Your glucose control seems to be fairly good. Ideally HbA1c% should be <7%, but 7.4 is also not bad. There appears to be no need to change any medication. However, looking at your weight and height, it is clear that you are over weight, because at your age ideal body weight should be < 63 kgs. So if possible a bit of additional physical activity and better diet control should be practiced. Thanks.

If you're having ckd hba1c is not a good option to assess your sugar control. I would suggest you shift to a basal bolus regime with a continuous glucose monitoring device or a glucometer readings at least 5 times a day. Please consult an endocrinologist.

Due to his increased Creatinine, he required modifications in treatment. Trajenta is safe for kidney patients. That's why it was added along with decreasing dose of metformin, which was higher in glimser forte 2. Metformin is excellent drug for sugar control that's why he might feel with it, his sugars we're better, But seeing his age and Creatinine status, higher dose of metformin is harmful in longer duration.

Both fasting and Pp blood glucose levels mentioned are very high. It appears that present medication (particularly Insulin) is not adequate. It may have to be increased. Plus a look has to be given to his diet and extent of daily physical activity. You may have to first target his fasting level and bring it down to 110 mg or so by adjusting nigh insulin dosage.
