The prescription is fine. What is important is to see that blood glucose should be brought down to FBG: <100 mg, PP: 150 to 160 mg & HbA1c% <6.8 (even <6%). That will help remain fit and healthy. Thanks.

Glycomet only has metformin. No glimepiride. Both the drugs good for diabetes. None of the drug cause kidney problem. In kidney problem we take some special care in prescribing metformin.

At this young age and of an athletic, you can be fittest person if you decide to switch to herbal support of DM. I will not say to stop your current med now. Dear Lybrate user, glimepiride stimulates betacells to secrete insulin. Metformin is supposed to reduce insulin resistance and slow down glucose absorption from the intestine. Usually both these are used together to control increased blood glucose. In newly diagnosed patients metformin alone is used. In some both drugs are used together. Which drug to take for a particular level can be decided by the treating doctor.

Mr metformin can be taken up to 2500 mg per day. But 100 mg is also a high dose only. If it is not causing any gastro intestinal side effects then no problem. It is really good that you have started on insulin because looking at amount of drugs that you are taking, it is necessary for you to shift to insulin. By doing self monitoring of blood sugar daily you can increase the insulin dosage every alternate day till you reach target fasting level. It is good to reduce or take away pioglitazone.

The glucose levels mentioned are definitely high. It appears that present (new) treatment is not suiting you. Since no details are mentioned in terms of drugs and dosage, it is difficult to comment. If the previous treatment was giving good results with better glucose control, what was the reason for changing?
