Neurologist is not at all related to your brother's condition. Taking MRI brain once is not wrong, though it rarely shows some findings. You can consult your psychiatrist and ask for counselling from a clinical psychologist. All the best.

Lithium level is normal only, so lithium can be continued if other blood parameters are fine. On your mood symptoms discuss the lack of improvement with your treating psychiatrist, and try alternate medications if needed. Being open about no improvement will lead to the doctor taking note and doing away with medications that do not work. All the best.
