki nahi sochna negative pr rok ni paa rha. Day to day life me struggle kr rha mai. Any suggestion. 4-5 baar medicine change ho chuki. Abhi filhal venpower 100, unidep 300, betapro, tofisern 50, altonil,sove 5 chl rhi. 2 din phle dikhaya doctor ko to bole ki ek baar doctor change krke dekho ho skta h approach me dikkt ho mere. Hi I am suffering from social anxiety disorder since long. My psychiatrist tried different combination of medicines but nothing worked notable. Finally, Bupron XL 300 and Panazep (25/0.5 mg) combination worked for me the most. I am taking this combination of these medicines since January 2015 and regularly since October 2015. Recently I gone through regular medical checkup and found elevated liver enzymes - Aspartate Aminotransferase (AST) and Alanine Aminotransferase (ALT), in my blood stream. I am diagnosed with no signs of Hepatitis (A/B/C), though a bit fatty liver and a small Kidney stone. I am a healthy person with no physical signs of any liver issue. I am developed little fat in my stomach in last couple of years but feel healthy. I don't drink, smoke and am pure vegetarian. I eat home-made healthy food mostly with balanced nutrition diet. I want to understand the possible causes behind my elevated liver enzymes and what could be the possible results of this situation? Is it happening due to my long-term usage of anti-anxiety medication. Do I need to stop my anti-anxiety medication? If I do so, I would be in big trouble as this will directly affect my social and working life. If this is necessary to stop medication, what are possible alternatives for me to control my social-anxiety? Please help and advise. I am worried as I have a family dependent on me only. Thanks in advance. Medicines: Bupron- Bupropion Hydrochloride Modified Release Tablets Panazep- Paroxetine Hydrochloride IP 25 mg + Clonazepam IP 0.5 mg Dosages: 1 Bupron + 1 Panazep daily in the morning with empty stomach before leaving to office Elevated Liver Enzymes: Aspartate Aminotransferase (AST or SGOT) as 169 U/L Alanine Aminotransferase (ALT or SGPT) as 113 U/L.

I am shaik ismail from visakhapatnam I think I misdiagnosed as schizophrenia .it han been 3 years when it started .before 4 years I am normal and im school topper used to over study in the mid of night and severely sleep deprived .next after few days I became calm not talking to anyone habituated for overstudy and some problem started like reduced functioning of brain and sometimes felt like brain stopped functioning in the middle of time upto 30 minutes became very worried not able to read hand shivering while writing and again started functioning .marks severely decreased .then consulted psychiatrist gave amisulpride then after public exams started risperidone &amisulpride ,brain functioning slowly got reduced, one day forgot to take tablets. Then suddenly everything stopped ,my feelings gone ,cognitive ability gone, thoughts stopped, memory reduced, imagination stopped, cannot understand books, this continued upto three years now. I tried all antipsychotic, antidepressants,ect ,combinations nothing worked. I think I am misdiagnosed .can you tell me what I have .i think this is neuroleptic induced deficit syndrome .now what to do .also clozapine monotherapy please tell me whether I am misdiagnosed ,i have negative symptoms of schizophrenia (doctor said. Is this neuroleptic induced deficit syndrome. Or schizophrenia I also think I have dopamine deficiency. I used risperidone, olanzapine,clozapine, quetiapine,lurasidone, blonanserin,amisulpride, ziprasidone,aripiprazole, and ssri ,snri, bupropion many more nothing worked. I am not even going to college because I am notable to study. I have compulsive buying disorder .buying costly items at cheap rates .getting scammed online frequently. I even have nerve problem in the neck linked to my concentration and reading. Difficulty reading, no memory, anhedonia ,poverty of speech ,poor thinking, I failed intermediate education.

Dear Shaik,
All your symptoms are symptoms of schizophrenia. In schizophrenia, there is dopamine deficiency. All the tablets that you have taken are to correct that deficiency. May be because of the tablets you don't have positive symptoms. Negative symptoms need to be treated with psychosocial methods. Unfortunately you may be having treatment resistant schizophrenia. You are probably suffering from nicotine withdrawal. Take nicotine gums or patches which are sold over the counter. Bupropion is a good medicine for nicotine withdrawal but you need prescription for this. Consult a doctor and get prescription.

Dear Manish,
You know cigarettes are the reason for foul smell. Get rid of both with continuous motivation to quit cigarettes and nicotine gum or bupropion tablets if necessary.
