Actually you have not mentioned age of the patient,duration of diabetes,hypertension,and any other complication.Patient must be investigated for cardiac status,any renal complication also.2D echo is must to rule out cardiac failure.Xray chest for lungs and bronchial tree and cardiomegaly.Hypertension will cause heart complication.So in short,go for complete cardiac work up.

Regular stretching and strengthening of leg muscles should be done so that you will not face any kind joint pain problem and soreness.
