42, gender: male hello, I had an anxiety and insomnia issue in early june triggered by job loss. A physiatrist I saw prescribed xanax -xr 1 mg everyday after dinner and 0.25 mg as needed. Although I got another job right after, anxiety remained and is triggered easily by work interactions for many weeks. Medication did help with my anxiety and insomnia. I don’t have as much anxiety issue anymore but whenever I try to taper of xanax, I cannot sleep. When I asked him again, he said keep taking xanax -xr for three months as he thinks different medications will have other side effects. I took a second opinion and the other Dr. told me to taper off xanax and use trazodone 50 mg everyday. 1 mg xanax is definitely helping me with sleep but I am apprehensive about switching drugs especially the first Dr. was emphatic about staying on xanax. Since the two opinions are conflicting, I feel like I need a third opinion to resolve this, as I am very uncomfortable staying on xanax but feel like tapering off it is not an option given my inability to sleep. A mild anxiety that I sometime have during the work days is manageable I feel like I don’t need medication for it. I would like your opinion on whether I continue on xanax or try to taper off and start taking trazodone 50 mg. I am also planning to have sessions with therapists also.

I am 37 years old men from gujarat. I did m.pharm and was a lecturer in pharmacy college at Gujarat. When I was talking my lecture on 10/03/2010 suddenly I felt fear n hallucinating condition and I diagnosed at that time with a paranoid schizophrenia n I was on combinations of medicine like antipsychotic n all but after that my doctor has told your disorder has been concerted to bipolar disorder n right now I am on lexpin 25 mg, lithium bicarbonate 450 mg, divalproex sodium 500 mg, nortriptyline 50 mg, trazodone 100 mg, fluoxetine 20 mg, nitrazepam 5 mg, trihexyphenidyl 2 mg on daily basis at night n this all medicines I am taking regularly although I am getting 2 to 3 episodes per year. Right now I had tried brintellix 10 mg for a month but I am getting depressed so I had stopped right now brintellix. Actually I am feeling tired from this condition n getting suicidal thoughts, lack of confidence, worthlessness n getting angry easily n feeling alone. Really I am feeling helplessness. Please can anyone tell me whether my treatment going is on a right way or not. Also I had taken 4.4.2 ect. And right now I am thinking about ultra brief pulse therapy n rtms but getting confused about this because this requires more sessions so financially also it will goes more. But ya if it gives a benefits to my conditions then I am ready to go ahead. I had taken many advice from various doctors in gujarat. Today I approximately passed 10 years with this illness. So please can anyone suggest me a right way. Thanks.

I think a complete course of ects with medication will make you feel better. If affordability is an issue am sure there are good medical colleges nearby where a psychiatry department can treat you at affordable cost of treatment.

Hello there, This is Nasrat, 29 years old, I`m suffering form depression since last four years while I have last my beloved brother. So, I took different kinds of medications such as fluoxetine 20 mg, escitalopram 10 mg, paroxetine 10 mg, mirtazapine USP 15 mg, venlafaxine Hcl 75 mg, Lithium carbonate B.P. 400 mg etc. But still I have the same problem, difficulty in thinking (poor concentration, poor memory), excessive feel of guilt, loss of interest in most activities, sleep disturbances, decreased energy or tiredness, feeling of low self confidence, feeling of hopelessness, decrease in activity, excessive or inappropriate worries, anticipation of the worst, thoughts of suicide. Besides, I have premature ejaculation and no sexual desire. So, I`m really disappointed. Are there any solution for my problems? Its mentionable that when I was in Japan, I took Lorazepam (0, 5 mg), Brotizlolam (0, 25 mg), Trazodone (25 mg), Suvorexant (20 mg), Escitalopram (10 mg), Lansoprazole (15 mg) for 8 months, with those medications I felt very well, but unfortunately, when I returned to my country, there were no those medications available, and my depression attacked me again. So, dear doctors Please please please recommend me what to do? I`m not interested to live in this world but, I`m the only person to support my big family. At the same time, I took (Clarithromycin 500 mg, Amoxicillin 1000 mg 14 days), (Lansoprazole 30 mg 3 months) for positive h pylori but did not work. Again, I took (Clarithromycin 500 mg, metronidazole, Amoxicillin 1000 mg 14 days), (esomeprazole 40 mg 3 months) for positive h pylori but still I have the same issue. You need a proper diagnosis before going in for treatment. If h pylori is confirmed, it needs active treatment. Premature ejaculation and loss of sexual desire also needs to be diagnosed and treated. At Japan, you were treated with Lorazepam (available in India). Instead of brotizolam, you can try etizolam available in India. Trazodone available in India. Suvorexant is another mild hypnotic. Just think of sex as normal day to day activities if you over thing and worry about it. You will increase stress on yourself and which will decrease your performance in sexual activity.
