Your father have hypertension. Hypertension over long term can cause secondary complications. You should get some workup done atleast once a year. Cbc, rft, rbs, ecg, echo, fundus, these are some bare minimum. If you had it done you can send me the reports, I will review. Now for treatment, telmisartan is really good drug. Anemia can cause swelling and dyspnea but a chest x-ray, pulmonary function test and echocardiography will help in diagnosis.

If your bp is fluctuating so much then better get an abpm (ambulatory bp monitoring) done to know the stage of hypertension which will guide us about further treatment.

Start bp medications only after consulting your treating physician, salt restriction and lifestyle modification. Proper evaluation is required whether hypertension is essential or secondary. Ecg/ echo/ usg whole abdomen/ urine routine/ vasculitis profile. Rule out diabetes and hypothyroidism. Anemia should be evaluated if persisting. Rule out sleep disordered breathing if complaints are there.
